I don't have written instructions for this card and I'm sorry to say the punch is discontinued. It's called the Tailored Tag Punch by Stampin' Up! You might find it for sale on Ebay.

Nice card but I wouldn’t be able to get it on the card as well as you, have only one hand to work with, projects like this not user friendly for me but love the card! I have a hard time even cutting straight with a paper trimmer! I don’t give up though I keep on keepin on and do the best I can. Looking forward to more videos. Thank you!

I'm glad you liked the video however I don't have written instructions to this card. I can tell you the base is 8-1/2" x 5-1/2" folded in half. Other than that you just fill the front with the punched shapes and add a sentiment.
