INACCESSIBLE EYE FLOATERS? USING A MIRRORED LENS

This morning someone asked about the use of a mirrored lens to get at floaters hiding out in the periphery. I referenced an old video from before 2015, but the quality was very poor. It just so happens I am treating someone this weekend whose large syneresis cloud floater like to sit inferiorly. It does move quickly up and down and a little faster than you can capture it. The mirrored lens allows you to take your time to set up the shot and on a static floater, not dynamically moving. This patient is an anesthesiologist struggling with large, intermittently obstructive floaters in both eyes, making interventional procedures, well, challenging.

    Hi doc! thanks for everything that you do😊 I’m a 21 year italian boy who is suffering from floaters in the last few months, I found out in january that I have keratoconus and it degenerated in a few months, till october/november my view was very good and i didn’t have any symptom from keratoconus, so it still shocks me how i’m reduced today, in the last 7 months everything degenerated and so floaters did, they started to appear in the last few months only in the sick eye, I had cross linking a month and a half ago and after that they are more, plus in the last 10 days started to appear even in the other eye (which sees perfectly and doesn’t have keratoconus and it hasn’t been treated in any way), I don’t know what to do cause here the doctors i went to, don’t know a lot about of how to treat them, they controlled them and they said I don’t have to worry and I have to deal with them, but i’m really concerned because they are slowly becoming more in both eyes with the days and saw that a few months ago i didn’t have any of these i’m really really worried. I know that with young patients is more complicated because they could be close to the retina but maybe there’s some exception I hope, I do everything I can, I always drink a lot of water so i’m hydrated, i eat healthy etc but they are really annoying and I would do anything to remove them.

    You are a gem, Doctor. I have a couple of dark floaters that are mildly to moderately annoying depending on the day. As can be guessed, I’ve been told they are natural, and that my eyes are healthy - in other words: stiff up the upper lip. Even though I may be able to get by with them at the moment, it greatly gives me hope and calms my mind that there may be a way to treat them if deemed necessary. You give that hope and it actually helps me put the issue out of my mind to an extent. Should I require aid, I’d feel traveling to the US and seeking your consultation is something I’d definitely do. Thank you for practicing this rare but valuable service!

    Hey doc, in my teenage i played video games with maximum brightness in a complete dark room, after the i see centre vision is slightly dull. Can my retina ever heal?

    This type of treatment is only for floaters in the center of the vitreous. If your floaters are located close to your retina, vitrectomy surgery is the only way to go.

    Hey Doc, I was wondering if Tretinoin could exacerbate eye floaters in any way? I’ve been looking to get on tretinoin for my acne but I heard it could cause problems with eye health and I already have a bunch of floaters.

    It is mis-labelled in one article as a 'nano-bubble'. It will require an injection of hyaluronin-coatet gold nano-particles. It is a very interesting prospect as ut appears to make the inefficient YAG laser more efficient, but I don't see it as allowing us to treat floaters too close to the retina which is the challenge in younger people. Not sure IF or WHEN it would come to market, and injections into the eye introduce new risks that don't exist with current treatments.

    What did the patient say about the treatment? Was it satisfactory and did it offer a big improvement in vision? Was vision clear after the treatment, or was it hazy due to large pieces broken into smaller pieces?

    It is more of a dispersion of the proteins rather than broken into smaller pieces. These cloudy/syneresis floaters will often reform into smaller, but still bothersome floaters and usually more centrally where they are more accessible to (more) treatment which is almost always expected and necessary.

    @@Thefloaterdoctor1 Thanks for the answer, I presumed as much. My floaters are fairly serious so the laser treatment may not be the best solution. Because of the risks involved, I don’t really want to go the vitrectomy route, but I may have no other choice.

    @@maartenb100 My husband had vitrectomies done after spontaneous retinal detachments in each eye. This was back in 2007, when the procedure was not as refined, yet they went well, considering the circumstances. FFW 16 years, and l developed Weiss rings in each eye, to the point that l couldn't read or drive safely, so l had vitrectomies done in each eye as well (go figure on the chances). The procedures went brilliantly, and the cataracts that developed were already starting at my age of 65, anyway, so l would have had to deal them with eventually, and again, a seamless procedure Not sure why that's such a big fear, frankly. I would choose a vitrectomy over laser treatments any time for people with dense floaters, depending on the skill of the retinologist. Actually, if my husband had had a vitrectomy for his second eye, it likely would have *prevented* the second detachment. That said, l have great respect for the Floater Doctor and *his* respect for his patients and the time he takes to answer questions here. He's also by extension a mental health practitioner.

    yes I have one 1 mm from retina, an eye inspection can reveal that.

    I used the Goldmann 3 mirror lens here in this video, but RARELY ever use it routinely. It makes it hard to judge distance confidently and I do not recommend it.

    Yes. These can be deceptive because the eye doctor may not see anything unless they specifically ask the patient to look down and then quickly to straight ahead and then see this drift across. The patient 'may' have pretty good measured visual acuity, but intermittent 'smear' or actual obstruction - hundreds of times a day.

    @@Thefloaterdoctor1 Do c!! STILL haven’t had the operation actually because the Japanese Doc is too “chicken” IMO to do what I’ve seen here YOU do almost daily in your cases!! f=_-!) Sure wish i was there because I’m SURE to be in recovery by now!! I’ve been suffering this “Weiss Ring” vision impairment for months now!! Driving me crazy,..irritable and heading for depression!! I have had sooo many photos taken in which I’m trying to obtain to no avail so far,..although many visits. From studying via yours + other sites and watching actual videos of the ops,..my case isn’t that complicated, i do get all the risks but i believe & think I DON’T necessarily need nor require the VERY intrusive + Super risky & complicating (Cataract treatment post the surgery!!🙄) I see clearly around the huge floater (Weiss ring) with good peripheral vision thus no much hemorrhage. So,if multiple lenses are used for the “vitreousys Laser Surgery,..I should be fine,..at the least be able to see a bit better!! This disorder has my Depth Perception all screwed up!! I think they 🇯🇵Docs are just trying to get more profit via a more expensive op + keep me coming back as a patient to gain more by complicating my case!!! They seen so annoyed that I’m so informed on the illness & treatment like I shouldn’t be so concerned of my own welfare and just leave my “quality of life worries to them as the natives here mostly do. I feel it’s my choice on what procedure to undertake due to its My Body & Responsibility in the outcome. In other words,..i must live with that outcome! I AM using your operation videos to prove my point!!! Laser Surgery IS possible AND Safer considering the facts in my case,…a “Vitrectomy” contradicts their argument about “hemorrhage” (some hemoglobin trapped in the floater that they’ve seen as well as i can from my view) but i see clearly around the floater and the WON’T let me see or show me my own photos because they know I KNOW what i’n looking at!! Familiar with the medical terminology of the eye,..hard to BS me. Know of just why & how the floater began & separated from the retina etc; ..and the risk of hitting that by laser as well, But I’m certain they could do it cautiously enough if pro at using it as yourself! Just may have to return to the States for this treatment to be on the safe side!! For what they wish to charge it may be more worth it considering!!…my plight!! f-_-!)

    What is the reason why the "cloudy collagen" brutally keeps building up instead of the eye working properly? There are now hundreds of thousands of people of young age who suffer from the floaters after barely learning to use their eyes. Meanwhile, there are hundreds of millions of old people who never even noticed such a problem. Somewhere, there must be a clear answer to the root of the issue, not only the "healing bandaid" options. I've been observing the floaters building up in one of my eyes since the age of 12, so the old eye explanation is not the case. Assuming that we are what we consume, I can make guesses about the nutrients in the eye. The question is: what is the eyeball lacking in the delivery of nutritional supply that is supposed to not let the collagen build-up that way while maintaining the internal balance of the vitreous fluid consistency? It's all great that every dear ophthalmologist invites patients for their treatment (that always carries risks of complication) to make money, however, I haven't heard one yet to confidently define the root of the cause. "It is normal because the eye ages" is as far as it goes.

    It might not be the 'lack' of nutrition, but possibly oxidation/inflammatory chemicals/cytokines. The typical Western diet is chock full of oxidated and pro-inflammatory materials. Seed oils like canola/corn/soy (in everything) and even worse with frying which introduces various aldehydes, etc. "Maybe" these are spilling over into the vitreous changing the otherwise generally simple and stable chemistry and interaction of collagen/hyaluronic acid/water. If you strip away the anti-stick coating of collagen, collagen wants to stick to itself. Just positing a hypothesis...

    Hi doctor. I have eye floaters in both eyes since 2020. Im only 17 but I had eye flashing?? For a half year back in that time. Also my floaters are not multiple small dots but they are big black mass. Is it good idea for me to visit you to see whether I'm a good candidate or not? Cuz I know younger patients are less likely to be a good candidate for your treatment, but I guess mines are different from other younger patients' floaters. Well but now I'm used to these floaters so that's a relief haha

    Almost every time a young person thinks they are an exception because of how their floaters appear, they ae not an exception. I'll see anyone, of course, but I still stand by the statement that the best predictor of candidacy for treatment with the laser is advanced age (>45-50 years).

    Assume Medicare will not cover treatment. I had to opt-out of Medicare because of that.

    I treat moving floaters all the time...they all move to some degree or another. The bigger question is to WHERE it is located in relation to the retina. Younger patients are often not candidates for the laser as their floaters 'tend' to be too close to the retina, unfortunately.

    @@polosochki Molecules 400 Dalton and smaller can get out of the retina blood vessels into the vitreous space. Bromelain enzyme is 20,000 Dalton. It will be too large to get into the vitreous to do anything and even if it did, it will be non-elective and break down all of collagen.

    I am not sure what you mean by mydriasis test. Does this mean you are still seeing a dense, well defined floater even while dilated? If 'yes', then it is almost certainly very close to the retina and would not be a good candidate for treatment with the laser.

    the risk is it can hurt the retina...it can cause retina tears...those with thin retinas are at risk...

    Risk is a relative thing. It is not enough to say a risk exists. The important part is what is the likelihood of that risk, and does the potential benefit outweigh the risk (especially with a very experienced laser operator). Have you looked into the risks of driving a vehicle? Do you drive? Why would you drive a vehicle? People die every day in vehicle accidents.
