Hyaluronic acid and its function in the skin.
Hyaluronic acid is a ground substance. Combined with collagen and elastin it forms the mainstay of the dermis. HA is produced by cells called fibroblast. Hyaluronic acid has come a long way over the past few decades with new formulations, both delivered as a cream topically to the epidermis as well as fillers delivered into the dermis. New methods of making HA has resulted in better, longer lasting HA molecules.
How is hyaluronic acid made?
Hyaluronic acid is manufactured using sterile techniques. In the past, hyaluronic acid was manufactured using rooster combs. Newer forms of hyaluronic acid for creams contain different-sized particles known as polymolecular HAs. These variable size molecules enable better penetration into the epidermis. As yet, there are no forms of hyaluronic acid applied topically that can reach the dermis.
What HAs are best to use?
Most manufacturers now provide a poly molecular weight hyaluronic acid creams. Companies such the Ordinary makes a cost-effective hyaluronic acid. In my opinion due to the fact that hyaluronic acid is degraded within 24 to 36 hours, spend your money on hyaluronic acid for dermal fillers rather than creams. Dermal fillers act in the deeper parts of the skin using cross-linked hyaluronic acid. Dermal fillers will last anywhere between 6 to 24 months depending on the brand and cross-linking. Hyaluronic acid applied on the skin will degrade within a day or two.
How can I incorporate hyaluronic acid into my skin care routine?
If you are living in a humid environment, hyaluronic acid can be used on skin without preparation. This is because hyaluronic acid acts as a humectant drawing moisture in to the skin. If you live in a low-humidity environment, hyaluronic acid should be applied after a bath or shower and occluded with your moisturiser of choice. This is to decrease dehydration from your skin due to large-particle HA creams which may not go into the lower aspects of your epidermis.
Hyaluronic acid in dermal fillers.
There are many types of dermal fillers consisting of hyaluronic acid, I do believe that working with many different brands and subtypes is important because it gives a good selection of material for your dermatologist or plastic surgeon. Hyaluronic acids which are cross-linked and dense can be used as a foundation; however, more fluid hyaluronic acid fillers can be used to improve areas such as under the eyes.
What Hyaluronic Acid creams do I recommend?
Every skin care company will have a variation of their formulation of HA in either a serum or cream. The Ordinary produces cheap, cost effective HAs. If you have sensitive skin, opt for a cream over a serum. If you live in a low humidity environment make sure you moisturise after applying HA creams/serums.
Can Hyaluronic Acid be mixed with other skin care?
Absolutely. HA can increase penetration of actives into the deeper epidermis. Because pure HA is part of your natural component of your dermis, this molecule is banal meaning it does not present itself as an antigen. Allergy to pure HA is almost unheard of, if you do react to HA creams, it is more than likely due to other components of the cream- for example Propylene Glycol, SLS, fragrance, preservatives etc..

    Thank you Dr. Davin. I built my skincare regimen around the basic - Vitamin A, B, & C, because I have watched your previous videos. I also watched videos from other dermatologists & drew conclusions from there using critical thinking though my understanding can be very limited being a layman. Please continue making videos explaining benefits of skincare ingredients in a layman’s term. Learning from you is a great start in selecting products and ingredients for my skin without spending a lot of money.

    I LOVE your videos! You give such complete, easy to understand, explanations about product ingredients, and cosmetic procedures. Also, giving opinions on what to spend your money on is SO helpful. Even drugstore brands offer many products, and most well over $10, and many $25+. Some say they brighten and reduce wrinkles. Others say they reduce pore size, and "rejuvenate", does that mean it reduces wrinkles or does more?

    This video is EXACTLY what I needed! I have been using The Ordinary Skin Care Line for the last 6 months (is amazing), and found that the moisturizer with HA was suddenly not working. It would literally made my face look like I was sweating profusely. It was a hot mess and I dont even use foundation. It's Summer time here in NYC and the worst day had a 97 degrees. I thought it was that the moisturizer was to heavy for Summer, but is the Hyaluronic Acid!!!! Without knowing that it was that ingredient alone, I would have likely purchased another moisturizer with HA. Understanding which ingredient doesn't wirk and why makes such a big difference. You solved my riddle Dr. Lim, Genius! I have learned so much from you I cant thank you enough. Also, I love the Ordinary Line so much I am scared I would be in trouble if they stop making it. I'm looking for other great, affordable products as back up. I also need to mention that I have oily skin. HA may act differently on dry skin in the Summer. Anyway, once again THANK YOU FROM NEW YORK CITY. :)

    Thank you for making this one in particular. There's so much confusion surrounding HA. I use the Good Molecules HA which is around 8 bucks. I apply it to moistened skin (hand held fine face mister) and then mist lightly after applying. I then use my TGF, Vitamin C and an occlusive moisturiser to seal it all in. I use the Violetta sunscreen (Aussie brand. Goes on like a serum.

    Sleeping Labradors I’ve been using it now for a couple of months and quite like it, I was using Zo Health skincare which is Very Expensive, and I Think the ASAP is very similar to that brand, it even has the same texture and smell....the only thing is I wished their Vitamin C was a Serum Not a Cream, But I use them All...Vitamin B, Vitamin C, Daily Moisturiser with SPF, Eye cream, and I’ve just started their Vitamin A at night, But too soon to give an opinion on that one, it’s now sold at a Salon near me when a long time ago I had to purchase it only online which for me is a Bonus because I don’t like buying online products.... I also have the cream cleanser and daily exfoliating cream I use at night...❤️❤️❤️🇦🇺

    As always such a great in depth video. Thank you! Question...in what order should we apply our skin care, so that the H Acid helps other actives to penetrate deeper. I use HAcid first thing in the morning...however in the evening fistly i use tretenoine 0.1% , then follow with H Acid and moisturizer lastly.

    Thank you for dispelling some myths regarding HA's and their effects on skin and the claims some companies make. I've learned to make my own serum using HA powder in multi weights using distilled water and glycerin. I don't let it dry out , rather keeping it moist with homemade rose petal hydrosol for 20 minutes. Using distilled water to rinse is key I think. I had Juvederm for volume loss in the apples of my cheeks and cheek bones and did last 2 years. However it's quite expensive though they did look great! Thanks again for another great video!

    @@jojomakes Just because it's "all-natural" doesn't mean it's better. Just because it's a "chemical" doesn't mean it's dangerous. Everything is technically a "chemical" and nothing is truly "natural". We are a chemical, water is a chemical, everything is a chemical. Cyanide is technically considered natural but it can kill you. Anything in a high enough concentration can kill you. It's more of a myth / fear-mongering / marketing tactic that "natural" is better. It's been spread by the media and now it's gone too far. In my opinion, I feel it would just be easier, safer, more efficient, and even possibly more affordable for her to buy a product that contains that ingredient then make it herself. Products are created in a lab by chemists who are experienced and are trying to formulate this for human skin. That often doesn't translate over to DIY skincare. But to each their own 🤷‍♀️

    Your face skin looks GREAT, Dr. Lim What is your AM/PM skin routine?. Question: Does applying HA after micro-needling of 0.25 or 0.5 mm help more penetration to the dermis or it still stay in the epidermis? For this, I have been using MULTI-MOLECULAR HYALURONIC COMPLEX (MMHC) from The Ordinary-NIOD. The Ordinary description on their HA says that the MMHC is a stronger combination of HA. What are your impressions on it? Also, The Ordinary has expanded a lot their line with their other name brands (NIOD, Hylamide) at a higher price.

    @@mnmira001 Yes, ideally, it should be applied to dry skin as a first layer. That's what the directions say on the label, so I'm going to assume that's the best way to use it. You can apply moisturizer next if you like. That's what I've been doing! I want to make sure I'm getting the best results, so I think it's important to follow the directions on the label.

    How often should you apply Ha serum, and is there any peptides or acids etc that can have a tightening effects for your neck , also what other serums go well with ha thanks 😊 I use the ordinary ha

    How about the effectiveness of HA when coupled with 0.25-0.5 mm microneedling? Will it still be an effective moisturizer when used in dry climates? Thanks so much for your educational videos I love them!

    Thanks for the info on how they work in different climates! I live in the NT and it’s dry season, and I couldn’t figure out why my face was as dry as a chip even after all my serums/ creams! Will need to slightly adjust what I’m using. 👍🏻

    Wash face Apply HA serum. I use la Roche Posay vitamin B5 and the vitamin C serum Apply Retin. I use Adeplene. Apply moisturizer. I use Cerave from the tub. Apply eye cream. I use Cerave HA eye cream.

    @@artiswilkins306 If you don't want to wait you can just apply HA in morning and retinol at night. 😊 Retinol is best at night anyway.

    I love your videos and always found them informative. Although, this video I felt was a little bias towards a product you are trying to sell..."injections." There is scientific proof that using HA's after microneedling allows the HA's to penetrate in a fashion similar to that you described by your injections. Would you clarify for everyone?

    I feel like HA is gonna pull moisture from deeper layers of the skin, pull it to the surface of the skin and make it easier for your moisture to evaporate into the environment.

    Twice a day. Depends on your skin sensitivity, as for repair, well atopic dermatitis has MAJOR barrier disruption. For those patients we use simple emollients. WSP. CETAHPIL, and many other simple formulations. Retinoids - late teens! DL

    One video said HA by itself wasn’t necessary as HA is in a lot of products. Made sense so I stopped and was sure to have products with HA in them like face cleaners, moisturizers and some cosmetics. Nope, for me it wasn’t enough. I’m back on The Ordinary’s HA. Yay! Feels wonderful. Thank you Dr. Lim.
