A lower eyelid blepharoplasty is intended to flatten tired looking bulging bags that form beneath our eyes. It is important to stress that as we age our lower eyelid muscle called the orbicularis oculi weakens and can create the appearance of bags and hollows. The beauty of a lower eyelid blepharoplasty is that it is commonly a scarless procedure that will produce natural-looking lower eyelids.
Dr. Kopelman discusses his favorite approach to lower eyelid surgery using a small transconjunctival incision behind the lash line of the lower eyelids that will forever remain hidden. Dr. Kopelman also teaches you how you can sometimes have too little fat in your lower lids. In these cases fat is implanted and repositioned along the eyelid hollows in a membrane called the orbital septum which will naturally fill in the “hollows” around your eyelids. Dr. Kopelman will also discuss how excess skin and muscle of your lower eyelids might need to be tightened and supported to achieve a further elevation.

Hi I have a question! You mentioned that you have been doing these a lot, so what has been the youngest age you have done this procedure to? Because I have genetical eyebags that are obviously going to worsen as time goes by and they have been bothering me ever since I was a teenager so I really want to get this done but I have read that some doctors refuse to do this procedure on younger people (like for example people in their early 20s) so do you have any insider info on this? I have read that you might have to re-do the procedure at one point if you get it done at a young age and that there are risks of having saggy eyebags (like loose skin). I watched a video of someone who was 24 and got this done and her results were so amazing that I can't stop thinking about this procedure. Her face just lit up just like that and everytime I look at myself in the mirror (especially in the car lol) I just look unwell. I would really like to look like I am not tired 24/7 but as I said earlier I would like to hear your opinion since you have been doing these for a long time. Thank you in advance!

Depending upon your problem a lower eyelid Blepharoplasty can be performed from the late teens all the way into the eighties but a conservative approach is wise in order to avoid removing too much fat which could potentially result in a hollowed appearance.
Do you think fat repositioning or fat grafting is preferable for a lower blephroplasty?

In my hands I prefer fat grafting to fat repositioning.There is often not enough fat to reposition to "cure" the volume loss
Dr. Joel... How likely is it to get hyper pigmentation under the eyes after this treatment? I had a consultation in Oslo and I was told that because I have brown eyes I could get discoloration. I am very white in my skin, and my eyes are hazel and hair medium brown. But the doctors here are used to treating blondes. So the surgeon didnt recommend getting laser to tighten the skin. Can you advice?

Nikki you could have a fractionated erbium laser treatment which can safely reduce wrinkles provided the surgeon or dermatologist is conservative and not too aggressive with their treatment.Avoid lasers that deliver deep heat intensive treatments can stimulate melanocytes to produce "post inflammatory hyperpigmentation".

Fat repositioning is a great procedure especially if you have enough fat to fill in deficits
I had lower blepharoplasty 4 months ago. I now have indented scars at the outer corners. How do I fix this?
