Neo to Neo: Dr. Tala on her favorite vent, residuals, extubating, and more!

I'm excited to have this series with Dr. Tala, from @TalaTalksNICU to answer NICU questions posted by YOU! In this video, we discuss choosing the Jet ventillator or the Oscillator for certain patients, which vasopressor is best for hypotension and why, how we manage residuals, when do we extubate, and how we deal with the emotions and conversations around death.

    I’ve worked at 2 different large level 3 NICUs. The one checked residuals prior to every feeding and like Dr. Tala said we were stopping and restarting feeds frequently which is never a good thing. They were also not refeeding residuals but just throwing them away causing the infants to lose even more nutrients along with the good gastric juices. This NICU had a huge number of NEC unfortunately. The other NICU which I’m currently working at does not check any residuals but rather puts more emphasis on clinical exam and we have very little cases of NEC. These are just my observations over the last 20 years. I have not been able to find much research to back up my claims😊🤷‍♀️ thanks again for the videos
