Navigating ADHD and Test-Taking as a Med School Student or Practicing Physician

How to Mitigate ADHD’s Impact on Your Test-Taking Performance
Test-taking for medical school and boards can be intimidating and nerve-wracking. A lot rides on your ability to accurately read and respond to these intense, complex questions and vignettes. For med students with ADHD, it can be even more daunting. But, there are ways to mitigate ADHD’s effects on your test-taking performance.
ADHD and Test-Taking in Med School
In this video, Ryan shares insights into how medical students or practicing physicians can successfully navigate ADHD and test-taking in med school and on boards. He outlines some of the ways smart students miss questions and explores specific strategies self-identified "bad test-takers" use.
"If you have ADHD along with a high miss rate (in testing), we likely understand why it's happening and how to lay out steps to start to fix it." - Ryan Orwig
Ryan outlines steps students with ADHD can take to help address bad test-taking habits. He explains the need for a structured process that limits the burden on working memory, contains impulsivity, and provides feedback.

left anesthesia school after 6 months. i would spend hours and hours studying get to my test and all the questions look like another language, i would have no clue what the question was asking, coupled with severe anxiety and down right confusion. i believe ive been going undiagnosed my whole life. The issues you talk about is probably what I was experiencing and didn’t know.

I have never related to a video more. It really effects my self esteem when I leave a test thinking I made a million impulse mistakes that don’t accurately reflect my knowledge, I definitely want to work on my test taking skills vs just “knowledge”

This video felt like you described my personal pattern with test taking over the last whole decade, something that I wasn't able to vocalize myself. I'm so utterly grateful to see this, and realize that I'm not stupid, or crazy. I can't thank you enough. If I can make a request, please can you go through some uworld questions for step 1 or 2, and highlight how to work through this problem on board exams where you're supposed to work through each question in less than a minute. Thankyou so much for making me feel seen.

This describes so well my last attempts at taking the NREMT paramedic exam, especially with the adaptive nature of the exam. Thank you so much for putting into words what I've been struggling to explain to instructors who always just ask, "do you get test anxiety?"

Thank you for using a "she" as your example; I went ages without getting diagnosed due to stereotypes that adhd is just for boys

We've worked with lots of different people with ADHD and we've heard a lot of misconceptions and stereotypes about it over the years. Glad you were able to get some answers!

Hearing about this type of gender bias practically makes my head explode. While ANYONE can have ANY type of ADHD, one of the older gender-based stereotypes we used to hear a lot was ADHD-Combined Type (emphasizing the overt, observable, classic ADD symptoms/behaviors) was the “BOYS ADHD” while ADHD-Inattentive Type was coined the “GIRLS ADD” because it was more hidden and internal and thus harder to see, especially if they were able to compensate, which is often the case with my med students and physicians (but can happen with loads of other students as well). I have worked with so many women with ADHD Inattentive Type and, for that matter, with Combined Type. The bottom line is this: anyone can have any type of ADHD. And I think it’s really important to know what diagnosis you’re carrying to then formulate a strategy to manage it. I’m glad you know what’s up now, but I'm sorry it took so long for you to get diagnosed!

Would ADHD explain why someone is a poor test taker, but can explain all of the test items perfectly when they talk it out, and not in a testing situation (ie, no time pressure, distractions from hearing or watching other students during the exam)?

I mostly miss details on the questions, or I get confused with the answer. Yes or no questions are very confusing to me because I never know what they’re actually asking. Similar words mess up with my knowledge and sometimes I have to make up acronyms and rhymes to remember them. And sometimes I think I know the answer? Just to find out I didn’t read the question well and miss small details like “no” or “isn’t”

2:57 in med school we had vivas (oral exams) and later osces with the whole class split over 2 days in shifts. Day 1 ppl would come out and post at least 50 questions each they remembered to the class facebook group to help out the Day 2, plus any corrections they may have gotten, and I still can't understand how they managed to do that. I managed contributed once, 7th semester out of 10, when I did exceptionally well and also lucked out with the format of the stations and haveing at least 1 internal proctor in each section
