Everyone should have their Lp(a) tested once. As a primary patient with no other obvious risk factors, I was surprised that my ApoB was 120. My primary was not even sure I needed a statin because my LDL-C (tested three months later) was 107. Fortunately, I tested my Lp(a) which was 280 nmol. The statin dropped my LDL-C to 79 and ApoB to 80. Without the Lp(a) I would’ve been satisfied. But with this knowledge, I started a PCSK-9 inhibitor.

have you checked ur ldl-p numbers and sizes? my regular lipid panel is good and stable, apo-b and lp-a are good, but ldl-p number and size are moderately bad!

@@belly4608 I have had my ApoB checked which gives me my particle number. Particle size is not particularly relevant since all LDL-C particles of any size are atherogenic. People with FH tend to have large fluffy LDL-C particles and they have the highest risk and worst outcomes.

@@davidzip8841 my Lp(a) is a bit higher than yours. Just started Repatha a few months ago to get my LDL under 130. I was always 131 or 132. HDL 80, Trigs 80. Very active lifestyle, low carb/keto and gym 4-5 times a week. I've been watching to see how the trials go for he meds that can drastically lower it. (And I'm sure once we lower that, science will find yet another risk I can't control!)

@@davidzip8841hi David. I saw your same reply on another Lp(a) video. It looks like these talks are starting to hit mainstream cardiology. My Lp(a) is 234, but my cardiologist just has me waiting, no statin or anything. How did you get insurance coverage with no heart problems? I didn’t think PSK-9 was covered for prevention.

i’m waiting for my lp(a) results. i had a stent put in 3 months ago 100% blocked LAD. my arteries all over my body are clean including my brain. my glucose tolerance test had a max of only 5.6, just because i don’t have any signs of diabetes might mean my lp(a) is high. my apo and ldl are in the lowest levels now with statins and post stent. maybe diet too .. 20 mg crestor. i’m tracking everything and am requesting my own bloods!

I would be very interested to know if the trials used a cohort of randomised 'healthy' people or were these people selected from patients already presented for cvd or or lipid monitoring?

hsCRP level affects outcome in high Lp(a) individuals, both hsCRP and hsCRP are acute phase reactants, so the levels change, in spite of genetics. It seems without measuring acute phase reactants, Lp(a) measurement may not reflect attainable baseline levels . Both statins and lowering Lp(a) increase incidental diabetes, so is there a common element ?
