Low back pain can indeed last a long time. There are often several factors at play at the same time. For example, osteoarthritis or a herniated disc or a narrow lumbar canal can be seen on X-rays, CT scans or MRI scans. This may possibly contribute to the pain, but there are other factors at play. At the pain clinic, they checked that there was nothing serious. But for your wife and for you, this lumbar pain is serious because it prevents you from having a normal life. When the lumbar pain persists for many months, there can be disturbances in the pain circuits that will sustain the pain. This is called dysfunctional pain. You will find an explanatory video on the same channel but for the moment only in French and Dutch. The English version is in preparation. It is a domino effect with amplification of the painful information through the nerves that go to the brain. It is a very real pain. It is not imaginary as some people sometimes believe. You have certainly been prescribed physical exercises to do regularly. They can really help when you do them regularly. They allow you to have a harmonious musculature around your back. Your physiotherapist will help you with this. We have developed a mobile application that allows your physiotherapist to prescribe exercises (with video). And if you need it, he can coach you remotely with the application. The app is called MyMedicoach. and is free for you and your physiotherapist. It's the goal of MyMedicoach: Free infos and free tools to help people with back pain. Courage to your wife and courage to yourself.

It is of course difficult to give correct advice for your back pain from a distance. In general, physiotherapy is a good solution for back pain. Herniated discs are very common in the population, even in young adults. It does not always cause pain. And fortunately, the herniated disc usually disappears by itself. Unfortunately, however, it takes a long time for the herniation to disappear. For some people, the McKenzie technique can work. Talk to your physiotherapist. He will tell you what he thinks. Your doctor may also prescribe some medication to manage your pain in the meantime. But you have probably already told your doctor about your low back pain. Take heart. Your physiotherapist will be able to assess with you whether the McKenzie technique is a good solution for this back pain with these radiations or whether it is a bad solution in your specific case. A herniated disc can affect different nerve roots. Most often, it affects a root that constitutes the sciatic nerve. The symptoms are then located at the back of the thigh, at the back of the leg and goes all the way to the toes. Sometimes the nerve root affected is a nerve root that forms the crural nerve. The symptoms are then located on the front of the thigh. More rarely, the hernia affects a nerve root located a little higher up. The symptoms are then like a half belt towards the genitals. It is worth discussing this with your doctor if he or she is not aware that the pain has developed and spread beyond the path of the sciatic nerve. It is important to seek urgent medical advice if there is a loss of strength in a muscle group of the lower limbs (front or back). For this reason, your doctor will assess strength by asking you to walk on your toes or heels, for example, and also to do a thigh bend. This will show whether the motor nerve roots have been affected. Of course, you should also seek urgent advice if you notice a loss of control of your sphincters (uncontrolled loss of stool or urine).

Home-traction Unit are sometimes used by patients with herniated discs and pain in the sciatic nerve or crural nerve. Some people do indeed feel relief. Scientific studies do not recommend them. But I have not seen any studies that advise against them. You'd have to dig a little deeper into the scientific literature on low back pain and traction tables to find out more. One could imagine that they act like the McKenzie technique. But this is purely a supposition on my part. It is known that a herniated disc tends to regress naturally. The lumbar or sciatic pain linked to this herniation also regresses. Unfortunately, a lot of patience is sometimes required... There is a lot of scientific evidence about exercise as a prevention and treatment for low back pain (whether it is caused by a herniated disc or lumbar osteoarthritis or other causes). Your physiotherapist is the right person to advise you on an exercise programme. However, it is important to know that studies comparing one exercise programme to another do not usually show any difference. It is therefore considered that doing any kind of exercise and increasing physical fitness leads to an improvement and even a disappearance of low back pain.
