If you see carefully, then all three upper moons were so powerful that 2 to 3 hashiras together also were not able to defeat them. And logically they even can't. It was always some outer factor that cause their defeat. Akaza = previous memories Doma = poison Kokushibo = previous memories. and realization And even in case of muzan, the poison made by Tamayo made him week by increasing his age.

Well, not only realization. The wound he got from red blade was not recoverable. So, after getting stabbed, there was no chance of him surviving. It's just that when dying to the red blade, he realized that he was wrong.
