Montelukast

Montelukast remains among the top 25 most common medicines with in excess of 25 million prescriptions each year. In spite of its popularity, montelukast regularly fails to provide sufficient advantages to warrant its use. Supposedly it improves symptoms of asthma and nasal allergies but studies suggest monetlukast and placebo may offer about the same degree of relief.
Accepted international guidelines fail to list montelukast as a preferred treatment for asthma and the American Academy of Allergy, Asthma and Immunology suggests not offering montelukast for allergic rhinitis.
In addition to its overall lack of effect, montelukast may cause significant neuropsychiatric adverse reactions. Postmarketing reports identify agitation, aggressive behavior, hostility, depression, disorientation, irritability, memory impairment and abnormal dreams. Although initially approved in 1998, it took until March 4, 2020 for the FDA to require stronger warnings about montelukast.
Prior to prescribing montelukast, doctors are now urged to inquire about prior psychiatric problems and direct patients to read the accompanying medication guide. Patients should be cautioned about the neuropsychiatric associations and advised to immediately discontinue montelukast and contact their healthcare worker should symptoms develop.

Montelukast has been an invaluable piece of my arsenal combatting asthma. It’s basically made my ICS work again. I never knew my lungs could open that as full as they could until I took arnuity and montelukast together…(w/o shaking all the time on a LABA). If you have allergic, mucus-y type asthma it really helps dry everything up. Oh, I also take it first thing in the morning - much better than at night - for me at least.

Montelukast works pretty good for asthma. However, taking it at night does give me those crazy dreams and neurocognitive side effects. Taking it in the morning gives it some time to clear out of the system and has less of those sleeping side effects. I will only use it situationally; less pills, the better. Also, cutting the pill in half can be a good idea and can still be effective with less downside.

Hi he said that less than 10mg is not very effective so I don’t know about cutting the pill. Everywhere I look it says to take it at night or he said evening. I wonder if he knows about the bad dreams taking it at night. I wonder what’s the benefit of taking it at night/evening

@@AJ88874 I think they recommend taking it at night because asthma gets worse at night in general, especially when laying flat, which constricts breathing somewhat. As far as cutting the pill, it still is effective for me but I'm not that big of a person. I would think it's more dependent on body weight. That being said, it shocks me that most will prescribe something as a one-size-fits-all dosage. Starting off less is always a good idea and then adding if that doesn't work. Thanks for reply. ✌🏻

@@AJ88874 Gotcha👍🏻. No, I don't take it everyday because I don't want to develop a tolerance or any toxicities long term that may result. Most doctors will disagree with this. What I do is take the corticosteroid inhaler some days, alternate it with the montelukast, and use the Albuterol situationally to not get tolerance from any of them. They have half-lifes that can extend and sometimes I don't take anything for a few days at a time. Always avoid triggers like chemical smells, perfumes, inflammatory foods helps and daily cardio to dilate bronchial passages. I also take hydroxyzine and it works on many levels including allergies, inflammation, and anxiety (all related). This one has helped the most, especially with sleep. The Dr has a video on it.
