Low Back Pain: Myths vs. Facts | Sid Anandkumar | TEDxChilliwack

Low back pain is one of the world's leading musculoskeletal disorder and sooner or later, eight out of 10 of us will be affected by it. In the past two decades, we have had amazing scientific and technological advancements in the field of health care. Despite having multiple diagnostic tools and interventions for low back pain, it seems to be poorly managed. Disability rates have consistently increased since the late 1990’s. Sid will bust the common myths about back pain and reinforce what the latest evidence says is best for your back.

Sudarshan ‘Sid’ Anandkumar works as a clinical physiotherapist at the CBI health centre in Garrison, Chilliwack. After passing his master’s program with a gold medal and distinction, he migrated to Canada in 2014. He is a certified orthopaedics and manual physical therapist and a Pinc cancer rehab therapist, with a keen interest in finding solutions for musculoskeletal pain. Sid has been voted as Chilliwack’s top physical therapist. He is also active in academic research work and has published many articles in international peer reviewed journals (indexed in Medline).     Unfortunately a nerve can get trapped or compromised leading to serious pain , and resulting in nerve blocks etc. It's life changing when you go through this pain. Hopefully no one experiences it like I have. I'm a trained NMT. Neuromuscular therapist . Someday maybe I'll do a TEDx Talk. I'm trying to beat L3,4,5 nerve pain radiating to groin , leg, feet that have excruciating pain, whilst dealing with Colon Cancer and operation with Chemotheraphy and now while dealing with virus problems I've developed neuropathy of hands and feet from chemo and that could last a long time. Laugh though everyone if you can . Enjoy life and hopefully that's how I'll get better. See you all soon on KZread 👍👍👏

    I'm so sorry you are going through this, Wayne! I totally agree with you that there are many legitimate cases that cannot be treated or cured by this speaker's solutions. That being said, I do think it is important that we do try his advice as a first line of defense, which in my case and many, many others, did not relieve pain in any way and in fact, in some instances, made it worse, like going to a chiropractor.     Hi Shiftylad. I have cancer in the abdomen and it creates back pain. I want to share with you what has helped me. You might not believe this but try it. You'll discover it works. Pain is actually liquid inside the body. In your situation, when a nerve gets trapped or compromised the body can sweat and it's this "inner sweat" that causes all the pain. Eventually, in most cases like in exercise, the liquid dissolves and the pain disappears. For people like you and me, our body is continuously internally sweating and generating continuous pain. Copper is known to dissolve liquid sweat inside the body. I paid $25 at the hardware store for a 4-inch piece of copper pipe. I work walk sit all with this copper pipe at waist level in between my belt and my back, against my skin/spine. This in 5-minutes continuously removes the ongoing production of inner sweat in my body, reducing my pain by about 80%. You may have heard of TOMMY COPPER - I think that's the name. They make clothing with copper built-in. Anyway, a piece of copper pipe works just as well is a lot cheaper and will last forever. Place the pipe against the base of your spine and give it a good 5-minutes to generate results. Give it a try and then spread this natural medicine cure. Good luck man. Peace

    Man.... I am so glad I found Dr. Stuart McGill and his book Back Mechanic. I suffered from lumbar back pain for 10 years. Everyone in the medical field failed me. People like this guy. So much misinformation that just leaves people stuck in a vicious cycle of pain.

    Disc can't slip but your vertbrae can move enough to touch a nerve? I can assure you if you get a ruptured disc that keeps you from walking and sleeping you would say oh I am in the 10% that needs surgery. Don't wish it on anyone but until you feel the pain that some of us have felt you will never know and can't even imagine and your whole speech here would make you sick everytime you watch it. I understand your speech and it sounds good to anyone who has never had to endure extreme sciatic pain. By the way crunches are one of the worst exercise for low back and depending on the injury yoga too.

    I respect your experience. I, too, was a victim of "ruptured disc" and it happened when I was carrying a heavy wheelchair while going down the stairs. I slipped and because I didn't want the wheelchair to fall off I tried to lift it as hard as I can and I heard a loud pop on my back. I was sure that came from my spine and aside from it I felt excruciating pain on my low back so painful that I couldn't walk. I was rushed to the ER and MRI showed a disc herniation in between L4-L5 vertebrae. My hurts for like a month. It was hard to stand up and walk, but I didn't take any medications nor underwent any back surgeries. I just did exercises and eventually, the pain subsided. There are a lot of studies, journal articles about conservative intervention and surgery. According to the studies, surgery is not superior or is equally effective as conservative intervention like PT. Yes, it is painful and we can all wish that pain will just disappear instantly, but our body is capable of healing. It will take time, but it's better than spending too much money and not having the assurance that it will disappear completely. Even if surgery is done, we're still at risk of getting low back pain. It is something we can't avoid, we don't need to just live with it. We need to live through it when it happens.

    @KB Adams I HAVE HAD CHRONIC BACK PAIN FOR FORTY YEARS , WHEN I WAS IN MY TWENTIES I MANAGED, BUT NOW I AM IN MY SIXTIES , IT IS UNBEARABLE DAY AND NIGHT.MIR SCANS SHOW I HAVE NO DISCS LEFT, ALL DEHYDRATED.

    Its NOT your vertebrae which is touching the nerve, or more specific the "scatic nerve", it is the nucleus pulposus from the disc which leakes out from the disc and then touches the nerve which afterwards brings pain down the legs and sometimes the foot. To summarize, the disc CANNOT slip, it can get a crack, but its to much ligaments surrounding the disc for that to happen.

    My back started to hurt after three days of doing NO EXERCISE (holy week Wednesday, Thursday and Friday). Before I was doing heavy ones. So what made it hurt is TOO MUCH "RESTING" (sitting specially), and what I needed was ACTIVE REST (doing light exercises). I also saw another video where hanging can help. We used to "hang" a lot in ancient times, by climbing, from trees, etc. I went to the gym on Monday, did light exercises, and then rest for two days. My back improved greatly. Today it started to hurt again. It's because I had to rest for just 1 day, not two. I'm going to the gym again today to help my back recover. I'm doing something similar with my shoulder. It's working.

    Knowledge yes,,,, but does such knowledge give back quality of life to patients? Allow me to doubt it! The question to this is : How many human beings have you healed sir, with your knowledge? Back pain is the simplest thing to understand...and to Heal. Such speech, you can find anywhere on earth. Unfortunately, that is the kind of understanding that does not give back peoples their quality of life.

    By my own experience, strengthening ur core by moving ur hips and legs etc... is the cure. I do a lot of jogging and other sports but if I stop training, stretching my core I will get back pain again.

    There is no studies suggesting that core exercise will help with chronic lower back pain, people who suffers from lower back pain are already training the core. There is no causality and core strenghtening for lower back pain is just another myth going viral around the world with no support at all.

    apart from exercise and keeping your weight down and building up good core strength...DONT get roped into lifting heavy and awkward loads. If your gut feeling is telling you the lift is too risky, then listen to it and avoid or find a better way of doing the task. You will thank me later.

    Hmm, this is not it. 1. You're inducing fear by saying you shouldn't lift heavy, which you should lift heavier loads AT THE RIGHT TIME. 2. Your body is resilient & adaptable. We treat it with delicacy like a flower & that's exactly why a lot of people are in pain.

    Little hard to understand, but Very helpful. I'm cancelling my appointment for next week. I'm no lying; he's the second person in the medical profession that said the body heals itself. My back gets so stiff sometimes I've had to go get an injection of cortisone. But recently I've found that just gritting my teeth and walking it out works the same. And laying in the bed for a little too long is exactly another cause of this annoying ailment. So get up, plug in the Rocky theme song and fight!

    It doesn't matter the color of the box, they're all myths, from "complete bed rest ir essential for back pain" to "core exercises prevent injuries". I agree that different box colors is confusing, but in conclusion: every statement inside the box is wrong.

    Sweet talk, it gives hope, but i dont think you have ever had ruptured disc, you have never wished god to can stand again on your feet like a normal person without excruciating pain and sweat. Its not because of stress or tension or all the psycological factors that you think it is. Its physical. Its a big nerve thats pressed. Its real pain and its not fun and its gets worse when you bend. Its not a myth. Its a fact. And it matters how you sit or stand or every posture matters for the vertebral column. Its a fact. At least for the patient that suffers from back pain. Its a myth for you and alle the others that have not experienced a real back pain(not psychogenic pain).

    I also suffer from pinched nerve and spinal problems,,, this pain is real but i have a question many people say its pyschogenic,,, there got to be many people who healed due to this thats why this concept exists,,, so be open-minded and let everyone find the way they want to be painfree,,,bcz even after surgeries people are in pain which means so called physical treatment also failed ???

    Hi Nima Mpour. I am very sorry for your condition and hope your pain has improved over the following months. I am a chronic sufferer of low back pain and have experienced the excruciating pain you describe. It is an appropriate term, excruciating. I also am a PhD in physiology and I happen to teach physical therapists how to keep up with the scientific evidence regarding the causes of musculoskeletal disorders and the efficacy of the treatments. The pain is real, very real indeed. There must have been a misunderstanding about the origin, actually it is called CENTRAL SENSITIZATION not psychogenic pain, psychogenic pain is in you mind, central sensitization refers to REAL pain without any tissue damage. EXERCISE IS POWERFULLY THERAPEUTIC FOR THE SPINE, so much that guidelines insist we move even in pain (except for people with serious damage, and i hope that is not your case since herniated disks are much more common than one would think) From your description i can infer you are one of the small 4% to whom surgery would be beneficial. One last interesting fact: psychogenic pain can, indeed, be as excruciating as real tissue damage-inflicted pain and the scientific literature is very consistent in this. I know this is counterintuitive, its one of the many unknowns of the brain

    @@tomdurkins my thoughts exactly during the entire talk…”promoting physical therapy” which isn’t a bad thing but imaging IS so important in a lot of cases. They wouldn’t know what to treat with PT without knowing what was going on with soft tissue AND could actually make things worse. That’s JUST ONE example of how people could debunk some of the topics he spoke about to promote PT.

    @B. L. The thing is that most surgeries in the back fail. Chronic pain does not really correlate to any structural issues. Many people have herniated discs or degenerative changes but do not have back pain. As we age, there are changes in our body and degenerative changes in the spine are like having grey hair. It is expected of aging.

    I have been teaching this stuff through my videos on my channel mission pain free for Indian fellows in Hindi, logo ko viswas hi ni hota ki brain can cause pain in the body, almost all the chronic pain is caused by the learned pathways by the brain, I have healed my spondylosis using this approach.

    I do question if this is just a boost to keep physical therapists working. It's been over 20 years that I have suffered and I have without a doubt tried everything, from surgery, to physical therapy, to regular routine exercise, to chiropractor, to acupuncture, to yoga, to meditation and the ONLY thing that has relieved my chronic pain is pain medication. It doesn't get rid of the pain, but it does provide a life where I can at least 'manage it'. That is now gone because of 'The New Guidelines' to the point of being non-functional.

    A very comprehensive explanation, Thank you. If you're struggling with pain that isn't going away and considering surgery, I highly recommend trying to find an adhesion doctor near where you live. Adhesion treatment often gives PERMANENT pain relief to those who have had more than 6 months of pain and have seen a few doctors.

    @B. L. Hi there! I'm discussion "scar tissue" or adhesion in the muscles of the low back, psoas, nerve roots, and joint capsules. Ha, agreed. Our practice specializes in only taking chronic pain patients and providing permanent relief. I never heard of Racz procedure!!! This might be a good option for patients we can't help.     @B. L. I hope it works for you my friend. I will definitely look into it. If you can, look into muscular adhesions as well. We don't need to do any surgical intervention to get most of our patients permanent relief. We had a 94% success rate in 2019.

    Exactly. It's a shame this guy didn't mention the revolutionary discovery that the underlying cause of back pain is muscle spasms due to stress/emotional factors. But at least he educated people on the myths about "structural" or "disc" problems, which are unfortunately still causing mis-diagnoses today. How sad to see the "quack" comment below...Sarno was decades ahead of his time and discovered the most effective treatment for back (and other chronic) pain. I know dozens of people who have permanently recovered from chronic pain by reading his books (including me).

    Very articulate speaker. Nice simple explanation of backpain issues and treatments sans surgery. VOMIT the acronym for Victims of medical imaging technology seems fitting, lol! Motion is lotion for the spine. The Brain has the capacity to deliver enough pain reducing and eliminating signals etc. wonderful wholistic self healing techniques and answers to back pain. Statistics seem accurate too, for the number or percentage of the population suffering from issues related to back pain, soreness and other discomforts. He clears up the differences between Myths and reality. Heavy Indian accent could be good or not, depending on the listener. Enjoyed it!

    This was excellent. If you want citations for what he's saying look up physiotutors on back pain myths. As an individual you always might be in the minority. 4% of the global population suffering from back pain is a large number - you may fit into that. These talks are always broad brush - they have to be and they'll be correct for the majority. I think one of the fundamental reasons for backpain increasing in the west is the other issues with western living - sedentary lifestyles with weight gain. Motion is lotion is bang on. And that's not just doing gym work 4-7 times a week. It's moving all day.

    Agree Stu McGill does not believe or at least acknowledge a great deal in pain science. I have a strong feeling that will change in the near future as he can’t ignore the growing overwhelming evidence that is out there. Room for both the CFT & the RI approach in tx of LBP.

    Dodger Palmer you should. I have mastered mundane astrology. Now learning medical astrology. My PhD is in bioengineering and I also talk about such topics in my channel. Modern science doesn’t know WHAT it doesn’t know and has the arrogance and ignorance to assume the opposite. I am ashamed of my old self after my research into astrology and occult. Ashamed of it. These stuff have so much merit. So much. And can do so much good for humanity. Yet we have dismissed them without proper research and I talk about the FEW so called research against astrology from the 80s which should make any real scientist ashamed of ever citing such flawed unscientific publications ( the 2-3 that we do have, yes. That few).

    Dodger Palmer right now Saturn entered Aquarius. Expect all sorts of issues with legs and ankles. In so many people who have major placements or angles in Aquarius. I have followed these for years. Re digestive issues and food allergies, Always suspect major virgo placements. And moon in virgo and scorpio as well as moon in 6th and 8th sectors of the zodiac ALWAYS are prone to gut related issues. Food poisoning. IBS. Gluten allergies. Etc. Damaged Jupiter? Always gives you issues with liver ( and sometimes gallbladder) Damaged Venus ( kidney issues, lower back issues, reproductive issues and always diabetes is a huge huge huge correlation) Damaged sun ( heart) Mars with sun ( ALWAYS high blood pressure. You can never ever change this w diet. ) Damaged mars? Blood disorders Damaged Saturn? Bone, knees,Teeth, skin issues. Major Taurus placements? Check your thyroid. Active strong mars? You’ll always have migraines. Always. Mars w nodes or mars and moon, or north node in scorpio or aries? Suicidal tendencies ( I have lost count on the number of sucides who have this combination, it’s shockingly common). And Uranus with moon. Omg. Close to 90% in my research, I have seen this with bipolar patients. Moon in Aquarius. Moon hard aspects w Uranus. Lesser degree moon opposition with mercury : bipolar Moon in Capricorn or moon with Saturn: 100% of the time clinical depression. Lilith in 5th house of children: paedophilia or child abuse ( scary common, I researched this but need more data). Also extremely important in psychological health is Ceres. I have a detailed video about this and analyse placement of Ceres in several famous people. Crucial in healthy parenting. Etc

    Bruh, this is such academic bs man. not one person i know has healed, per say. It's been 4 years since i herniated mine and i've done literally everything. Even lost 19 kgs but news flash - the disc will never regenerate and it is not going to heal. The pain might reduce but it'll come back. fml

    Profiles against & discriminated against? Why because pain science doesn’t fit your self efficacy or lack of it? Your attitude possibly contributes to your situations. Because science differed from your perception doesn’t mean science is discriminating against you. Science is clear, you don’t believe it that’s your decision but crying discrimination isn’t helpful for you or anyone.

    I do believe there's an epidemic caused by lack of regulation and control around how it's prescribed. Pushed out by greedy Pharmaceutical companies to people who actually never needed it to begin with. That's one reason why you are getting hit hard now.

    Another worthless video. I’ve been in pain do 11 months and physical therapy is not helping. I have enough of these bastards telling me that it will heal, that PT helps… I just wish that all those experts had disc herniation so that they would know what sort of pain we are talking about

    The usual speech on pain.... That leads to millions of peoples with important reduction of quality of life. Yes, lots of what's said here may sound true. But, to the ears of a hands-on therapist with over 25000 hours on giving back full quality of life to thousands of individuals, such a speech sounds so misleading. A speech that mostly serve peoples not having the patient's interest in mind, or worse, at heart. Guess who benefits out of such paradigm. To the contrary, if you're the type of human being caring about your own well-being, and other', know that Pain is simply a language; a means of communication from your body to you, living in there. As much as thirst asks you to hydrate, feeling cold asks you to cover up (keep your body temperature balanced), too hot, the opposite, and feeling exhausted, to rest, the signal of pain is asking you to bring response ( to do a simple action) to a physiological need. Ex; keep your arm up in a steady position to the front of you for 5 only minutes...... and tell me now, that your shoulder isn't asking you to bring the arm down. Muscular fatigue asking for some rest. Do that for 7-8 minutes at the time, multiple times a day, 5-days a week for months..... Then, tell me that it is your brain that is reminding you about an ancient pain.. Tell me that such a lack of physical awareness can not create yourself a chronic pain..... If this last, what kind of compensations will take place to build subsequent various pains in a single human body? ALL OF THAT BY SIMPLE IGNORANCE.
