Obstructive Uropathy - Anatomy, Symptoms, Differential Diagnosis, Work Up, Management, Prognosis.

Obstructive Uropathy. We start with the anatomy of obstructive uropathy and how that translates into the differential diagnosis. We then discuss each differential diagnosis and whether that causes unilateral versus bilateral hydronephrosis and partial versus complete obstruction. We dive into the signs and symptoms of obstructive uropathy. The pathophysiology that leads to acute kidney injury. Laboratory workup including the utility of FeNa and FeUrea. Imaging modalities, their pros and cons. Management of this disease. And finally prognosis.

Can recurrent uti cause obstruction lead to hydronephrosis ? , I read that TB and belharzia most common to cause obstruction, any another bacteria can also cause obstruction ?

Great video- thank you! What approximate time does it take for the hydronephrosis to resolve after decompression and treatment of the underlying cause please?

Thanks for checking it out! We appreciate the kind words! The hydronephrosis often resolves fairly quickly. The creatinine/BUN (lab markers of kidney injury) will take longer to normalize or decrease back towards their new baseline (often over a matter of days if the obstructive uropathy is bilateral). Hope that’s helpful!

I was just diagnosed with bilateral ureteric obstruction. Both hydronephrosis. Left was mid ureteric obstruction and right was distal obstruction. What will be the management for this? I’m so stressed out. I’m 28 female :(

Nice work. I have seen delirious behavior in old folks with complete obstruction. Generally, men with BPH. Is obstruction with infection still considered a Urologic emergency?

Delirium with obstruction, great clinical tip! And yes it is still considered a urologic emergency. Especially if you start having trouble passing a catheter to decompress!

I wish you were my doctor! I currently have an obstruction (involving a duplex kidney), and I can't find a good doctor in my part of the country. i have sharp flank pain, low eGFR(39), huge kidney cyst/renal mass?/high BUN & Creatinine, severe hypertension, unilateral hydronephrosis...and NOBODY CARES! My FP is insisting upon a CT WITH CONTRAST...and I am saying..."no way." I'm 66 with a low eGFR and I don't want permanent damage from the DYE! I'm wondering if a DPTA(nuclear scan) would be better? Wish I could find a good Nephrologist. I'm thinking it may be an obstruction of a bifid ureter by small calculi, but who knows? If anybody can recommend a good teaching hospital or large Nephrology/Urology Center in the U.S., I would greatly appreciate it. I live in south Georgia(Valdosta, GA).

Definitely, this is relevant for all of my older male relatives. I am all eyes and ears, especially, for prevention and solution. The right to pee is not in the Constitution, but it should be.

Sure hope the guys in particular, watch this. Early diagnosis can very often prevent Kidney Failure to save a life .....not to mention finding tumours in 1 kidney, before the other kidney's affected. Share this with father.
