I started to have tailbone pain last November and it took me till February to finally see a physician about it. I took about a month or 2 seeing this guy before I diagnosed myself and pointed out to it might be a cyst. Well I was sent to a surgeon where he confirmed it was a pilonadial cyst and got it removed. After a month and a half of still trying to recover, he told me today how serious it was. I had a cyst that caused the rectal fistual which is why I was still having drainage and he mention the Seton installed. He tightened it today and restarted my pain cycle so I just hope this fixes my issue.

I feel fine, a bit of pus once in few weeks, no blood or poop or reappering abscess. Will discuss with specialist if operation is needful as the one from anoscopy did not gave me any other option. Doc google ai disagrees and I would like to add that you could develop another infection if not careful enough. Hygiene is a must if you’re gonna do this. And i recommend talking to your doctor and exploring every option available to you. They’re the doctor and you’re the lab test subject. And I’m not saying you can pick what you want, some maybe required or the surgeon doesn’t practice that. Sometimes you may need to look elsewhere like a bigger city and find out how to go there and back.

Finally somehow how doesn't make the patients nervous. I'm 31 and I have had a fistula for several years now and I'm quite used to it and it does not bother me much. I didn't even know the term Fistula until recently and and after searching on internet I became very nervous about it. 99% people are talking about surgeries or they say there will be complication in the future. I have just one question, is there any chance of developing cancer from this fistula if I keep it as it is? Note that I will try to maintain more healthy lifestyle than I used to since I came to know about many things last 2 days. Btw thanks for this video Doc, I really feel calm now after 2 days!

Who knows, I have read it that if you don’t treat it for 10-15 years it can lead to bigger complications. Complications can also include further branching. Best to get MRI to see how it’s progressing every year or so. If it is getting complex, definitely get a surgical procedure done. Diet, sleep etc. helps to manage it, not treat it fully.

Not all cases laser will work as for my experience. Im having complex fistula and my 1st doctor did the laser. It was a hell of experience for 6 months im suffering from severe pain, bleeding, pus! I never felt better after that surgery. Now i had another surgery to a different doctor. He placed a seton and this helps me a lot! Now i can leave At least normally with my life. Pls for those people who’s having this sickness. U better have 1st, 2nd even third opinion with doctors before u decide to have any procedure. Because at the end, if the doctor is not good. everything will get worst and it will be hell jn ur life.

Hi, have been enjoying your videos these last few days. I saw a colorectal surgeon a year ago for a perianal lesion . He said the only way to tell what it is was to excise it. Because this started with the skin there was constantly being covered with feces due to problems with incomplete bowel movements that needed to be dug out. I told him I was afraid of infection so I wanted to get the bowels more under control first so as to not constantly subjecting the surgical site to feces. Went to see a GI doctor to work on the bowels. She wanted to the lesion. She said she would work with me on the bowels but she wanted me to see another colorectal surgeon. I started the Miralax routine. The other day I received a call from a colorectal surgeon’s office telling me they have me scheduled to come in next month for condyloma treatment. Well as a senior who has not had any sexual contact in over 20 years I find this a questionable diagnosis especially since I never even saw this doctor before. Just thought you might enjoy this crazy trip I am going through.

They are perverts! Don't go to this appointment. When they call you and ask where you are, tell them you are not there to entertain their perverted fantasies! I was diagnosed with Crohn's disease at 15 after I went to a doctor because of a perianal fistula. I've had surgery on it 3 times total and I honestly won't have another one unless they develop a legitimate fix for it. I'm 34 now and I still get drainage as well as some pain on occasion especially during the summer when it's hot out. But constantly having doctors tell me that they can fix it only to continue having the issue just isn't worth the risks of surgery in my opinion. If I have to live the rest of my life with my fistula the way it is now I'm honestly perfectly fine with that. So I can definitely appreciate your approach of not just jumping into surgery, I wish more doctors/surgeons had that mindset.

Hi: I widh you had your office in Toronto, so I could come and see you and get a proper advice for my fistula. The only issue I have is of the various type of drainage (blood or yellowish tan stuff) and changing dressing daily and can not sit on hard surfaces due to severe itching of the skin head. Otherwise I can do my normal daily stuff. I have had this fistula for over 2 years now. Although many times it closed skin head but then opened again or i opened it myself and latley I do not get any bumps even after closure of skin head but when open again leads to alot of drainage (red). So per your advice do I need to get a surgery or not? I do not want surgery and there is laser treatment availabe here but its not covered by insurance and its expensive. Please give me advice on what you think I should do. Also the fistula skin head is about the diameter of a green pea and no drainage from anal at all. Only one sided drainage. Thanks in advance.

Thank you Doc From India ! Could you help me with a few doubts sir ? I have been getting a heat boil on my buttocks very near to Anus , initially I used to think because of to much heat this is forming and I left it to mature and drain out by itself . But It has no become a recurring boil at the same place . This boil or bump is not painful , I have no problem passing my stool , I can sit comfortably without any pain and neither there is any blood or pus oozing out . I am really confused as to what condition is this ? Some are saying fistula some are pilonidal sinus and my dermatologist thinks it’s just normal furuncle . I am really confused with the diagnosis being done with no clarity on what the condition is for proper treatment . Could you please help me sir ?

I always slip a tissue paper in between the bums, sometimes there's pus out sometimes the tissues are dry, but lately the fistula was closing very often and I always thought it was good but apparently it hurts like hell when it closed and the best way to feel better is to scratch it and let the pus out, the only problem now is I dont know why every time after I pooped the pain would stay for 6-8 hours, It's already 2 weeks now, I'm not considering to have a surgery done because from what I researched most surgeries success but still the fistulas came back. @@l.g.8028 I’ve gone to proctologists, dermatologists, and colorectal surgeons… none were able to help me. Why would you assume I haven’t done the most basic of things?

    What causes fistulas?

  • @dazeofmylife

What causes fistulas is a watery bowel. Water will always find a way to travel, so when the anal sphincter muscle holds back wet feces or diarrhea, the water will move through the skin and cause a swelling lump filled with liquid which then turns to pus. This abcess must be lanced or stuck with a needle to have the liquid drained out on a daily basis. As long as a person has a liquidy bowel, surgery will not work. If the fistula start place is above the sphincter muscle, you have a chance to heal it, but if the start of the fistula is in the sphincter muscle you are stuck with the damn thing. I came back from Vietnam in 1967 and shortly thereafter I became sick to my stomach with sharp pains and after five days could not move in bed and was taken in for emergency surgery at Presbyterian Saint Luke's Hospital in Chicago where Dr. Jehoom Yang performed emergency laparotomy removing two feet of my terminal ileum due to perforation from Crohn's disease. That was over 50 years ago. What a wonderful surgeon he was and now retired at the age of 92 I spoke with him recently thanking me for saving my life. Nowadays, approximately 25-30 percent of people who go into this surgery DIE from sepsis poisoning! So at the age of 75, I have been squeezing the pus out of my fistula daily for over 50 years after one fistula surgery failed years ago. You asked the very best question of all when you asked "What causes fistula". Dr. Chung did not address your question so I did.
