Malunion Of The Clavicle - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes malunion of the clavicle, explains the etiology and treatment of this condition and tells you all you need to know about it.
Regardless of a sling or figure 8 strap is used, the clavicle will heal.
The clavicle will heal despite the fracture displacement.
There will be deforming forces in the midshaft area of the clavicle.
Despite the presence of a bump, fracture displacement and deformity, healing of the fracture still occurs rapidly.
Healing occurs in about 85% of the cases; however the clavicle will not look aligned due to difficulty in reduction of the fracture.
It is hard to achieve reduction of the fracture without surgery.
Without surgical reduction, the fracture may end with some degree of malunion and possible of the clavicle.
The fragments will not line up with the distal fragment appearing to be downward and anteriorly rotated.
Shortening is clinically significant because it alter the dynamics of the muscles around the shoulder.
It also narrows the costoclavicular space.
The patient may complain of decreased shoulder strength and endurance if the patient had a displaced midshaft clavicle fracture that healed with more than healed with more than 2 cm of shortening.
What are the symptoms of clavicle malunion?
- Pain
- Easy fatigability
- Cosmetic problems (especially in females)
- Neurological dysfunction (possible involvement of the brachial plexus- especially the ulnar nerve)
- Patient may have thoracic outlet syndrome.
X-ray:
- Get x-ray of both shoulders: bilateral panoramic views to measure the shortening.
- Check the amount of overlap at the clavicle.
Treatment:
- Surgical treatment of the malunion may be successful in restoring the function and relieving the pain.
- Clavicle osteotomy is done in the plane of the healed fracture.
- The fracture is recreated with correction of the deformity and the length of the clavicle is restored.
- If an osteotomy is done with reduction of the clavicle to its preinjury position and this leaves a significant bone defect, then you will need a tricortical piece of bone at the osteotomy area.

    This is very good to know. I broke my clavicle six days ago in a car accident. I went to the ER three hours later. The specialist took an x-ray of my left shoulder and said I didn't need surgery. My primary doctor requested an x-ray of just my broken clavicle. Yesterday, I got the results. It's short by 1.5 cm. I've read that it has to be 2cm for surgery. I have an orthopedic appointment in two days. I hope they tell me I don't need it.

    this answers alot of questions I have had about my collar bone. I had a head on with a tree, and the speed recorder said 56 mph. I was thrown forward into the seatbelt, thrown back and then forward again. so I took 2 hits to my left collar and 2 ribs were broken. I NEVER have been hurt that bad and, dont want no part of that hell again! 9 months later now, the left side feels out of line with the right, and at the break it feels swollen and pops if I raise my arm. in the video, it shows exactly what I feel. and I havnt told my dr. but now I think I should. because there is still pain where the break is and my entire left shoulder feels like its bent forward. I wasnt told about the muscles and tendons and ligaments had alot of healing as well. it took 7 months to be able to sleep on my left side, and even now if I lay on it to long it begins to hurt. I suspect this is my problem.

    Thank you Dr. for deciding to reach here at YTU. You described my right clavicle injury at over 90% accuracy. Anyone recalling Aaron Roger's injury on MNF can date my injury. It was the Monday am 1 week before, or 8 and 1/2 days before his. I stood there, with my arm slung, and stated before the end of the slowmo," broken collarbone". My concern with such an operation is my nerve to the hand. My injury immediately created a 6 inch numb area just above and inside my right wrist, over the left bone. It took a year to allow full extension. The numb spot has 90% healed. Your operation as described concerns me as to possible permanent or long term nerve damage.

    My clavicle bone broke when I was 14 years old. It was broken and it turned around and started poking through my skin but luckily I got the right treatment and care immediately.The displacement was fixed MANUALLY. The doctor along with some nurses pulled and idk what they did but my bone turned around to its place. Now about the breakage, I went to an orthopaedic specialist and he gave me sling to wear. Wore it for roughly two months. The bone has healed and joined but by malunion or so it is called. Since then the strength and endurance level of that arm and hand is reduced to a great extent.I can't lift heavy weights as it results in muscle getting sore and pain. But overall I am happy that my bone healed and I didn't had to go through the surgery.

    I broke mine 7 weeks ago and was operated on 6 weeks ago. Made a full recovery. started working my desk job 2 days after surgery. I'm 19 and healed incredibly fast. Already back to snowboarding. GET SURGERY. My collar bone also looks normal (straight), the scar is bad ass. The full recovery was approved by my surgeon from the 6 week post Op X-rays and ultra sounds. I did not self diagnose my recovery.

Actually I didn’t make my surgeries delicate because of the situation before of Omicron and Italy was one on the red List then The Manila Airport Int’l totally closed for European countries. But we understood very well. My Dad passed away since 1999. It’s really very Hard with me without HIM anymore especially if I am sick even headache only, he was a PROTECTIVE Lovely Dad. But I am not the youngest. Since my mortal accident happened, my cross on my shoulder as 10x more the hurtful and heavily. My Mom ignored my mortal condition. Iam begged her as Dad left many properties to us. One of house and lot if she has a willingness to help, this I have a good quality of life again and again instead. Embarrassed. I need to reopen my Cervical spine surgery because of the side effects of three times I fell in the shower box. My right lest sensibility and it happened no control. I need $6000,00 to complete the surgeries included the recovery for six months. And I pray fervently that I will be healed and ready to work lightly as a computer programmer then I am studying basic Marketing and financial. I am not aware stayin my home sweet home. I keep learning as well my English , Children here from Etrio country wants me to teach English as they still in elementary and I say a big YESSSS. Italian government stopped my invalid allowance. I wish this year, I will have because that medicine the one who help me to move otherwise I am totally paralyzed.     Im a poke dancer n I bbroke my left clavicle in a drunk driving crash it was really severe I had surgery October 13th 2021 a big ass metal plate with six screws in me forever I’ve been going to therapy almost a month I’ve improved sooo much just can’t pick nothing heavy up … I can’t wait to get back right .     Thank you, I recently had a 2b2 fracture that the doctor was on the fence about operating on. I am so glad I did, significantly reduced likelihood of mis/non union. Greater strength. I feel immediately better after the surgery. I feel like I will retain the full utility of my left arm. Your videos are wonderful even for the layman.

    Update, 2 weeks later. Have utility in my arm again. Looks and feels like it did before I broke it. Still a few weeks off before full recovery, but wow such an improvement.

    Kool Cat hey. So it has been about 2 months. Bone sometimes gets a bit sore but I have most of all of the movement back. I am trying not to push it too far as it is still healing. Scar has healed up. I have been lifting drywall and mussing. So far so good.

    @@pandacg7026 Been a year, I have all of the mobility back. It sometimes gets sore if i really push it. It is infinitely better then something out of alignment. Almost have all the feeling back. That probably took the longest. I dont regret it at all.

    My daughter just broke her collar bone and is scheduled for surgery in 2days. She’s only 20 and fell on her shoulder. The Dr.s say it is completely severed. I feel horrible for her. I wish she could’ve healed without surgery but it was highly recommended. They still say she may not have full functionality of that arm. I hope thats not the case but so far it seems like the worst case scenario. 😥

    You could leave it 10 years and still have it corrected. As long as your body is capable of healing bone then your all good :). I slept on my right shoulder allot between the ages of 19 and 21 (my last growth spert) and because of this my clavicle didn’t grow out as much as the left side which constricts my shoulder, I’m looking for a surgeon to break and reset my clavicle.

    saifu rehman: Yes but diet is very important. You need to be on a nutrient dense diet to allow your bone to heal properly and correctly. Raw eggs and raw animal fat will massively reduce recovery time and the new bone tissue will be extremely healthy and strong.

    Damn thats what happened to me 1 winter i fell towards the hockey boards the pain was so painful i couldnt put on my.jacket was out for 1 week missed 1 tourney and was back on the ice in 2weeks no slap shots tho that happened 10yrs ago now i have lil.pains here and there

    Had a malunion collarbone break 11 weeks ago. It seemed to be healing well and as of the 10th week I could do a few pull-ups (could do 10+ before the injury), but stopped because I didn't want to shock all the muscles without working up to it better. The broken tips seem to overlap close but under 2 cm, did not seem too chronic of joint pain.. Anyways I thought it would be fine to ride a skateboard... collarbone rebroke after a tuck and roll crash that would have normally been no issue. The two bones still seem attached so, more maybe it's more of a crack than full fracture. Anyways do people think im done with action sports for the rest of my life if I don't get surgery? There was already a large lump from the malunion healing that causes the area to pop out more and recieve more pressure on something like a tuck and roll (somersault), which is going to worse once this 2nd break heals. Does anyone know if 10 weeks wasn't enough for the break to harden and perhaps I just loosened up bone healing joint again? (I would describe the healing process by week 3,4 as if the lump where the broken tips of the bone meet, creates "a joint", that stiffens more and more every week until about week 7 its a rigid piece again)

    Can somebody tell me if it's okay to exercise or work out like planks and lift weights? Cuz it still hurts after 2 years, i cant sleep on my right side and sometimes my blood circulation on my right hand isn't right tho i didn't have surgery due to financial issues ( prolly don't have a plan for having surgeries but i wish i could)

    @@supriyaaaa no, unfortunately I’m still dealing with the same problem…. It seems that clavicle injuries are really hard to deal with. If you’re in recovery from the break try to stand up straight and keep your posture good. It honestly is a issue stemming from posture while it heals. If it’s already done all I can do is try to gain strength in the area and do helpful things for the pain like acupuncture.

    Can someone help me please? My 4 year old broke her clavicle (the beak is upwards like an upside down V) we were told no surgery or anything is needed. She does gymnastics and is very good at it and loves it so much. I’m just wondering if she doesn’t have surgery how will it heal? What are the future consequences? How will it look / function? It’s been two weeks and she’s still in a lot of pain. Any advice is appreciated. Thanks

    I’m wondering similarly, I just broke mine and it’s quite displaced but I’ve been advised to simply wear a sling for six weeks. I got an x ray and the bones are overlapping 2 cm, so if it were to heal that way it would end up 2 cm shorter, which could cause problems.

    My girlfriend broke her clavicle many years ago. She suffered from it healing improperly. She suffers from pain everyday. Has nerve damage, she can’t feel on majority of her Trap. And has hypersensitivity on that side of her back.

    Ya dude, I'm 48 now.....I was amazed at how well my body was ok with being broken and unuseable on one side. No consistent pain or anything.....Sure it hurt in spasms to test the joint at first ....drying off after a shower was challenging.......and getting socks on was a bitch...but overall....it was not a big deal if you could give yourself the time to heal. My bone was broken for at least 2 to 3 weeks....but then started to heal rather quickly. My shoulder now is about 1/2" closer inward and forward ....so choose your medecine wisely ...but otherwise, no real problem. Cheers bro.

    @@PhilipJosephMadarang Hey Phil, I can see that my shoulder is closer in and forward slightly.... and that is not appealing to me but overall it seems functionally ok. I'm sure there are some consequence to having my shoulder move/rotate with a shorter linkage than before but I don't do rock climbing or much work with arms over my head so I don't seem to run into issues. It may contribute to lesser posture and maybe back strain but I've always had some pains on and off in my back so not conclusive there. I do think that if I had been younger I would have requested surgery to straighten my clavacle even if I had to have a plate/pins removed later with another surgery...but it is only an assumption that I would want it removed.... because the docs told me I would want it removed. I have never had pins/plates installed anywhere so I have no direct experience with that. There are risks with surgery - bone infection might be pretty nasty but it's probably not much riskier than having a tooth pulled or studs installed for teeth etc. It would be worth finding out what those people with plates experienced.     I had z type fracture i think it heal own because i have no money for surgery and i am not interested surgery it was higher risk. And iam 20 years only i think right hand useless in my full life

    @@gopivarun90 Even I didn't opted for surgery and that I decided after seeking opinion for many Doctor's and my own research. Only extreme cases of clavicle fracture requires surgery i.e. when bone is piercing through or it's cutting your nerves. In all other cases, it heals on it's own. You need to use sling for 1.5 months to keep it still and heal and after that you need to start exercising under guidance of physiotherapist. I'm still recovering, this kind of fracture takes time to heal but physiotherapy is very important which almost takes one month to fix your frozen shoulder and i can see the progress

    I didn't opted for surgery and it's still recovering. I understood that important is to follow right process, sling and no movement for 1.5 months and thereafter, i started small exercises and recently started physiotherapy

    Can anyone help me? I broke my right clavicle back in march 28, 2022 during a car accident. I didn’t do surgery and during the early stages of the healing process i won’t deny that i didn’t take it as seriously as I should of . I wasn’t using my brace as much due to work and not wanting to miss any days. I needed the money. I was left with a permanent discomforting feeling on my right clavicle and a small bump as well. It’s completely healed but it’s not in its correct position so it’s uncomfortable 24/7. My clavicle cracks all the time, when it’s cold it hurts, and i just feel like it’s weaker now although it’s suppose to be my dominant side since I’m a righty. I just want to know if it’s a good idea to get late surgery and have them cut the bone so they can reposition it and hopefully i won’t have this horrible/annoying discomforting feeling anymore! Please if anyone has gone through something similar or the same please share with me. I can’t stay like this my whole life!

    😭I had an accident before that was 4 yrs ago and I'm experiencing now that after a couple of minutes working without any backrest a wall or a chair with backrest ,I can feel a lot of discomfort at the back of my shoulder blade and also a numbness of it and its giving me a lot of backpain ,until now im suffering😔I dont know it this really a forever problem😭Its really the same on the video😔

    So I broke my clavicle almost 3 weeks ago, the break was pretty severe and the bones were overlapped. Thee first week was absolute hell and I couldn’t really do anything but then after 2 weeks it stopped hurting as much, I could pretty much do anything that didn’t require me to lift my arm over my head. Because of this reduced amount of pain, I thought the bones might’ve started getting closer together and I thought it was healing just fine, but when I went back to the doctor he told me that the bones were still in the same place. I’m confused because it doesn’t really hurt anymore??? I don’t know if I should be moving as much as I am because I don’t want to slow down the process of the healing or risk having to get surgery. Should I just not move it as much as possible even though there’s barely any pain?

    The bone hurts as it sticks into soft tissue. That pain subsided within days and the muscles around the shoulder start to compensate and you feel support. The bone takes 5-6 weeks to find union.

    @@theking6244 even though my collarbone was broken I was still able to move my hand easily, I mostly just had trouble lifting it in any way haha I hope u heal soon man, having a broken clavicle definitely isn’t easy, just try not to move it as much as possible and eat healthy :) I feel much better now!! It’s been a few weeks and I can move it just fine and it doesn’t hurt at all anymore, thankfully I didn’t need surgery even though the break was pretty severe, I just have trouble picking up heavy things because it strains my arm where the broken bone is since it’s still healing Overall I feel great tho :)

    Broke mine Tuesday night, I can move my hand fine tho my fingers feel swollen and my lower arm can move fine for the most part, but from my elbow up I can not move anything and the pain is incredible, have ortho surgeon appt Monday the hospital didn’t say much just that I broke my clavicle, I have no idea where tho I have an idea from where it’s swollen bruising and hurting, worst injury I’ve ever had and I’ve split my eye in half almost on a tree branch and puctured my hand through a coffee mug and cut 85% of my tendon. I need to heal quickly tho so I can get back to work, any tips anyone? Also some what and what nots to do?

    @@jaykeen7163 Thanks Jay! Don't forget to take certain vitamins D Calcium, C, and K ..Im also taking magnesium. Don't over do it on D though. I'm lucky to have already a adjustible bed that goes up to sit and sleep in it helps because I can't be in any position else wise. Supposedly one more week and the major non Stop pain will be more gone.

    Hey guys I broke my collarbone exactly like the one in the video my doctor said 3 months but it still hurts when I do stuff any thoughts on how to fix this or if this isn't normal any feedback would be great thanks

    FabDino get a second opinion. I fractured my clavicle the same way as well in 1996. Still having issues. I've finally decided to really address the issue and I'm seeing an orthopedic surgeon to have it examined again after all these years

    Yea I saw an orthopaedic doctor he said that the bone hasn't healed correctly so I need to do a bit of rehab but I will be all good to go in 6 weeks ps hope your collarbone heals well :)

    i broke my left collarbone last month and it healed in wrong position now my bone is pointing upward dirction through the skin.

    I had the same problem too one month ago. It's really hurt and wondering what to do Is my bone is gonna polish itself because right now I have a lip and skin is really fragile? Am I going to get all my strength back? Will I get the full range of motion of my arm? If anyone has answers or had same problem before

    Hello Doctor...I had clavicle Right Fracture 5 months ago. I underwent conservative treatment with arm immobilizer and figure 8 brace. I am from Nepal...I have healed but I have problems in overhead activities, like I can't take off my vests on my own. I am afraid that I will have limited range of motion throughout my life. I am 40 . Where are you from doctor?. Can I have surgery like you mentioned in the video. How much is the cost of surgery in the country that you are working ?. I can not raise my right hand straight up. I underwent physiotherapy with some improvement but I guess when you have overriding anatomical defect; Physio can't help much. Please advise . Anyone who has knowledge in this field is very much welcome to suggest me some advice. Thank you

    I'm 8 weeks into healing after a comminuted fracture 4 places.. this is helpful to understand what's going on.. I was told by 4 doctors I need surgery but the ortho doc I went to told me he thinks it will heal on its own. I figured he was just saying that due to my state insurance so it wouldn't be worth it to him.. I'm able to move around now, only in pain when I wake up but my shoulder blade is constantly in pain.. I can't raise my elbow above my head either.. does anyone know what that might mean? Maybe a tightened up muscle from no use for 2 months or does it mean the clavicle is so knotted up that there's no way to do so?. I'm sure I'm not making much sense but I was looked at a few times and sent on my way. Thanks

    I broke my clavicle 33 days ago it's in 3 pieces but pain has decreased by during the morning while waking the bone that's on the top raises a little bit and after 5 min it goes down there is no pain during this . Can someone help is it healing. Am 20

    I'm 24 broke mine 4 days ago. I feel the pain has reduced but i feel the bone moving around, i hope it's getting fixed lol
