It's a plugin (probably a custom one + Crucible, ItemsAdder, or Oraxen, but they could have made everything specifically for the video.) as well as a resource pack. A lot of what you called the "mod" is pretty clearly unmodded as it does not match the level of quality a mod could bring. It's mostly custom titles, particles, mobs that look different, items, and custom HUD (which can all be done in the game with plugins). The rest seems to be done in editing, such as doing a cut to an actually modded version of the game with the "Web Displays" mod.
