Nocturnal Panic Attacks / Panic Attacks at Night

Nocturnal Panic attacks, waking up from a deep sleep in a state of panic can be alarming, horrifying and can even be debilitating for some. The terrifying experience can begin to associate bedtime and sleep, what is suppose to be our calmest period, with high anxiety.
Nocturnal panic attacks, or panic attacks at night, feel like daytime panic attacks and can include the same symptoms like a racing heart, sweating, trembling, hyperventilating, and a sense of doom. Surprisingly, night time panic attacks are different than night terrors and night mares, which both occur at different stages of sleep.
Nocturnal panic is very common among people with day time panic disorder. But, it is also seen in people with other anxiety disorders like social anxiety, generalized anxiety disorder, and phobias. It is also seen in people with ptsd and depression.

I started having these about 4 years ago. It’s always the same thing.. I wake up in a panic and get up and frantically walk around the house half asleep while my heart is racing more than it ever has before. After a few mins of shaking , tingling and sweating, I start coming to reality and calming down. I describe it as being in between sleep and wake state. Has anyone else had this?

I just woke up from a nocturnal panic attack. It freaked me out because I usually have panic attacks during the day. For me it's usually a slow buildup which I can control most of times (not always), but that happened out of the blue. I was probably asleep for only a few minutes when I woke up jumping from my bed unable to breathe. I experienced that choking sensation. It felt like my lungs had shut down. It was one of those times I was sure I was gonna die. For some reason, I rushed to the kitchen to drink water and I was able to calm down quickly, though my body is shaking, tingling and my muscles keep spasming. I did some tapping (EFT) which is super effective for me but I doubt that I'll be able to go to sleep anytime soon. I always come to comment sections to read people's experiences. It helps me to relax, though it sucks to know that so many people go through this. I hope y'all can get better someway. Just never give up!

This is exactly what I dealt with a couple years ago. It happened nightly. I even went to the ER a few times because I thought I was having a heart attack. I got checked out by my doctor and a cardiologist. Once I realized I didn't have any heart issues, it still took awhile for my subconscious brain at night to catch up. These sensations would still wake me up nightly and I started to feel frustrated. Wondering WHY is this happening if everything is ok health wise. So once I started to allow these feelings to happen, note that they are happening but not worry about the sensations, taking a few deep breaths, and then try to settle comfortably back into bed, then I noticed the sensations happening less and less. Over the course of a couple months the nighttime panic attacks started happening less and less. Now I might get heart palpitations a few times a month but I just wake up, notice them, roll over and go to sleep. Practicing mindfulness in the day, getting some sun and fresh air daily, not drinking caffeine after 3pm, doing some light exercise or stretching in the day, and listening to a calming app at bed has really help me with my anxiety and panic attacks. Try the Calm app, Abide app or Dare app.

The worst thing about this is that you cannot explain this vividly to someone without sounding like you are overreacting. I started to feel detached from reality and developing fear towards nighttime. Everything becomes more difficult; shortness of breath, lumpy feeling in the chest, decreased heart beat (52-60), occasional palpitations etc. But nothing compares to that terrible feeling of being woken up in such a catastrophic state that I cant even describe it. The mornings are usually fine but as the day progresses it just gets worse. And another thing... people also assume that it must be depression, but I feel genuine happiness in my life. I don't know. It's hard to process this and put it into words. Btw, thank you for this video!

I am so sorry that you have this experience every night. Try to give yourself a message that you can jump awake as often as needed, as long as the panic wants to come, you are fully welcoming the panic if that is what your brain wants to do. Now, I know that sounds crazy. But, your brain is most likely panicking because you have conditioned yourself to be afraid of this very reaction. So, your brain thinks you are in danger and therefore tries on purpose to wake you up to save your life. Take an opposite approach. Give your brain the message that panic can come and go as it pleases, it is okay with you and you are not alarmed by the panic at all. I hope this paradoxical method calms your brain down.

With normal panic attacks you learn to spot the thoughts building up, and learn to understand that their bark, so to speak, is far worse than their bite, in other words you're not dying. You're not going mad. But these night time waking panics are like going back to square one, they feel totally physical and not psychological. Breathlessness, headache, dry mouth, and racing, pounding heartbeat, made me think I'm like totally dying - with a heart attack. This video has put my mind at rest, I now know that the anxiety is causing these symptoms. I now see them as like normal panic attacks. They will pass, no need to phone an ambulance, or the emergency helpline, I mean I've had about forty in last two years, if they were heart attacks, or strokes - I don't think I'd be up and about ten minutes later. If they were serious, they wouldn't pass so quickly. Still all the same, the terror still hits hard in the middle of the night, but it will pass I tell myself, thank you for posting,

Sometimes our brains can take work stress and react to it with a high anxiety or panic response. 😊

It always wears off after a short time. The causes for this that you provided are really accurate, I think--for me, at least. I am in a nearly constant state of stress, I have a drinking problem, and I've been depressed for a long time. lol so it makes sense that I would wake up in a freakout every day.

Hi Patrick. I often work with people that have a great deal of stress in their lives and different substances like alcohol or benzodiazepines are used in attempts to lower the stress and relax. Unfortunately, there is a rebound effect when these substances wear off, and people can wake up with a spike in their anxiety and their quality of sleep diminishes. I so appreciate your sharing your experience because you are absolutely not alone. Possibly by sharing, you will be inspired and possibly inspire others to begin to make changes that will improve your health and sleep quality. Thank you so much for your comments. 🙏❤️

@@drifting202020 hey man just wanted to share my experiences with panic attacks and anxiety because I myself have a problem with alcohol and substance abuse. The first very severe panic attack that I ever had landed me in the hospital after a long night of drinking. I started to see a trend every time I would drink, the following day I’d have a bad panic attack. It ended up becoming a vicious cycle and I would drink to quell the feelings of panic and anxiety. It would work for a little bit, but always come back way worse when the alcohol wore off. So eventually from doing this every day I became physically dependent on the Booz and Xanax. I knew it was time for a change. I ended up in the hospital once again but this time detoxing from benzodiazepines and alcohol. Then went to a 30 day in patient rehab where I worked through a lot of these issues. This was the best decision I ever made. Bottom line, don’t drink if you have anxiety and depression!! It might temporarily relieve your symptoms but you’ll always be chasing that feeling and eventually put you in a bad position. The best advice I can give as someone who’s suffered from this crap since I was 19 (I’m now 28) is; Eat a proper diet, exercise everyday, get a good nights sleep, and avoid alcohol. Also try and learn some techniques to get you through those uncomfortable scenarios when you feel like you’re about to lose it. If I can do it anyone can. Hope all is well and this helps just hearing that you’re not alone in this. You got this 👍🏼

I'm going to practice your suggestions, and I already did a good job tonight of calming down (despite some bumps). This video has helped so much already. I feel heard and validated, so I don't feel so scared now. I'll definitely make sure to avoid my bedroom after I have one, though. I didn't even think about that. Thank you so much. :-)

Im currently 14 yrs old and ive been struggling w anxiety ever since I was 10,but today was one of the first times I woke up having a panic attack. Ive been having a very messed up sleeping schedule, going to sleep between 5-6AM and I decided to change it. I pulled an all nighter last sunday and then was awake the entirety of monday, only to sleep at 10PM,i had a very good sleep up until I woke up a midnight to use the bathroom, sooner I knew it I was freaking out for no reason and trying my best to calm myself down. Its currently 05:13 and i still have not been able to fall asleep,i am indeed tired and my eyes feel heavy,i have cried out of frustration for hours but besides that I cannot fall asleep,it has been real tough. Ive watched every calming video, listened to every calming music and heard every ASMR but it gets really bad when no one around really gets it. I live with my mom that works all day and if I were to ever wake her up she'd be angry and I'd feel bad, sorry for so much text, but i really needed to let this out,the whole night has been a living hell. Great video however!!^^

I think it's very true that you need to be careful about how you react to an attack. The things you do can become a sort of hook that causes, or worsens future attacks. If the cycle has already formed you need to break it somehow by avoiding whatever you associated with panic, but ideally you want to be aware before you form the cycle.

I have also been struggling with this for more than 10 years. I sometimes feel as though I am on the verge of dying and sometimes I even have mild hallucinations and paranoia, like my roommate is going to do something bad to me and I get up an run out the door. It goes away after a few minutes. It is usually at its peak when I've had upseting experiences in my life, but sometimes it is completely random. I took antidepressants as medication once, it made great amendments, althought they did not completely disappear, but they were far less terrifying. But as I finished my 9 month medication treatment, they slowly started happening again. Last year I had started another treatment with plant-based formulas, that you can take almost indefinitely without causing addiction and they also have had a great effect. Now I take them from time to time and it is much better, the panics are less frequent, but I do notice that when i take long breaks from them, it starts getting worse. I also started therapy and hope it would have an impact on my anxiety.

Great explanation. I was sitting listening to music recently and without warning I felt very hot, my heart rate went up and I felt anxious. I sat calmly and started deep breathing. It passed after 10 mins. I went to bed but woke at 3.30 am with tachycardia and this put me into panic mode. I started the deep breathing again and eventually it passed. I was drained and my nights sleep ruined. This happened me a good few times in January 2019 and I ended up in the Emergency Room. They found nothing but I still went to bed at night dreading the return. The GP gave me Xanax for 5 nights which I took to get a full nights sleep, and sleeping tablets that I didn’t take, put me on Lexapro, I got a 48 hour holter at my own request as I felt I’d ectopics, and that came back normal. I was on the Lexapro for a year and quit in April 2020. The nocturnal issue was always in the back of my mind as it’s pretty frightening and it’s starting to come back now again. Really pissing me off as I’m ideal weight, exercise daily, eat well, don’t drink or smoke. Seems to be the long time GAD lurking in the background that I’ve had since 2014. I still do worry about my heart daily even though I’ve been checked out numerous times and had Echo/Stress tests as recently as June 2021 that came back excellent. I do feel flutters some days and this is bothers me. I get upset by the sensation. Can the underlining constant thinking or fixating on the heart do this?

That sounds terrible and I am so sorry that you experienced that. What may be happening is what I see in many people with anxiety sensitivity and a combination of health anxiety (somatic symptom disorder) and panic disorder. In these people, they feel movements and changes in their heart intensely and sometimes it can even be painful. Their sympathetic nervous system is activated and then panic symptoms and the heart symptoms become wired together. When this occurs, the brain can trigger a panic attack if it becomes aware of heart changes even if you are sleeping. One way out of this is treatment for health anxiety and panic disorder to rewire the brain while you are awake. If you are interested in help, I have a course and a support group that can help. @@PaigePradkoTherapy I have developed exactly this. The moment I think I feel a heart palpitation the anxiety kicks in. During the day, this can manifest itself as a minor ghost chest pain just briefly but enough to start a panic attack. Typically controllable during the middle of the day. But I’ve searched and found this video at night as I’ve just recently started to have increasing nocturnal panic attacks. I immediately wake up always nearly 30-45 min after falling asleep with profusely sweaty hands, sit straight up from bed which usually wakes my wife. Once my heart is racing my mind immediately starts racing with thoughts about “why is this happening” and I start to mentally panic reason that it’s my blood pressure, my blood sugar, I start eating things or drinking milk. I think something is going on with my heart, ie, a stroke or heart attack condition. I then like clockwork start having an extremely debilitating anxiety attack and start trembling and shaking uncontrollably, my left arm goes numb first so it makes me think more about a stroke but I can squeeze very firmly. Then my lips start to vibrate, my feet start dripping sweat along with my hands, my right arm gets vibratingly tingly and a bit numb and by then something about eating and drinking a bit of sugar seems to slow it down, though this can rear back up for 2-4 hours. I went to get blood testing and things done several years ago for normal daytime anxiety but the nocturnal stuff only started happening as recently as 6 months or so ago. I feel that maybe I should get some form of heart and blood pressure monitoring device because my fear would only be alleviated immediately if I know it’s in my head. My fear stems purely from thinking I’m possibly having a cardiac event. I cut back stimulant use like too much caffeine thinking caffeine was mainly the culprit and also quit using cannabis thinking it was the main culprit but my attacks have gone on unabated. I should also mention I’ve been in a very high stress engineering career but the past few years all forms of work and world stress are triggering it. At this point I think it’s become a bad panic disorder but I am not willing to take Klonopin or Xanax or something for this. Strenuous exercise fixes it for me and it seemingly ramps up in the winter.

Zach, I am so sorry. I understand well the panic disorder that has evolved to include health anxiety, as this is a condition that I specialize in treating. Many clients are referred to me from a group of cardiologists because it is a common phenomenon of believing the heart pain and palpitations are indications of a serious cardiac event. When in fact it is panic disorder. The worried thoughts and checking bodily symptoms and searching for reassurance all feed the health anxiety cycle. The nocturnal panic is an indication that your brain has now associated certain bodily sensations with your fight or flight response. All of this can be helped, but you will need to do very specific treatment. If you would like help, I have an online course and an accompanying support group that can help you. You can find more information here www.paigepradko.com/healthanxiety.

This is a normal anxiety response and nothing to fear…but I certainly understand that waking up like this can be confusing. If this continues, you may want to get a sleep study for a diagnosis.

Hi Christopher. I am so sorry that you went through that. Panic can be horrible. But, it is important to not tell yourself messages about fearing having another. Instead, tell yourself that you are okay and if panic and anxiety want to come…it is fine with you. Welcome the panic and welcome the anxiety and use phrases like “bring it on”. If you fear anxiety and panic, the brain can create an anxious response and cause you to actually go into a fight or flight mood. We have to give our brains the message that we’ve got this…..no problem…bring it on. kzread.info/dash/bejne/c5OCpsGPp8fcmps.html

This can be very upsetting when it happens, but, it typically settles down for most people on its own. If you are having these nocturnal panic attacks for more than 2 weeks, I would suggest having a sleep study. I do not recommend medications without knowing what is going on from a sleep study as sleep apnea and other issues need to be ruled out.

I've had these now for almost 2 years on and off - typically presents when I wake up in a half sleep state - I notice my chest really hurts and then boom - my heart takes off and starts racing. When I first got them the attack could last quite awhile and I had a hard time calming down - I called the paramedics several times thinking I was having a heart attack - it's a brutal and scary feeling. Now I still get them, but they are not as aggressive as I have learned to deal with them now. But for awhile I was associating the night and my bed with panic because that's where they were happening - and they still do - but they are much shorter in duration now thankfully.

I have visited a doctor a year ago and he prescribed me Magnesium with vitamin B6, Melatonin and valerian root, and 0.5 mg rivotril before going to sleep, after I took the night Magnesium for 2 weeks it made the panics a lot easier but sadly I became really aggressive during the day and I quit taking it, and rivotril never worked. Now I'm feeling very hopeless. I think I know what it caused me the nocturnal panic attacks. Before I got married I only had 1 in a month, after I got married and especially when I gave birth for the first time I have them every single night. Its all about stress and rapid change of life. I’ve had this for about half a year, and it’s mainly because of episodic migraine. I usually start panicking because of nausea, I’m incredibly afraid of feeling nauseous and even more afraid of throwing up. It’s quite a problem with episodic migraines. Having that during a migraine attack is not that bad, it relieves the pain and nausea. It’s worse when I have it outside of a migraine attack, cuz it makes me feel extra nauseous and the rest (yawning, trembling, sweating,…) except for numbness and dizziness. I want to get it fixed but nobody believes me- it can be a real issue

Hi. I've been having nighttime panic/anxiety attacks for more than a week now, for the first time in my life (always had anxiety, though). The symptoms you all describe are really terrifying, and I agree with someone who said that describing this to someone else makes you sound like a drama queen. But I can assure you this is one of the worst things that have ever happened to me, like I told people I know. A few times I've been in such state of desperation at night to PLEASE JUST LET ME SLEEP that I nearly sobbed. Kid you not. I've seen many sources now that tell you you should get up out of bed and do something to calm down, but it's so damn hard because you're neither fully awake for asleep. Last night I forced myself to sit on the couch for a few minutes. Next time I'm going to do what the video suggests and force myself out of bed and splash water on my face. This is like hell on earth; my normal activities and cycles are totally disrupted (day and night). Thank you for the video and thank you all for commenting; it really made me feel better knowing that so many others are experiencing this. Virtual hugs to all.

Hi Hayden. I am so that you are experiencing this. It sounds like you are not struggling with panic or anxiety during the day. I wonder if there was a past trauma that has been triggered. I suspect that this will calm down in you in time. Try to look around the room before you go to sleep and remind your brain that you are feeling safe. Do not give your brain messages that you are afraid of this happening again, or that can perpetuate the problem. The brain creates anxiety to wake you up because it believes you are in danger from the very panic that you fear. In other words, if you fear it happening, the brain activates the fear center and it happens. Give yourself the message that you are not concerned at all…if anxiety wants to come, that’s fine with you. It is a paradoxical treatment.

It always happens to me within 1.5- 2 hours of falling asleep. I suddenly wake up, sometimes not even knowing who I am, with fast heartbeat and trembling from the inside out. I feel the urge of standing up, and there’s when the panic kicks in. I feel claustrophobic, like trapped in my body. Everywhere I look triggers me more panic. Feel like if I want to escape but I can’t. Feels like the feeling is going to last forever. No coping mechanism works. Feels like I can’t see, but I can see. Feels like eternal suffering. But then it goes away, and I feel stupid. However, it becomes my greatest fear to go to sleep. I can control and avoid anxiety and panic attacks while awake. I learned to do it without medications, since I was 11 years old. But I can’t control it when it happens while I’m asleep. It’s horrifying. I am so sorry you are going through that. Your brain senses things even when you are asleep. It sounds like if something is off…and your brain detects that there is no fan noise…it wants to alert you by activating your nervous system through fight or flight response. Think of this problem as something that your brain has misinterpreted and now you will have to slowly teach your brain that you are safe and you are not alarmed even when you wake up in a panic. As soon as you are awake…act as if you are not concerned at all so that your brain learns in that moment.

In your description, you describe a fear, and then you describe all of the things that you do before sleep to calm yourself…to make sure that the feared event does not occur. We call those behaviors compulsions because they reinforce an anxiety cycle. Your brain truly believes that you are in danger and all of these calming behaviors are necessary. The fear seems to be conscious to some extent but unconscious as well. I can only give general advice here, but would recommend stopping all calming behaviors before bed and doing the opposite of what you have been doing. Your brain has to learn that you are safe whether you wake up at night or not…and that you are safe whether you panic or not. Here is a video on the ocd cycle. I want you to imagine how you can stop this cycle by stopping your behaviors to calm yourself before sleep. I realize that this may sound counterintuitive, but it works. kzread.info/dash/bejne/a2F7t6SsadfVoMY.html.

@@PaigePradkoTherapy I'm certainly open to that. The symptoms showed up without the "trying to get rid of symptoms" I would say they can disappear the same way. The symptoms progressively got worse over 6-8 months and I didn't do anything about trying to get rid of them during those months and they didn't disappear so the last 2-3 months I've started trying to find a root cause and do something different. I will check out the video. Thank you for your response and sharing your thoughts.

Hi Ivan. I am so sorry you are experiencing this. I am not a physician and of course cannot diagnose you, but, what you are describing are symptoms of panic and high anxiety. The fact that it comes and goes fits panic as well. I do suggest getting checked out by your physician if you have not had a checkup recently, but it sounds like anxiety. I hope you feel better.

Breathing is my worst problem, breathing techniques are impossible for me, they have the opposite effect. It makes the ability to learn meditation useless. Exercise increases my heart rate and I breathe faster…boom can’t breathe. My bedroom is where most panic attacks happened and the association has been firmly made, I am working to correct this. I wish I could get help but it is almost impossible to get a doctor right now because of the stress the pandemic has created. Some days I cry not because I am stressed but because I am tired of fighting just to breathe, I wish I could stop needing to breathe. I hope no one feels like this. I wouldn’t wish it on my worst enemy. Be well people.

I understand what you are describing. You will most likely need to do Interoceptive Exposures to correct this breathing issue. I left you a message on another video about my online therapy course for health anxiety. I walk you through step by step what to do for this issue in the course. Breathing techniques are not going to help you. Go to PaigePradko.com if you need more help.

I’m going through some stress as my sons gf is pregnant and I feel she is turning him against me. I woke up in the middle of the night - heart racing, sweating. I thought I was dying. I called to my husband for help and I ran down the stairs straight into my living room table, and fell flat on there- HARD. It took a LONG time to come out of it. At first I thought is was a blood sugar thing so I drank some juice. But now - idk. Absolutely terrifying

I am so sorry, Nicole. That incident sounds scary and what you are sensing with your son’s girlfriend is heartbreaking. It is so difficult to have raised a son to the best of your ability and then have that relationship be manipulated by someone. It is truly heartbreaking. I wonder if you can have an open conversation with both of them with the goal of connecting and not accusing. We have grown sons who are both in wonderfully supportive relationships now, but we have been through some trials and tribulations with past relationships. I can relate to your post in many ways. Sending you a hug from one mom to another.

I have something similar to this but havent found anything precisely about it. I have what you described but it happens just a few seconds after I fall asleep and I wake up in panic but I'm not completely awake, I see things around me but my vision is blurry and my head feels very heavy and then I feel like I'm thrown back into my sleep just to have another panic attack right away, this happens sometimes over a dozen times until I simply give up on sleeping. It feels like I'm stuck being thrown around in a cycle, I have a thought and this image becomes a dream ( not even a scary image or dream) but it throws me out of my sleep kicking in panic, but I'm not really awake and can't help but falling asleep again and the cycle goes on.

Hi Diego. What you are describing sounds like Hypnogogic Hallucinations. They are normal and happen right when you are falling asleep. However, if your brain is now afraid of them, it could be that your brain is monitoring or watching for them, and that may create the cycle you are describing. Perhaps when you read about them, it may calm your brain down, knowing it is not dangerous.

Have they seen this in pregnancy? I had this a couple weeks ago... I didn't have a bad dream or anything... Just woke up with extreme fear and had to get out of bed because I didn't know what to do. It's was one of the worse feeling I have ever had. I did tell myself that I was safe and I'm ok.

Hi Shaun. That sounds like a terrifying experience. I cannot diagnose someone over KZread, but, I strongly recommend that you do a sleep study. This can be ordered by your physician. It may be sleep apnea or nocturnal panic attacks or night terrors. It cannot be fully diagnosed without knowing what stage of sleep this occurs. If it is sleep apnea, it is important that you get this diagnosed and treated.

I’m sorry this happens to you. It is common for people to panic even when they are relaxed or sleeping. There is evidence-based treatment to correct this problem called interoceptive sensitivity reduction training. If you are interested, I have a program that will become available in 2 weeks where I walk you through how to do interoceptive exposures.
