This was a talk given in The DermaMeDeCon conference held in Dubai in May 2023. It gives an updated review on hoe oral minoxidil can be given for a variety of conditions that involve the hair.

    Hello I'm a 40 Years old man .In 2020 I went to a cruise for 7 days , I didn't protect my scalp from the sun and it burned it was very painful. Severe sun exposure .Important to mention before the cruise my hair was perfect and healthy no shedding or thinning. Next 20 days or month it started shedding like crazy all at once . My hair started growing again but thin , weak and short. It's been 4 years I doesnt Shed anymore but it's thin and it doesn't go longer than a certain point , it's like if it was stuck not growing beyond a certain point compared to the other areas of the scalp that didn't suffer , so I can see my scalp.it's very frustrating because before that event my hair was perfect and it hasn't been recovered fully .even though I have hair it's thin after that . I'm taking oral Minoxidil ,oral Finasteride, vitamins and having mesotherapy since last week . Can I recover my density and thickness one day ?

    Hello, Dr.Yesudian, awesome video. I wanted to ask you about what do you think on 1.25mg dose of oral minoxidil daily for male hairloss. Is 1.25mg enough? and is it as effective as using topical minoxidil twice daily?

    Thank you for your kind comment. Usually we start at 1.25mg for women and 2.5mg for men. The dose can vary as some start on lower does and other use higher doses. The literature seems to suggest that oral minoxidil is better than topical minoxidil though head to head studies are limited.     Hi doc , I’ve just been told that I have female pattern hair loss and was prescribed a topical solution called FOLIJUV which I have been religiously using for 3 months now. Haven’t seen too much difference and am still shedding quite a bit of hair. Am wondering if I can add low dose Minoxdil to the routine too. I take blood pressure meds (2.5 mg Ramipril once a day) and so am unsure if oral Minoxidil would interact with this. Would appreciate any advice. I am 59 yrs old. Would also like to say how much I love your videos. Really informative and straight to the point. Thank you so much for explaining the science so concisely and plainly for us lay people !

    Thank you for your kind words. I am not sure what ingredients the topical solution you are using contains. Oral minoxidil can be used in combination with almost any topical scalp treatment. As the doses of oral minoxidil for hair loss are very low it is unlikely to affect the blood pressure. However, it may be useful to monitor blood pressure for the cfirts couple of weeks to ensure that it is stable and the body has got used to it.

    this isnt related to the video but to a different video on body odour, i have a musty body odour much like the people you usually find on buses and people who are categorised as 'tramps', how do i get rid of this body odour. i have showered with antibacterial soap, i exfoliate almost every day because my dead skin cells grow back very fast for some reason. is it because of the humid weather in the uk, the clothes im wearing, bed and pillow sheets, food that i eat, or something else. every time i shower the smell from my shampoo and body wash doesn't seem to stick to my body for very long and they disappear very quickly, i have a trick i use to smell my musty body odour though it doesn't always work it only works sometimes, so i know i have it and i sometimes smell it directly after i shower. could it be the towel im using or something else. also the sebaceous glands that release that white stuff (not acne pimples) also smell musty or foul, stale or something like that, could it be because of them that i smell weird, its like there is another layer of odour that i cant get rid of. any tips, help or directions would help.

    Hello Dr. I have trichtillomania (since I was 5 now 30). I had some recent success with not pulling (using ACV shampoo and NAC). My hair grows very slow and thin in affected areas (high pull areas). Can I use oral minoxidil for a boost ? And would I have to use it forever or the new growth will fall following cessation?     Oral Minoxidil seems to be effective in treating hair loss of almost any reason. If the underlying condition like trichotillomania settles then oral minoxidil can be stopped.     Hello doc First of all thanks for thanks providing us such a informative videos. I had some doubts Im male 21,I do have nice density when im 18,19. Now my hair is thick, but not that much density which I got in 18,19 (im not bald,might be 0.5 noorwood).My question is I do have thick hair nd I shower daily nd I loose 10 to 15 hair while washing, for example if i loose 10 hairs in which 3 will be e slightly thin,others will be thicker, am I suffering from MPB ? Even though I loosing my thick hair, is this MPB or some other hairloss. Does loosing 10 to 15 strands of hair during hair wash daily is normal? Should I worry about that?

    Loosing 10 to 15hairs a day will be considered normal. Here is a video that analyses how we all loose hair throughout the year. There is a gradual decrease in density of hir as we get older, and this usually occurs from our 20s to 30s. kzread.info/dash/bejne/dGd5rLuToNe-icY.html

    Any treatment for hair loss may result initially in hair shedding. It takes about 6 to 8 weeks for any treatment to stabilise hair loss.

    Sir i have been using 5% topical minoxidil for 2 yrs now and this winter i developed heavy dandruff, sort of fungus like. I had to shave my head in December to clean my scalp but then it started happening again after a week or 2 and i had the similar condition in the February and i had to shave my head again. I went to dermatologist yesterday and she prescribed me 2.5mg oral minoxidil once a day after dinner instead of topical. My concern is will my hairs shed again like they did when i started topical minoxidil or the oral minoxidil will resume the effect of topical. i also have been reading on the internet that it is less effective than topical and hairs start to become weaker as the effect of topical minoxidil wears off and you lose your hairs which topical minoxidil saved or made stronger over the course of its use? I don't know why i started to develop dandruff with topical all of a sudden as there has been no change in my daily routine or diet. Topical was working great for me and i am afraid that i will lose my hairs after switching to oral.

    Both oral and topical minoxidil are effective in preventing hair loss and switch from one to another is commonly used by dermatologists. Oral in fact is thought to be better than topical by most researchers. The dry skin in the scp can occur with topical minoxidil which is why your doctor may have changed to oral minoxidil. In the long term it may be beneficial in stimulating hair loss. Short term hair loss may occur, but in the long term it seems to be worthwile for most.

    Thanks Doc for the info. Can you explain, please, what exactly are the reasons for increasing the dose from 2.5 to 3.75 and then to 5 mg? Do all patients need to increase the dose at some stage?

    The dose increase is suggested in most studies. Trying to use the lowest dose that can increase hair density would be the best option.

    Hello Dr thanks for this informative video. i do no have a serious hair loss but i dont have hair on my scalp and hair on the from are very tiny not growing. Do you think this product is for me? Can i combine both oral and topical minoxidil? Also can i stop it once i get the expected results??

    Oral minoxidil may be helpful for any type of hair loss. However it is unlikely to bring about complete hair coverage if there is minimal hairs at present. Usually just one of the topical or oral minoxidil may be sufficient, using both is possibly not required. It can be stopped after there is a response, but there may be a recurrence once stopped.

    Thanks doctor for this informational video . Do side effects like puffy eyes and face go away after some time even when you continue the daily dose ? I have been on 5mg of oral minox a day since the last 5 months and I believe my eyes have turned a bit puffy .

    Puffy eyes are not a usual side effect of minoxidil. It may be useful to exclude other causes of puffy eyes and to have an assessment by your doctor.

    Hello Doctor, I was recently diagnosed with hypertension and have been taking telma 40mg( temlisartan) for the Same. (Every morning). Since OM also impacts blood pressure, Can I still start with oral minoxidil 2.5mg once a day in the night ? And then eventually increase it to 5mg? And if I do it what should be the time period between increasing the doses? Looking forward for your reply. Thanks

    Oral monoxide used for hair conditions is low dose and does not affect the blood pressure to any great extent, as mentioned in the video.     My personal practice is to use just one of them - either topical or oral minoxidil,. Using both is unlikely to have extra benefit.

    Thank you for the lecture I've been taking 1mg finasteride for 1 year and have seen significantly postive results however i still have some thinning in the vertex area and tempral regions. I added 3mg oral monixidil, it's been 14 weeks, I'm noticing diffuse midscalp shedding and somewhat significant shedding at tempral corner and hair line. Is this supposed to continue at this point?     Minoxidil can cause some shedding of hair when first used. If there is shedding even after the first 3 to 4 months, the it may need to have a different dose or consider stopping.

    @@RD-dp3qt All studies suggest that once stopped, there may be thinning of the hair again,. It may take a few weeks to months but sometimes the hair could persist as well.

    Hello doctor,I have been on oral minoxodil(2.5mg)for 6 months but before that i used topical minoxodil for 2.5 years.Currently,my hair fall condition is quite normal,have Good amount of thin hair and don't think i have got significant improvement by using oral minoxodil though i still use topical minoxodil sometimes to get better result.My question is,should i use topical minoxidil alongside oral minxoodil?Is there will be any potential benefit?Another thing, i want to share that Since my hair fall(Telegon efflovium) started suddenly 3 years ago,My scalp becomes very oily and that makes lots of dandruff,can't control with any shampoos.Is there any reason behind this? Give me some good suggestion to improve thin hair and can i ever stop all these medications?

    There seems to be no additional benefit to using topical and oral minoxidil. No studies have been done on out though. Just one of the preparations would probably be enough. Oily and dry skin is common and called seborrheic eczema. Most of us have it. It may settle with time by itself. Washington the hair frequently helps.     @@DiscoverDermatology Sir,though i am using finasteride and oral minoxildil for quite sometime,my hair thinning is not improved that much,just stable situation(Not satisfactory result)and besides that sometimes my hairfall stays normal and sometimes it increased.What should i do sir? what will be my future plan? should i add low level laser theraypy and any other suggestions sir?

    It deepends on the dose and the duration. Each dose is usually used for at least 4 to 6 months. If it is still ineffective, then there are other options like dutasteride. I do not have much knowledge on lasers for hair growth as we do not use it in the UK. @@johny3754

    Doctor. I would like to take 1.25mg oral mino once a day initially, but the only tablet in my country is Pfizer 10mg loniten. Is it ok to cut it into 4 parts (2.5mg) then take it every two days? or I should cut it into 8 parts(kind of difficult) Thank you!

    Im a mbbs grad from chennai sir....ive been on topical minoxidil 5% with finasteride(topical) for past year...but i see a lot of dandruff,hair thinning and hair fall happening sir ( 250 hairs per day avg)....i use kz 2% everyday leave it on for 20 mins and the dandruff returns even if i dont use it for a day.....can i go on 2.5mg oral minoxidil, 1mg finasteride oral every day....or should i use 0.5mg dutasteride sir?? Im using calcium and vitamin supplements sir.

    Usually, unless the dandruff (seborrheic eczema) is settled, the hair loss is likely to continue. The inflammatory changes in the scalp needs to be addressed. Isotretinoin has been reported to be helpful in resistant seborrhoeic eczema.     Sir I have diffused male pattern baldness. Taking fin 1mg and Minoxidil lotion 5% 1 ml in evening from 5 yr. Still hairloss. Should I shift to min tab 2.5 mg along with all medicines?

    Yes there are quite few treatments available for keloids, and the commonest is injecting steroids into the scar. I have not done any videos on it. It may be helpful to discuss options with your local dermatologist.

    Can a person with mild harmless liver condition called Gilberts syndrome take oral minoxidil and oral finasteride long term? since its metabolized in the liver does the condition have any relation with this?

    As Far as I am aware, Gilberts syndroem is not a contraindication to use any medication. It is useful to seek the attention of a general physician or the person who made the diagnosis of Gilberts syndrome.

    Hello Sir, The session is quite informative. I have a doubt- I've used Tropical minoxidil but didn't get any result and during that period there was a great hairfall too. It's been 2 yrs i stopped tropical minoxidil. If I start ORAL Minoxidil, will there be any hair loss again? Also like the study says - minoxidil once stopped, the hair starts falling again - Is it the same with oral Minoxidil also?

    The answer is yes to both questions. The hair loss can recur when started and can also recur when stooped.

    There have been reports of pericardial effusion with oral minoxidil but this is exceedingly rare. I have not seen it though I have used it for about 3 to 4 years.

    In the UK, oral Minoxidil is used more often than oral finasteride. Like minoxidil, the dose can be variable and depends on the response, 1mg every day or there times a week are both acceptable if there is a reasonable clinical response.

    Usually topical minoxidil can cause initial shedding before it stabilises. If topical minoxidil is ineffective or causes side effects, oral minoxidil is certainly an option.

    Hi, there is no good study comparing the doses of topical and oral minoxidil, so unable to answer question1. Question 2: Yes, the dose can be reduced at any time and the dosage can be tapered or increased according to the response. Question 3: Oral minoxidil has been used only for the last 2 to 3 years so we do not have long term studies yet. It looks like most require long term treatment of a few years at least.

    Thank you for the amazing video doctor. How do you recommend taking 5mg oral minoxidil. I'm 25/M I have no side effects and currently on 3.75mg oral minoxidil at night for the past 3 months with 1mg finasteride for androgenetic alopecia 1)As 2.5 mg morning and night since minoxidil has short half life OR 2) 5mg single dose at night since more of it will be available for hair after metabolism in liver Thank you

    My general advice to my patients is to take oral minoxidil as a single dose at night. It improves compliance and reduces side effects like (postural hypotension) dizziness.

    Hello sir, I'm a 27 yrs male .i have seen a dr who said i have Androgenic Alopecia and have dandruff my dr prescribed my oral minoxidil 2.5mg/ day i have seen lots ofcomments and videos where people are complaining about the side effects it has.. I'm worried if any side effects would happen then it will be temporary or permanent? and if someone stop using it then side effects will Stop permanently or not ? if we consider the serious and minor side effects both !!! Apart from that my dr also suggested Morr Pro serum and vitamin Capsules too and i feel i can't continue any treatment for a long timebecause i don't believe to take medicine for a long time for your health .Thanks sir waiting for your reply and suggestions 😊

    The pragmatic way of avoid side effects tih any treatment is to start with a low dose and slowly work our way up. This avoids side effects in most patients. The problem is if we start high and experience side effects, then we will not be keen to continue. I have hardly experienced side effects in my patients as I try to use this principle. Once the hair thicker we may be able to maintain the thickness with low doses or even stop, but no studies have been done on it yet.

    @@DiscoverDermatology Thanku sir for your reply... sir my dr is starting with low dose 2.5mg/ day and also suggested dutestride in 4 sitting but I have read about it there are some side effects of it like libido, erectile dysfunction it's not FDA approved. In India many drs don't go with this but my dr. Suggested this so can I assure myself that I will not face any kind of side effects and if any then it will stop or not if I will stop taking medicine ? Thankyou

    Dutasteride has good medical evidence of its effectiveness. Side effects can occur with any medication to any person and is very difficult to predict. The combination of oral minoxidil and dutasetride/finsatreide is also used frequently in clinical practice. #Disclaimer# Please note that this video is for education purposes.     Gud evening sir.i an diagnosed with alopecia areata nov24 three patches.topical ointments mrn and n8.tacroz 0.1 and flucort-h ointments.oral minoxidil 2.5 mg ,mtx week 1 tab 10mg,folvite 5mg, fr 1 mnth.i can see results in 1 mnth.so depressed.whether after recovery stopping medicine will my alopecia will come again.r ishould take tablet till my life ends.so depressed.nt able to mingle with my family.can u suggest to me to stay positive.mentally disturbed.help me sir... thank you...

    The treatment that is detailed is good and most dermatologists would use it. Alopecia areata is a condition that we cannot predict. It may settle tomorrow or takes a lot longer. The new JAK inhibitors are fairly effective. Here is a vide on it kzread.info/dash/bejne/gXyI27eglti7iLg.html

    Everytime i start OM i face extreme shedding of hair and my density gets drastically reduced. This makes me to stop after few weeks. And i continue with topical one.

    Thank you for your education and time. My Dr wants to put me on oral Minoxidil. I am currently in a hair vitamin which he said to continue. I’m concerned about taking OM, both the initial shedding and long term possible health effects. I’m a 46 yr old female.     Hi doctor i have a doubt. 1. I have hypothyroid and does taking oral minoxidil of 2.5mg affect my thyroid? 2. Earlier when taking thyroid medication (50mg) i faced palipitations, so doctors suggested inderal 5mg tablet, and i ended up with low BP and sometimes i was close to fainting. Will it oral minoxidil affect my heart? 3. Am i supposed to take oral minoxidil forever? Will i loose my hair if i stopped taking it? Or are we supposed to reduce the dose slowly?

    There is no evidence that minoxidil interferes with thyroid function or the heart in the doses used for hair conditions. All studies suggest that it needs to be taken long term. There are no good studies to show what happens after stopping oral minoxidil.

    If oral minoxidil causes any side effects, then it is worthwhile stopping it. However, it is very unusual to get the side effects that are mentioned. If the symptoms remain after oral minoxidil is stopped, then it is unlikely to be due to oral minoxidil.

    @@akhilkatiki3111 Generally the hair texture is not affected by minoxidil. Minoxidil makes it thicker and usually has no effect on the colour.

    As far as I am aware, there is no potential interaction and taking them after food may be ok. Starting at a lower dose is always a good option.

    @@akhilkj5206 Gilberts syndrome tends to be a harmless condition that does not impair the use of any medication. As far as I am aware there are no contraindications for the use of minoxidil, but it is worthwhile getting the opinion of your doctor or the person who made the diagnosis as well.

    The important point is to confirm the diagnosis. If it is patchy hair loss, it may be a different condition called alopecia areata for which other treatments may be required.     Both topical and oral minoxidil can be used if one does. to work, the other can be tried. There is no specific literature available on it though.
