Leaky heart valves
Today’s video is on the subject of leaky heart valves.
We are blessed with 4 heart valves - 2 on the left side which are known as the mitral and aortic valves and 2 on the right side - the tricuspid and pulmonary valves. Sometimes due to a congenital problem, age related changes or even an acquired cause, these valves can start leaking and when they do so to a severe extent, they can adversely impact on a patient’s quality of life as well as their lifespan.
The adverse impact of leaky valves is dependent on the severity of the leak as well as how quickly over a period of time that leak has developed. What I mean by this is that if a leak has slowly developed over a period of time, the heart may have time to adapt and therefore patients may not notice anything until the leak is really advanced. On the other hand, if a leak has developed very quickly then the heart may not have time to adapt and therefore the patients may become symptomatic even when the leak is not as severe.
The main problem is that leaky valves over a period of time cause a condition called volume overload meaning that with leaky heart valves, the overall blood volume that the heart has to contend with over a period of time increases.
I will explain why this happens - my explanation is probably a little oversimplified but hopefully it will help with understanding the basic concepts.
Let's say the heart hypothetically has to pump out 100 mls of blood with each heartbeat (these are just arbitrary figures) and 10% leaks back. Well now only 90 mls of blood get pumped out. And our kidneys only receive 90mls instead of 100mls. The kidneys will then behave as if the patient is dehydrated and therefore will try to absorb more water from the urine to try and increase the blood volume back to what they expect. So again very simplistically, they create another 10mls of blood. So now we have 110 mls in the system but again 10% of the 110mls leaks back and the kidneys still get less than what they were expecting and they then again try to absorb more water from the urine and produce more blood. So in essence there is a vicious cycle that develops with less blood going to the kidneys, the kidneys increasing more volume within the system and increased leaking of the heart valves. This is why this condition is known as a condition of volume overload - slowly the amount of the blood in circulation increases.
Now the problem is that the heart eventually has to contain this blood and as the amount of blood is increasing, it causes the heart to stretch. The heart behaves like a rubber band when it is stretched, When you stretch it, it twangs back with more force. If you stretch it even more, it twangs back with even greater force but if you stretch it too much then it loses its elasticity and therefore becomes weaker. So as the leak progresses, the heart responds to the volume overload by contracting with more vigour but if it is left unchecked then eventually it starts weakening and this is something you want to avoid because you may not be able to regain that strength again even after operating and correcting the valve problem.
So if you have a very leaky heart valve there are 2 main reasons to consider an operation
If the patient is symptomatic from the leak and the symptoms are usually breathlessness, exercise intolerance and progressive swelling. The patient is breathless because he is not able to push out as much blood as the body needs because so much of it is leaking back. He is intolerant of exercise for the same reason. He swells up with swelling of the legs and the abdomen because of all this volume overload. So if the patient is symptomatic, then the valve needs fixing.
If the patient is not symptomatic but if we see that the heart is beginning to weaken then again the valve needs fixing. How can we tell that the heart is weakening? In leaky heart valves, you would expect the heart to beat with even more vigour than normal. If the heart function on an echo looks normal or even slightly less than normal then this is suggestive that the valve needs fixing.
This is why when you have a leaky valve, the cardiologists will organise to see you at 6-12 monthly intervals where they should do 2 things:
Ask you about whether you have symptoms
Do an echo to see if your heart is looking weaker.

    When I was a kid he said he could hear the swishing. I'm 73 and still alive and never had any problems as far as I no. Never could ran no more than a mile. But I haven't been to a doctor in 60 years so I have no idea how my heart is.

    I've had swishing since I was a kid to I'm 48. I workout like I'm 19. The doctors found my murmur accidentally & can barely hear it.. They monitor it but i have no symptoms except occasional palpitations that ladt a few seconds. Sometimes I think doctors just want to keep milking us. Dr. Gupta is a great help 👍

    Thanks for that clear explanation. A few years ago an echocardiogram showed that my mild to moderate mitral valve regurgitation had progressed to severe. After expert evaluation, A MitraClip was recommended and placed via the femoral vein in the groin. It was so easy from the patient’s point of view, with only one night in the hospital and no incisions. Three and a half years later it is working perfectly and I am very grateful.

    @@xymonau2468 I had to be on aspirin short-term, until tissue grew over the clip. I was already on an anticoagulant for Afib.

    @@sushilaananda3365 Thank you. The reason I ask is that I do have issues with my mitral valve, although they have been very stable and even improved with a change of lifestyle. I also have afib, but blood thinners give me unbearable nausea. I wondered if they were required because a clip is not tissue and might cause clots. I can take aspirin when I'm not on blood thinners. Anyway, hopefully it won't get to that stage, but it's good to knw what might be required. Thank you so much for your response.

    @@xymonau2468 Most people who have Afib take an anticoagulant due to the higher risk of stroke with Afib. If you have tried the newer anticoagulants like Eliquis etc., and do not tolerate them, you might be eligible for a Watchman. I will be evaluated for a Watchman in the next couple of months. This is a device that is placed over the left atrial appendage, one of the main sources of clots.

    @@sushilaananda3365 I was hopeful about that device, but many people in the US are suing the company who makes it, so do your homework and find out why. Also, I'm unsure if they give them to people with valve issues. I did read up a lot about it at one time. I'll take my chances, and continue to work on my lifestyle issues. That has helped a lot. I can tolerate one or two doses of Eliquis if I have to, so I can have an electric cardioversion, but they make me want to vomit all day. They also affect my emotions so that I can't cope with stress as well.

    I have just watched your video and amazed at how poorly my leaky valve has been dealt with. My leak was discover 10years ago, I have had 4 echocardiogram done in that time, but have never seen a cardiologist. Recently after a bad viral infection I have become symptomatic: attend local hospital to be told I had heart failure and started me on Ramipril and discharged same day. I was told my wait time for a cardiologist appointment was 50 weeks as I am classed as routine. So I have booked to see a private consultant to discuss things further I hope she is as helpful as your video are. Thank you

    Hi, Not sure if you'll pick this up, but I wanted to ask how things have progressed. I hope you've seen a cardiologist by now and are getting treatment for the leaky valve. 50 weeks is an insane amount of time to wait for a cardiologist appt.

    I have stage 3a kidney disease diagnosed two years ago. In that time my doctor has explained very little, but in watching several of your videos l have noticed so many things coming together and answered a myriad of questions for me. I’m in Australia and have struggled to find a GP that will tell me more and not just offer one pill on top of another. Now our bulk bill system is fazing out and although l have reached the stage of many unexplained pains within my body, particularly my legs l now find l cannot afford to visit my GP as often as l need to. Thank you for giving your time to explain so many things to the general public. I wish there were many more Doctors like yourself in this world, dedication is a dying breed, you are one in a million…well maybe one in 500.     My son had the transposition of the arteries when he was 2 weeks old .he had stents to the lungs 3 years ago..wow we have had a rocky 20 years I want to say ..but his doctors here in south africa are amazing..now last year we came to know he has a leaking valve..he is 20 now ..a walking miracle..for us all..it's doctors like you ..who give us strength and courage to know our kids are safe and in good hands ..thanks for the video..it helps us understand better ..blessings from South Africa.     I came across a video by Austin Goh on youtube - 7 exercises to help with leaky heart valve - or similar title. He also has exercises that may help with heart inflammation. He is Chinese - very helpful and positive nature. Love watching his videos. Also look into Ayurvedic medicine and herbs that may help with heart inflammation. Also I take Turmeric and Ceyenne Pepper. Try taking taurine 500mgs, magnesium citrate 100mgs, and potassium citrate 99 or 100mgs 3 times a day and one dose before bed. Also try 100mgs of co enzyme Q10 or CQ10 plain 1-2 times a day. Also look into Acupressure. There are points you can stimulate or put magnets on that may help. On youtube there are many acupressure videos. But before you try any exercise please look into it first

    Thankyou so much! I had palpitations almost every day constantly for weeks, I was sent for an echo and diagnosed with a leaky valve and put on bisoprasol which Has worked wonders. Although I did not understand at all about the leak and wondered what it was all about....now I know!!! Thankyou so much!!!!!

    I am so glad to have found you, and have learned so much from your videos. I have a pace maker, and have had some problems with irregular beats, but am doing pretty well now, after taking it upon myself to take electrolytes. I have definitely had breathlessness, and weakness as I try to exercise, and tiredness. I felt pounding in my head, my feet, and hot and cold sensations. It's much less now, after taking the electrolytes... I think it is the potassium that makes the difference. It was pretty continuous, and now it's much less. Thank you for your explanations of these issues. I know the doctors have only a little time with me, and the rest is up to me, so I try to be informed. What you're teaching goes in the bucket of information to help me ask the right questions when I see them. Thank you. Lynn

    Have had regurgitation for decades and have enlarged left atrium but not serious enoughv for a replacement or fix. This is the first time I've heard about blood volume increasing due to regurgitation. That explains my enlarged left atrium. Thank you for this clear explanation

    Thank you so much for the explanation doctor. I was diagnosed with a leaky valve (left side of my heart) many years ago. I was able to see the leaky valve while having the echocardiogram procedure. That’s when I noticed the blood flow going back out and I asked the technician what was I seeing. Anyways, my physician just told me that when the time comes/if gets worst that’s when it needs to replaced. I have much more understanding watching this video. ❤️

    A cardiologist told me it was like a rubber band and it could just snap one day. Hm..he suggested a pacemaker so when you mentioned rubberband, it was aha that's what he meant.     Very helpful explanation, thank you. I had a mitral valve repaired six years ago, and now occasionally check myself with a stethoscope and a small ecg machine, have a fingertip monitor also; good to be able to check for myself that all is well and still not deteriorating.

    @@Thesimplelife00 Nice to hear from you. The ecg is a 'Heal Force PC-80B", made in Shanghai, about U$120 4 years ago, from Ebay I think. Very useful for seeing if I'm still in sinus rhythm etc. The stethoscope is a basic Chinese one, traditional pattern, $20 from a local stockist, valuable for directly listening to the sounds; it's obvious when there's a leak, as that makes a woosh sound instead of a regular beat. So far, no problems! Occasionally I also check oxygen level with a fingertip monitor, and blood pressure and beat rate with an Omron unit from a local pharmacy. Completing my instrumentation armory is a Fitbit wrist monitor, a fabulous modern instrument with many uses, including logging. All this is made more necessary because my surgery was the old-fashioned way, maximally invasive (not using a DaVinci robot), and the big incision through the heart muscle left me with poor rhythm, susceptible to AF etc. if I'm not careful with lifestyle, including diet (excessive tyramine is risky). It's great, and simple, to check myself with these inexpensive instruments. I hope that helps you.

    Fantastic explanation Dr. Just a newb question though? Has there been any studies related to Phlebotomy and having a less stress on the kidney and/or heart to work harder to expel blood?

    @@YorkCardiology dear doctor, im in a panic attacks since I discovered by chance, I'm 35 years old with two kids, 5 years 7 months. suddenly I knew that I have Trace mitral regurgitation, with every thing else normal. ( Trace mitral regurgitation with no physiological significance ) Doctor's report. And told me to check after 5 years?!!! Since i knew I got panic attacks and all day crying after I made tons of searches about the topic ... so scared will it gets worse 😫 😞 😢 it will definitely lead to moderate then severe then.....complications at the end.. right????? by how many years this will happen ???? Plz plz inform me I'm so panic 😭 I don't exercise or hard working to be tired, I'm healthy, I'm slim . But I stress a lot and worry a lot. Why did my doctor said this in a cool way!! As if there is nothing??? Why the check up after 5 years???? Is thus really normal??? Will it be moderate or severe after 5 years . Plz let me know I cry 😢 all days and nights because I was living a normal life now I am worried a lot a lot . Thank you I advance

    @@YorkCardiology Hello. My cardiologist labeled mine as Trace TV & MV not Mild but Trace. It read: The tricuspid valve appears normal/there is a trace tricuspid regurgitation. Same with Mitral Valve.

    I wish I found your video sooner. I was denied a second Echocardiogram for two years after symptoms led me to discover I had mild mitral and Tricuspid Regurgitation May/June 2021. Being told my symptoms of chronic fatigue, exercise intolerance, and shortness of breath just walking to kitchen to eat was not heart related by my electrophysiologist who refused to test me or notate my worsening symptoms as I continued to get so much worse. Denied a second look even after my EKGs started showing right atrium enlargement in May 2022. I had to leave him as my provider so finally at the end of next week I finally get my second Echocardiogram. I won't know until then how much damage was done during that time I can only tell you simple acts of getting dressed, preparing a meal for myself, or even showering is almost impossible some days more than others and all my cries and pleas have fallen or deaf ears until now.

    I came across a video by Austin Goh on youtube - 7 exercises to help with leaky heart valve - or similar title. He also has exercises that may help with heart inflammation. He is Chinese - very helpful and positive nature. Love watching his videos. Also look into Ayurvedic medicine and herbs that may help with heart inflammation. Also I take Turmeric and Ceyenne Pepper. Try taking taurine 500mgs, magnesium citrate 100mgs, and potassium citrate 99 or 100mgs 3 times a day and one dose before bed. Also try 100mgs of co enzyme Q10 or CQ10 plain 1-2 times a day. Also look into Acupressure. There are points you can stimulate or put magnets on that may help. On youtube there are many acupressure videos.

    I've been following your channel for 5 years now. I first became interested when I was searching for information regarding PVCs/ectopic beats. I recently had a stress test done. It came back all clear but in the write up the cardiologist noted I had non significant (>1mm) Upsloping ST segment depression. After googling for days I've read numerous reports that an upslope st segment is indicative of Ischemia. I wish I could find answers somewhere.

    thank you doctor for a reassuring video. i have an appointment with the surgeon next week as i'm about to have aortic & mitral valve replacement surgery but just nervous as i'm 61 male and type 1 diabetic. Does this make it more risky having diabetes?

    At 6.20 in the video you said I quote. "If the function on the echocardiogram looks normal or less than normal, this is suggestive that the heart valve needs fixing". What does this mean, if the test was normal. Don't understand that comment. I get all the rest of it. Thanks

    I have this and it was so helpful. Thank you! I was wondering why I am scheduled to see my cardiologist every 6 months, even though I currently feel good. I also have sleep apnea and that affected my heart as well, even though it was mild.

    Dear DR. This is exactly what I have, mitral valve leak regurgitation. 65 F. 48 kg. 5.4f I’ve been on beta blockers for three years. Now the doctor is putting me on statin as well for cholesterol but it’s giving me muscle pain so I’m cutting them down myself and adding natto beans supplement. I will know by end of this month if it works. He also prescribed amitriptyline for years which causes side effects and I have stopped taking for now. But he kept insisting. Anyway my heart leakage is diagnosed moderate to severe. I try to get an echocardiogram every one and a half to two years. My doctor hasn’t said I needed an operation yet. Wait and see as you said. I see him briefly every 3 months. I get tired easily at times. At times I can walk for an hour with no symptoms, but sometimes walking in the heat or in a building I get breathless. Maybe it’s the trapped air and I’m not sure. Lately, and especially after Covid vaccines, I get more irregular heartbeats, more often now I could hear my heart sound like a tugboat…like drrrrr drrrr all the time. I read that irregular heart palpitations sounds scarier than it really is but bothers me nevertheless. Yesterday the heart suddenly beat very very fast and I got dizzy. I sat down and did some deep breathing exercise and after a while it went away. Not sure how much I should panic. I live alone so sometimes it’s worrisome. If there’s an emergency of condition should I take something immediately to prevent a heart attack? Aspirin? Or? Death is part of nature and I’m not afraid of that I just don’t want to be dead for days without anybody knowing. Not a good sight. 🤭 Thank you very much DR. You’re one of the very very few physicians that care from your heart. No wonder you are a heart doctor. Stay well and happy. 🙏🏼💕
