Dr. Vaibhev Mittal, based in Sangrur is a well-known ophthalmologist who has more than 12 years of experience in this field. After completing his DOMS from Government Medical College in Gwalior, he finished his Fellowship in Comprehensive Ophthalmology from L.V. Prasad Eye Institute in Hyderabad. Soon after completing his studies Dr. Mittal joined as Junior Consultant at L.V. Prasad Eye Institute. Later he joined as an assistant consultant at Centre for Sight Group of Eye Hospitals. And he is here to discuss about the Opportunities of Ophthalmology. With more than 12 yrs of clinical experience, he is here to guide us about the branch.     @@rachnashah3701 This is copied from an article published in newspaper  New norms to make it easier for DNB doctors to get teaching jobs at medical colleges DNB doctors, who get their training at private hospitals which do not have medical colleges, obtain their post graduate qualifications from the National Board of Examination (NBE). PTI November 05, 2018, 14:30 IST The newly appointed board of governors of the Medical Council of India has approved a proposal to relax norms for DNB (Diplomate of National Board) degree holders wanting to teach in hospitals like PGI, AIIMS and other state medical colleges, Niti Aayog member V K Paul said on Sunday. DNB doctors, who get their training at private hospitals which do not have medical colleges, obtain their post graduate qualifications from the National Board of Examination (NBE). As per the proposal, DNB degree holders working in over 100-bed broad specialties and super-specialties private hospitals will be eligible for faculty positions on completion of one additional year as senior residency at any MCI-recognised institution, he added. Hence, they are not treated at par with doctors who obtain post graduate degrees of MD (Master of Medicine) and MS (Master of Surgery) from medical colleges who have advantage in teaching jobs having attended teaching institutions. The stiff guidelines of the now dissolved MCI had practically barred DNB doctors from getting faculty jobs in MCI recognised medical colleges. The government in September had superseded the Medical Council of India and had appointed a board of governors to take over the council's power and functioning pending passage of the National Medical Commission (NMC) that seeks to replace the MCI.

    One question sir, it someone want to start an independent practice of Cornea and refractive surgery, how much will the setup cost?

    @@avijitmudgal7554 refractive surgery set up is very costly. If you are in tier 1or tier 2 city, you need to have atleast femtosecond LASIK.. Cost of instruments will be nearly 2 cr. May be more Building cost extra

    I never really cared for ophthal much…. But now when I think of pg.. all my criteria for selecting branch is ticked by ophthalmology. I don’t want 100% med or 100% surgery and in ophthal I get both but they mainly focus on eyes. Which is a plus for me. Just like Dr. Mittal said..the branch chose him. And yes the best point is ….you get one of the best work life balance in medical field.

    @@suyashsaoji2012 I am satisfied with my stream and I see ample job opportunities all around me. .Atleast 10 doctors around me are looking for ophthalmologist and we all are searching for last 6 months but not a single ophthalmologist sent his or her resume. So, there are still more jobs than ophthalmologists

    Sir can people with slight fine tremors take up ms ophthal?.. mine are mostly due to fear & anxiety which I have learned to control.

    Tremors due to anxiety and fear... i personally would suggest you to choose non surgical branch.. during surgery, situation sometimes become tense and tremors would ruin surgery especially eye surgery where there is very little scope of error.

    Sir, as you might know govt. converted all PG Diploma seats to respective MD/MS last year and new Diploma courses are started on same concept by Nbe this year. So, just wanted to know your opinion wheather taking this Nbe Diploma Opthalmology will be a wise decision; as preparing again for neet pg seems tough.

    Sir I want to take ophthalmology but I'm worried about too much investment that I need to do to open my own clinic since I will get less salary from corporate job. It seems to be just impossible to open own clinic with such a huge about of investment needed. Like in psychiatry, its very easy to open a clinic

    Ophthalmology practice is not costly.you just need probably 10-15 lacs to start basic practice. You can earn and slowly upgrade

    Dnb is first step.. Don't get disappointed if you don't get hands on. Finish your dnb.. Try to learn subject during dnb. You can learn surgery after dnb. There are ample opportunities after dnb for surgical exposure

    Greetings🥰..Just completed my mbbs sirji...What will be the best fellowship in terms of money...if it is phaco surgeries..is it not like people will prefer going to the best institutes like lvpei,Shankar...instead of spending here n there...one of my friend has travelled from north india to south..lvpei for his lasic..he said anyway I have to spend same money...why should it not be done from the best..also lasic/prk/iol are not emergency surgeries..

    Good evening neetpg.. A. Best fellowship ( monetory wise) : cataract and refractive surgery B. Yes... Very few people travel to these big Institutes but 99.9% patients prefer doctors in their city... Surgery is not a one time job... It's difficult to travel to far away places for follow up visits. These days people are very busy and usually prefer a nearby clinic

    for cataract, set up cost depends may be around 50 lacs but for refractive surgery it will be in crores

    Respected sir M in a huge dilemma of ent opthal & anesthesia. But my mote intrest is inclined towards ent but everyone is discouraging me to take it.as there is very less scope for girls in this field. But in opthal ...i didnt understand the subject in my ug days.. Plus i have no experience of surgries in both the fields . Please sir help me...     Good morning NEET PG preparation. Ophthal is probably the easiest subject. A. UG life is different... There is very less weightage given to ophthalmology in UG but ophthalmology is probably the best branch especially for gals. No emergency, no blood, good income, multiple options( medical ophthalmology/ surgical ophthalmology) B. Many ophthalmologists practise medical ophthalmology... And they are earning good. Refractive surgeries are the future of ophthalmology and does not need great surgical skills. Having said that, this is just the beginning.. You will learn surgeries with time. Practising ophthalmology is easy...

    Both aiims and lvpei are good institutes. I would choose aiims for post graduation and lvpei for fellowship

    Hi kshitijaa Eye surgeries are delicate... But success rate is more than 99%. Cataract and refractive surgeries have very high success rate. Don't listen to anyone... This is a cool branch with good income

    Good evening. I personally don't have knowledge about amrita school of medicine. My advise A. Don't think about hands on training.. If you get it then enjoy it else don't get disheartened. Ophthalmology is not about cataract surgery... Learn basics of ophthalmology. Learning surgery is easiest part. You can learn cataract surgery after MS also.

    I don't think there is job saturation. I do agree, job opportunities are less in metro cities but there are good opportunities in tier 2 and tier 3 cities

    Sir, I love physics and Ophthalmology has been one of my preferred branches since college. On the basis of my neet pg score, I'm confused between opting MS Ophthal or MD Pulmonary Med. The only confusion I have is about job saturation in Ophthalmology. Would the job opportunities be similar to that in Pulmonary medicine? Or is Ophthalmology more competitive now?

    Ophthalmology is competitive but getting job is not difficult. I don't have much idea about job opportunities in pulmonary medicine but in ophthalmology you can easily earn bread and butter with cofee.

    @@ritikagupta2010 neither. I'm more interested in medicinal branches but I thought I wouldn't get them. But I did in the 2nd round. I went for MD pediatrics. Seeing the situation in the hospital, I'd suggest Pulmonary medicine over ophthalmology if you had the same doubt as I did if you had to change the preference order of your choice filling between the 2.

Im 26 now, girl & gonna take opthalmology coz it will check all my criteria ...i didnt get exposed to opthal much before but liked it theoretically I want to settle in metro sir .. people say its saturated 😐 can u please if its difficult to get work outside ?? Fellowships ? What is the scene for budding once im yet to join PG now ... Sir i might get an average college should i take now ? Or give one last trail for best college is it mandatory ? ... I have no reservation & rules are going worse im scared to try again ( is it wise to leave seat am getting in college for opthal to aim for good college in opthal ? Or just join & do fellowship later ? ) im really confused sir plzz help 😑🙏 should i join now ? Or study again

    First of all, congratulations for scoring rank in competitive exam. you have asked 10 marks question ( really long one 😉) let me try to answer this in brief a. In my personal opinion, its not advisable to leave your seat and prepare for examination again... rules changes every year and you may not be lucky next time b. every branch is saturated especially in cities but I believe there is space for everyone... count of doctors does not matter.. if you are good then you will be on top and believe me, diseases are increasing at a fast pace.. number of patients opting for lasik are increasing... and every human is bound to have cataract, so no scarcity of work

    c. fellowship gives an edge and I recommend every ophthalmologist to do fellowship ... d. There is little difference between good and average college for post graduation. Avarage colleges are not bad.. they just reduce the pace of your learning.. finish your post graduation of average college and do fellowship. believe me,after fellowship you will be at par with your colleagues of good college. moreover learning is a continuous process... you can keep learning throughout your life.

    I am a phaco refractive surgeon in 1st year of my practice. I did DNB and cleared in first attempt and it has been very satisfying till now both monetary and workwise and pgs thinking of taking ophtha should not worry.

    @@nihalmuniah it was good. I got good hands on exposure to almost every thing in ophthalmology. Work environment was non toxic and my mentors were awesome 👍. Academics I had to manage myself but I just read what I saw in opds and ots. But yeah people do fail in dnbs so you have to work hard to earn the degree.

Thank you so much for the video and your genuine guidance :) I have a query, I’m getting a private seat for ophthal in Karnataka for my rank, but the patient load is said to be very low there, should I take it or give another attempt ? I’m also scared about the next year’s new rules for NEXT exit exam or NEET 2023, there’s no clarity and so much uncertainty. But if I take MS in a college with less hands on experience, I might not gain enough confidence even in cataract surgeries . I would be really grateful if you could guide about this

    Hi priya. Don't worry about patient load or hands on training. Life is long and you will get ample time after college to master your surgical skills. Enjoy your college life and try to learn basic clinical skills like refraction, slit lamp examination etc

    Fellowship after MBBS or MS? I think these days laws are very strict and except few basic Surgeries, MBBS doctor can't perform surgeries. If you are talking about fellowship post MS ophthalmology then in this competitive world, fellowship is recommended

    patient may sue you for hearing loss also. I would anyday choose ophthalmology over ENT. moreover, chances of vision loss ( after medical treatment/ surgery) are very less in ophthalmic practise. proper documentation can save you for litigation

    @@mukeshbadmera4399 I faced similar situation few years back but I chose ophthalmology because a. ophthalmology is less risky b. anaesthesia is little dependent branch.. dependent on surgeon unless until anaesthetist owns his / her own intensive care unit c. ophthalmologist earns more... in anaesthesia, earning is limited by hours of day unless until he/ she owns hospital d. easy life... no emergency, no sleep disturbance.. no running from one hospital to other hospital etc etc

    @@sanika3149 I personally would not drop one year for LVPEI or Sankara because A. Motivated person does not need a good institute to learn. These days we have online option like KZread etc to learn B. Complete your DNB and then join fellowship program at a good institute. You can learn a lot during fellowship. Learning is a never ending process.

    Hlo Isha All medical colleges are good. I don't have idea about ophthal department of devangere medical College. During residency,you can start with Duke and elder book of refraction and kanski.

    Hello namita You can join MS ophthalmology. Finish your post graduation. In government service,you get opportunities to upgrade yourself. You can go for SICS training and then for phaco training all sponsored by government. Learn surgery there and practise in your government hospital.likewise,you can go for retina fellowship ( short term) and practise retina.colleagues and seniors will always be there for your help.

    You can definitely stop with MS but fellowship improves skills in very short span. Moreover, fellowship trained ophthalmologist is always preferred for a job over plain MS candidate.

    for small set up in small town, you need around 30 lacs... you can buy refurbished instruments to reduce initial investment. keep earning and keep upgrading

    Many Institutes in India offer fellowships... Both long term and short term You can choose best Institutes and apply for fellowship. Some Institute will call you interview and if you are good then you will get fellowship

    surgical training is not good in many colleges.. I myself did not operate a single cataract during my post graduation. just finish your PG... learn as much as possible during PG.. and you can learn surgeries after post graduation. don't worry about surgical training

    @@dn7707 eye surgeries including cataract or refractive surgeries hardly take 10 minutes and don't trouble back. But if you are working in high volume settings and operating dozens per day then yes,your back may trouble you. This is true even for other surgical branches

    Hello A. Cataract surgery is very quick surgery and average time is between 10-15 minutes B. We don't stand while operating cataract. We sit on chair and sit comfortably while operating C. I myself had no exposure during UG. So there is no need to worry. If you have interest in ophthalmology then go for it

    Hello. A. Yes,you can establish an eye hospital of your own even if you are from middle class. These days it's relatively easy to establish hospital.we can discuss various options on call B. Sankara and osmania are both good. Don't listen to what other people say..listen to your inner voice. Sankara nethralaya Chennai is definitely a very good institute.

    ​@@bonalakavyasree7384 There is no magic number. Try to learn atleast one new point from each surgery. In LVPEI,I used to get one surgery per week during my training period. the focus was not on number of cases but on learning basic skills,tissue handling. With good training even 100 cases are enough to master surgery. However there is always room for improvement

    Hello Diabetes affects retina and causes diabetic retinopathy and can cause blurred vision and we must have sharp vision for surgeries. I would recommend you to choose some other branch. However if you have interest in ophthalmology then you can practise medical aspect of ophthalmology and hire another ophthalmologist for your surgeries

    It is possible You can practice medical retina Glaucoma practice is mostly medical...very few patients need surgery You can also practice refractive surgeries...these surgeries are very easy to perform. Many ophthalmologists do OPD only and hire ophthalmologist for surgeries

    There is no scarcity of government jobs for ophthalmologists. But yes demand is less as compared to gynaecologist, paediatricians and Anaesthetics. However, there are ample opportunities in corporate hospitals and charitable hospitals.

    diploma is good if you want to work in private set up. I am also a diploma candidate and doing well. however MS Candidate is preferred over diploma candidate for government job

    Yes... Ophthalmology is good and ophthalmology does not discriminate based on gender. It is good for females because A. No emergency so no sleep disturbance B. No blood shed during surgeries C. Quick surgeries so no back ache D. Good money E. Happy faces all around F. female ophthalmologist who is not interested in surgeries has lot of scope in medical ophthalmology D.

    Sir is it worth taking up diploma ophthalmology in a deemed medical college where the fees is around 15L per annum Can we do fellowships after diploma ophthal for us to be prefered in any job or is it neccessary to do post diploma dnb followed by fellowships later on?

    renowned eye institutes like LVPEI don't offer specialty fellowships to diploma candidates... there is an alternate route... do comprehensive ophthalmology fellowship from that Institute (2+1 year). after 2 years,.. you can choose any sub specialty for fellowship ( 3rd year)... after fellowship, nobody will ask you if you are MS or DO. you will be known as cornea specialist or retina specialist etc. there is no distinction anywhere except in government medical colleges
