My Cat Has FIP

Suppose that your cat was diagnosed with feline infectious peritonitis (FIP) and you searched the internet and did everything you could to find the latest and best treatment for FIP. However, despite your best efforts, having spent thousands of dollars on the best treatment available, your cat deteriorated and died. A post mortem on the cat revealed that he didn’t have FIP after all-he had another condition which would have been perfectly treatable by a course of antibiotics, or an operation. How would you feel? You could have saved your cat’s life, and saved yourself lots of anguish and money, if only the diagnosis had been correct.
In 2016, in the Canadian Vet Journal, veterinary pathologist Dr Cohen and her colleagues described a misdiagnosed case of toxoplasmosis, who ended up on their post mortem table. A simple blood test for toxoplasma antibodies, and a course of the antibiotic clindamycin, would have saved the cat. Immunosuppressants, commonly given to cats with FIP, would kill a cat with toxoplasmosis. A wrong diagnosis, especially one of FIP, can be worse than no diagnosis.
Or take the case of Bree, who was diagnosed with FIP and started on prednisolone treatment. His guardian sent me his details and I advised that he have an operation immediately to look for a foreign body in his intestines: his veterinary surgeon found a rubber stopper and Bree recovered after an operation to remove it. Had he continued his prednisolone treatment, it is likely his bowel would have ruptured and caused his death. Bree is alive and well 5 years later. Around 80% of non-effusive FIP diagnoses turn out to be erroneous.

    I am SO thankful for your algorithm! I work as a technician and adoption coordinator at a veterinarian . My rescue kitten Judith had a tentative diagnosis of FIP after she broke with massive fever, uveitis and lethargy after her spay. She was about a month out from Fully recovering ( or so I had thought ) from being hit by a car and having to had a leg amputated . Everyone immediately started saying FIP was the most likely . I refused to take that as my answer and In all my research came across your articles and your algorithm. Using it , I was able to rule out FIP and continue on other possible causes... toxoplasmosis being one of them . I felt so damn helpless after our Dr said FIP was the culprit and there was nothing I could do 😔 I had to admit .. all symptoms sure pointed to it. I’m constantly telling people about your algorithm and how incredibly helpful it was for me and Judith Marie ❤️

    Thank you for this most encouraging post. How long is it since Judith Marie had those clinical signs (uveitis)? I am especially glad that you know about the algorithm in your job - you'll be able to spread the message and I bet the veterinarian you work with now knows more about FIP thanks to you. I want to tell you how much I admire you for rescuing Judith Marie and giving a great home to a wee three legged cat! You have a big, kind, heart.     DrDianeDAddie this happened back in July . After she recovered from the fever and everything , the uveitis stayed. I sent her to an eye specialist and she said that the damage was permanent , and also that Judith had an ulcer ( presumably from playing with one of my other cats ) She was started on a regimen of Atropine Sulfate ointment , sodium chloride drops, and oflacaxin ,as well as Famciclovir. She also had a regimen of clindamycin. This poor little cat has been through so much , one thing after an other. She ended up having to have an enucleation done on one of her eyes because it ended up getting completely ruptured roughhousing with her best friend housemate. She is tiny but mighty! I could type for days her story of everything she’s been through in her short little life . She never missed a meal though, no matter how sick, and remains sweet and sassy as ever ! Thank you again for the peace of mind your algorithm provided . It was the first Ray of hope I had felt in the days after her initial FIP diagnosis . ❤️❤️

    Update : Judith started running a fever again.... wouldn’t respond to antibiotics ( doxycycline and Famciclovir ). She spent 10 days struggling with it ... but to no avail. Even IV fluids and anti-inflammatories and buprenex didn’t help. We sent out panels and she tested positive for feline herpes as well as positive for the 7b protein. Blood work showed elevated total protein, high WBC count , and high globumin. Serial X-rays showed a slow progression of fluid in the chest and abdomen . She still ate and drank and played... but was slowly getting worse. We tapped her chest yesterday and drew out straw colored sticky fluid 😔 I made the choice to let her go. She was tired , and nothing we did helped her. It wasn’t fair to keep asking her to fight . She passed away peacefully at home in her bed with her big brother Ghost right by . I miss her so damn much . We had 9 wonderful months together ❤️

    Hi Diane. Very interesting conclusions and cases about miss diagnosis of FIP. I would like add another point of view about this. A cat with a bacterian infectious disease. These bacterias are very resistent to anthibiotics like "clindamycin" "amoxicicline" "penicillina" and other but very very sensitive online to doxycicline, tetracycline and furantoine. These bacterias don't appear in other citologies. Thank you very much for your advises and please, you keep in mind this possibility. Bacterian infection (streptococcus) discovered post mortem in a case miss diagnosed with FIP with ALL PARAMETERS of the algorithm.

    Thank you for your comment. I didn't intend this video to be about differential diagnoses of FIP: there are so many that the video would last for weeks and, as you say, bacterial peritonitis and pleurisy would have to be included. In a future video, I'll be dealing with anaemia, which is a common feature in non-effusive (dry) FIP. Infectious anaemia caused by haemotropic mycoplasmas is a major differential diagnosis and is often responsive to doxycycline and oxytetracycline. The point of this video above is that if you don't get the CORRECT diagnosis, the treatment won't work. It's also to give people hope: a diagnosis of FIP is a depressing thing to get, but it's so often wrong and the cat may have a treatable condition or not even actually be sick. I remember two cases misdiagnosed as having FIP: one cat was pregnant, the other chubby: neither cat was actually ill! Really great to have had your feedback, Martina, thank you very much.     My vet says my cat has FIP and only has 1 week to 4 weeks to live. She drained the liquid out of him 2 days ago and said it will come back. My cat is very young about 2 years old. Is there anything I can do to cure him or make him live longer and make him happy. He is the sweetest kitty I have ever had. I love him soo much!!

    I understand how you feel, my 3 year old and she got diagnosed recently she doesnt look like shes going to make it and im sorry for your lost if he doessnt make it either. There sadly isn't anything we can do.

    I lost my baby cobi from this on 28/11/17 I was wondering if it is safe for me to get another cat

    I am very sorry to hear of your loss. If you have no other cats it will be safe, because more than 6 weeks has elapsed, but if you have other cats get them tested for feline coronavirus antibodies first: if they do have antibodies, then send faeces to the laboratory to test if they are shedding virus. Don't use Antech laboratory because they dilute the cat's blood sample too much and get false negative results. If you are going to buy a pedigree cat, make sure he or she is FCoV antibody NEGATIVE so that you don't go through this again. Would love to hear how you get on. Best wishes, Diane Addie
