ISTDP - Avoidant Attachment - My sister died but I am not sad - Session 1 | Dr Sia

My name is Dr Sia, I am a psychologist, psychotherapist, and assistant professor at Griffith University. In this video (video 1 out of 4), Chantelle (played by actor Danielle), presents to therapy reporting that she recently lost her sister but she does not feel sad. She quickly intellectualizes her problem and it becomes visible that her deeply seated grief is locked underneath multiple layers of defense preventing her from healing increasing her risk of depression, anxiety, and more.     Just stop intellectualising already! ;) Can you go straight into this kind of defence identification if the client shows resistance? Is it reasonable to expect an element of trust or therapeutic alliance to develop prior to really challenging them?

    Thank you for your great question deadlypalms! All patients are different and the approach here was suitable for this particular patient. Remember that the video is a reenactment and shows what actually happened with a real patient (my bad acting not accounted for). This then means that we must have a method of assessment that allows us to change with our patients. We can do this by assessing the patients ability to tolerate presure and their anxiety. Patients who can tolerate pressure will benefit from pressure and spend less time on therapy and be less likely to distrust an intensive treatment. I hope this answer makes sense.
