There is such a thing as too many nose jobs, and it usually results in necrosis aka death of the skin. Trans model, Jessica Alves, was formerly known as the Human Ken Doll. She's undergone at least 12 rhinoplasties, which has led to her nose nearly falling off her face.

    I agree, maybe it's drastic but I think that surgeons shouldn't be allowed to operate on people with body dysmorphia unless it's a reconstructive surgery

    I think one surgery limit per body part minus a NEEDED revision sometimes. trans people still will need surgery and need multiple things done. We just need to find the balance so they don’t end up like Jessica

    @@meoueo Incorrect. He insisted on having an obscene amount of rhinoplasties that caused his skin to turn necrotic (die). This left him with what's known as empty nose system. Michael suffered from a SEVERE case of body dysmorphia, and I got an A+ in my abnormal psychology class illustrating that point. I used him and "The Cat Lady" as prime examples. His discoid lupus had absolutely nothing to do with the self-destruction of his nose. Edit: I understand your need to defend him, but using demonstrably false lies are counterproductive. He isn't the first patient, nor will he be the last to suffer from empty nose syndrome.

    Very sad.😞 I recall seeing this person when they were known as “the Ken doll” on the show “Botched” after they had 3 rhinoplasties…in one year! I remember the doctors stating the possibility of skin necrosis if “Ken” did another rhinoplasty & it’s evident he didn’t heed the doctor’s warning. The doctors who didn’t refuse the surgery should be ashamed of themselves for this atrocity.

    I work in the medical field. If patient decided they didn’t want their healthy, left hand, we would send them to the psych department. If a patient came into ER after a dog had “redefined” their nose, leaving their nose necrotic &/or collapsing and making breathing difficult, the dog would be put down & surgeons would fight to keep the nose as patent as possible. Surely any reputable surgeon would just say “no” to someone like Jessica or are there some surgeons that will do anything for the money? It’s okay, I already know the answer to that.

    Ironically, in some cases the patient can get the healthy left hand removed. I can't recall they psychological condition, but it is a form of body dysmorphia. In the case, the surgeons and the psychiatrist have to agree that the condition is too severe, wherein the patient will do *ABSOLUTELY ANYTHING* to have their healthy hand removed. It's bizarre how our minds and societal conditioning can corrupt us to the point we cannot tolerate our own attachments.

    Such "physicians" should be hunted down and their licenses instantly revoked. They took Hippocratic oaths just like every other doctor and have willfully, viciously violated it for profit. Check out the 60 Minutes Australia docu (it's on YT) on the criminals doing cosmetic surgery Down Under. ZERO conscience, all reckless greed! The Aussie medical boards make some kind of a distinction between plastic surgeon and cosmetic surgeon that allows any GP to hang out his shingle to butcher their overly trusting patients with near impunity. The examples they show are hair-raising AND LEGAL, even when the patient nearly died (in agony).

    A friend of mine when to Tunisia for excess skin removal and the hotel stay for friends accompanying the patient was extremely cheap. The reason being is that after the surgery, they send you back to the hotel for your recovery and your friends have to take care of you and do all the after-care bandage changing... for which they were not trained and the environment was not sterile. She came back to Switzerland and had to be admitted to the hospital for infection and wounds which had opened and she almost die. It was not worth the savings from the price difference in surgery

    Being in the medical field myself, I’m curious to know if there comes a point where you guys would maybe discuss referring someone out to Psych if you start to notice some symptoms of a significant case of body dysmorphia? I mean ultimately it’s the patients decision but there has to come a time where it’s considered that they could become a potential danger to themselves right?

    Many surgeons have patients fill out a survey that screens for body dysmorphia before they even enter the consultation room. If we see warning flags we will often refer them to a psychologist or broach the topic with them if they appear to have some degree of personal insight and are receptive to it. The thing is, body dysmorphia is indeed a spectrum... On the low end, many surgeons will go ahead amd operate. However, a patient with significant/true body dysmorphia will NEVER be happy with the outcome of your surgery. You might create a nose that in your professional eyes balances the individual's features and is within the surgical aesthetic ideals. For the patient with body dysmorphia however, they do not see things the way you do and you are setting yourself and them up for a lot of potential grief. It can be very difficult to communicate this and to turn a patient down, especially when they are flattering you and insisting that you are the one that can fix all their problems... And then have them turn to anger when you decline. However, at the end of the day, this is ELECTIVE surgery. The patient selects the surgeon and equally the surgeon selects the patient. As surgeons we have a responsibility to first do no harm.

    @@Mister_Listener These are not mere suggestions but the common practice of the majority of surgeons who perform a lot of cosmetic plastic surgery. Cases like this are the extreme owing to the psychological conditions of the patient who insists on repeated revisions and doctor shops (and likely has been turned down by many surgeons) until they find a less than scrupulous surgeon who will agree to operate if the patient is willing to pay or because their ego has been stroked. Because such cases exist does not make the "lovely suggestions" I made untrue. I attended a Rhinoplasty conference in Germany last fortnight and we had a whole session on this very topic.

    @@sunshinesmile94 I do believe surgeons are not out to hurt anyone, however, even with a survey patients will say anything to avoid not having the surgery, it’s like by any means necessary I’m going to have it done. Can their medical records be noted for the reasons why they want it but advised not to.

    @@velmasmith4604 The survey is only one of several ways surgeons evaluate patient suitability. We get feedback from secretarial staff on patient behaviour on the phone, when checking in... We reserve 45 minutes to an hour for a first consultation for the reason that by asking the right questions and looking at the patient's body language and non-verbal response, most surgeons will get a gut instinct on the matter. Having said that, you can do all those things to screen and still be caught out. It is not an exact science, since as you said, patients who have read up on this and faced several rejections by other surgeons can learn to mask their behaviour and say whatever they think needs to be said to avoid raising red flags and getting the operation. Unless the patient divulges to you who their previous surgeon was (occasionally)... Or who they have been doctor shopping prior to seeing you (almost never happens), there is no way to know who to contact to access their case files. This is ELECTIVE surgery. The surgeon has the right to decline to operate if they do not feel the patient is psychologically ready, or if they feel they cannot meet with the patients expectations.

    Amazing how much the human body can take. This person had 20+ surgeries before this. In any case, when he lived as a man, he went to the Botched doctors and they told him to stop. Obv didn't listen. You could tell right away they had major issues. Whoever performed the last surgeries should be denunciated and removed.

    She began with a significantly bulbous tip and thick sebaceous skin. She probably kept insisting that the surgeon "make the tip smaller" to overcompensate her likely dysmorphic perception of her nose. Consequently over the process of multiple revision rhinoplasties, she had excessive (likely repeated) alar base reductions and defatting of the tip tissue. Combined with the transcolumella incision in the open approach her skin soft tissue envelope became more inelastic and blood supply was compromised. This led to a contracted, shortened nose, pinched nasal tip with parenthesis deformity, alar notching... Not to mention internal nasal valve collapse with excessive lateral crural cartilage resection and scarring/contracture. Most reputable surgeons would turn this patient away... And directed her to a psychologist. Most surgeons probably did exactly that. Especially by the time she reached her 4th rhinoplasty, let alone her?12th. However unfortunately there will always be someone out there with dubious morals who is willing to operate if the patient is willing to pay.

    People need to realize smaller doesn’t always equal better .Kylie Jenner actually made her nose a little bigger. It’s Proportions. Not everyone looks good with plumped cheeks and huge lips small nose etc. we really need to start appreciating other kinds of faces. None of he true beauty’s of he world look anything like the people who go overboard with plastic surgery.

    He was rejected by the Drs on botched they refused to give him another nose job, a Brazilian Dr rejected him and told him he was going to have complications because he’s had way too many nose jobs and that his nose was going to cave in 🤦‍♀️He was warned I don’t feel bad for him

    I once watched a ‘nose job’ on KZread. It was horrific. Worse than brain surgery. Cartilage was ripped out and removed, etc. everyone should watch that video. Keep your nose safe!

    I have so much compassion for people with Body dysmorphic disorder. As a former fentanyl addict, I totally get it, believe it or not. I understand how lonely and depressing it is, living with this this disorder. Both are lethal delusional nightmares a lot of people never wake up from.

    Cheek implants caused serious nerve damage and disfigurement to one of my best friends. She started with some simple procedures but the cheek implants started to migrate and became infected, surgery after surgery failed to fully correct the problem and my beautiful friend eventually succumbed to infection.

    This all started due to being able to travel all over as a flight attendant (if I remember the first Botched appearance correctly). The idea of doing this type of tourism, entering countries with questionable ethics because those doctors will say yes, is scary. I do hope Jessica is at least happier as a woman, though I don't have much hope of that because the power of the spotlight is just too strong for her.

    We need nose to breathe, why risk a collapse and inability to breathe through your nose just to have unnaturally small nose? There was an episode on Botched where both doctors told Mrs Alves NOT to do any single more nose surgery because she won't be able to breathe through nose ever again. At the end of episode she said "of course I am going to do more rhinoplasties my nose is still too big" (hint: it wasn't). So it's kind of hard to feel bad for her, she refused to listen to doctors.

    She already had breathing issues, when she was on Botched and doctor Nassif would usually fix her nose by making it larger and she would've denied it, BUT even dr Nassif refused to do any further surgery. Dr Dubrow and him work on such difficult cases yet hers was long gone to be fixed. I know some people might disagree, but I find them both to be extremely talented surgeons, maybe even the best in handling the most extreme cases, and if they denied it, that really showed how bad the situation was. I hope she gets help with her body dysmorphia and is at least a little happier about her physical appearance after the transition 💜

    I read a very recent post by Jessica Alves that she's planning on another rhinoplasty. And recently had the "fox eye" procedure done - the Bella Hadid eye she called it. Sad thing is Jessica Alves is beginning to resemble Jocelyn Wildenstein who was a very beautiful woman but went way too far with plastic surgery. What I don't get is surgeons who would even agree to continue doing these procedures on Alves.

    Ppl are forgetting that Alves gets surgery overseas, where the surgeons don't ask questions. They won't send anyone for psych evaluations, and they won't get called on bs for botching surgeries because the patient is intent on surgeries, no matter the danger.

    I have dysmorphia self diagnosed. I now have a life limiting neuro disease and realise that I was wrong all along. It was all in my head . Just wish I'd realised that years ago x can't wait To watch. Thanks Dr m and ethan x

    When our surgeons refuse to do surgery , b/c it's a safety risk , many people go overseas . A few of them don't make it home alive , but they are willing to take a chance , with their lives . #at what price vanity 😢

    Lesson to learn from this is don't go to extremes either too big or too small , each part of the body needs to be kept in the right ratio to other parts of the body so the body and face looks natural . The best kind of plastic surgery is when you look better and refreshed but no one suspects that you have had any plastic surgery done .

    It bothers me when surgeons see a person with clear mental health issues and they keep doing surgeries. At a point these surgeons needs to be held accountable for putting people's health in danger. They are career criminals when they do stuff like this. Mental health evaluations need to be required for people like Jessica Alves. Her nose is gone. I feel bad for her and hope she gets the help she needs.

    Jessica used cadaver (pre-transition) and it caused an infection. She was on botched and talked about it. She had a bad cases of MRSA.

    @Luis Guillen. That's disgusting. First of all Jessica is a man, DNA doesn't change unless you've been vaxxed, and even that doesn't change a person's DNA it mutates it.     How was she even able to be diagnosed as transgender with such a severe case of bodydysmorphia she had already done everything she could as a man? Maybe being a woman just seemed like more of the same. But then I don't think she's used to going to reputable medical practitioners for anything.

    @@1917yee I don't really understand what you mean, I understand a trans woman getting plastic surgery to become what they envision themselves to be. But this case seems like she has a male ideal first for a very long time and when that didn't make her happy she went for female surgery instead. Part of what a normal gender transition includes is therapy and living as a woman for a year , things like that while they figure out of you unhappiness with how you look is gender related or bodydysmorphia that will not be helped by surgery and hormones. I think she would be a very iffy case on that one.

    This is the surgery my nose needs. I had revision rhinoplasty with a terrible surgeon who removed too much cartilage and now my nose feels so heavy, looks ugly and droops. I wish I could have mine repaired too. I have regular Botox to try and stop the worst droop, but waaaaah, I’m just trying to live with it, but I’ll always yearn for a pretty nose.

    I see a lot of people really pushing the boundaries of plastic surgery-either they are asking for it or the surgeon suggests it. I’ve even had surgeon’s suggest things to me that I know are not safe & would be risky with possible bad consequences. Thankfully I am educated enough to say no & find a surgeon that is more focused on safety that I feel I trust. But for people who are not educated it is very sad.

Although Jessica does not have a uterus, there are many other women born with the xx gene who do not have a uterus due to the condition mrkh. These women are not less of women than people with a uterus so the fact that jessica isn't fertile and doesn't have a uterus doesn't make her any less of a woman. Furthermore the xx chromosome codes for a protein called estrogen and although Jessica may not have initially had much estrogen in her body, later in life when she transitioned she will have had hormone therapy to amend this. Therefore if medically she is a woman and socially she is a woman and expieriences the same sexism somebody who was born a woman would experience, she is a woman. I realize that you may just be transphobic and not very educated, however this concept should be pretty easy for you to understand, but whether or not you understand this, trans women are still women regardless of what illogical opinions rooted in transphobia you may have. I hope this helped to educate you 😊

    What I do not like as a woman about Jessica is the way she is sexualizing her image as a woman by overdoing everything. The feminine woman is absolutely not her twisted idea of femininity .     You need to considerate 2 things: 1: All those 1782399291 procedures BEFORE the transition only happened bc she was struggling with her identity, when she fully realized that 80% of her dysphoria was due to her being a girl in a “male” body. She always talk about how unnecessary all those procedures was and it would be easier if she knew she was trans back then. She regrets and been fixing past surgery mistakes. 2: About sexualizing her image, you need to know that transgender woman are very judged, specially if their features are not extremely feminine, delicate, bombshell like. A cisgender woman don’t go thru this, since many cisgender woman can choose to be “rustic” or “manly” without society’s judgement. So don’t blame her, blame the mean society

    I think some psychological evaluation must be done before plastic surgeries. Because most of the time people going through a breakup, can't decide their gender , or some other sudden changes in their life that can cause depression can end up like this.I think doctors should find the underlying motive

    In 2017 I had a septoplasty complete to correct a deviated septum/septum spur while removing those turbinates in the right side nasal passage. Because my ability to breath through that nostril had diminished from a broken nose in the 4th grade at the age of 9 years old. I'll say, it was a challenging time after recovery, cleaning, of the dried blood, drainage and even a clipped view that had to be cauterized to stop the bleed out. then more healing and cleaning, Rinsing with saline to clear any dried mucus. It was a great choice for it has made my breathe far less taxing especially while walking. However, I believe that those people who keep having multiples cosmetic surgeries have a deep rooted psychological self image issue. At 58 I'm not with elective surgery, unless something gets broken or damaged. the recovery is difficult. Honestly, the trans person has sever mental concerns and should have not been given the green light to change his primary gender.

    Dr. Motykie is so respectful when talking about people's lifestyle choices. He really does only seem to care about the healthcare and science of their choices. I used to be so heavily against anything plastic surgery (even fillers and botox) but its Dr.'s like this on youtube that have changed my perspective to maybe being open to the idea of small adjustments for myself.

    I remember in one of the Botched series Jessica (then still looking like the Ken doll) wanted a nose job & the Drs refused to do any surgery and they clearly pointed out that if another nose job was done there was a very high risk of the nose tissue dying & falling off! Didnt listen did she. We are living in a world of absolute madness where surgeons are putting profits & popularity before the risk to the patient. Thank god for those surgeons who will give honest opinions as to risk, will refuse to do surgery if it means loss of tissue or long term complications rather than benefit to the patient. We need more of them. If this was the case, these patients that need help with body dysmorphia will get the right help without putting themselves under a barrage of high risk surgeries making themselves something they are not. Cosmetic Surgery to repair post surgical damage, injury, burns or disease related surgical removal was once the need for cosmetic surgical intervention, now it is becoming the leading cause for cosmetic surgical intervention when surgery is performed despite warnings of damage eg..entire nose reconstruction as in this case. When is enough, enough & doctors refuse to do surgery that will lead to these outcomes?

    That’s extreme in the extreme. I highly spent that will be possible as there are currently only a few physicians who can do that, the donor has to consent AND she doesn’t possess the vasculature or ligaments needed to support a uterus.

    @@codename495 take a deep breath, we’re at least 50 years or so away from doing something that difficult and advanced. Uterine transplants are a nightmare and not permanent even for cis women. They let you have two pregnancies with the uterus and them remove it again.

    makes sense that growing up trans and not knowing it or having the knowledge or opportunity to transition would cause serious body dysmorphia and going to extreme measures to try to love one's appearance

    Excellent video, but you have other things to address that would turn this content even more compelling: she can no longer breath through her nose, the size of her implants cause excruciating pain on her back and joints, nerve damage, numbness, and loss of the capacity to feel pain in some areas, etc. As a woman who had nasal restoration done twice due to respiratory problems, I feel scared of not breathing! The size of the implants can cause serious problems.
