    The first 2 mls placed is for patency of the line. If the line is clogged it may need alteplase- TPA (clot buster). Most places don’t use heparin locks anymore because it’s high alert and too many errors. Also- flushing a line is considered clean technique and gloves are not needed so long as the nurse does not touch to line port (where the leur-lock is being connected) or the inner part of the plunger of the saline syringe while pulling back or administering the NS. I would just say that all PICCs or midlines NEED to have a sterile cap placed at the end of the procedure. If you aren’t giving a med prior to and are aspirating blood you really should flush with a consecutive 20 at the end. So unless the person is in CHF and on fluid restriction the order should go 10 with aspiration and initial patency check followed by a 20ml flush especially with an end cap change. To the young lady monitoring her moms line. I would ask for resources for that. Bring her into a professional. The chances of introducing microorganisms into the line is high and especially higher when you don’t have the skill or knowledge needed to maintain one- sepsis is scary! Good luck to you.

    Just wanting to give a WARNING on PICC lines, they should never be inserted into a person with total leg paralysis that only has their upper body for transfers. The transfer from wheelchair to bed is enough to severely damage the brachial plexus nerves, and cause permanent paralysis to the patients arm.

    Nice video, you have a pleasant way of explaining your methodology. At my hospital, we don’t push the plunger all the way to the very end when flushing as to not introduce air into the line.

    @@NurseMinderI had a PICC Line put in several months ago for 8 weeks, did the medication and flushing mostly myself, but I did have one drawback...because or the medication my speech was slurred as if I was drunk.

    Thank you for making this video! My doctor only sent me the saline flush and not any heparin or sodium citrate. Should I be concerned that I'm not using either one of those? Also, how often do you recommend a picc line flush when not in use?

    There are some PICCs that can be locked with saline. They are called closed end - meaning they have a valve that or events blood from coming up the cathalon (no clots). The ones that do not have this are open ended and heparin is used for those

    Picc lines are flushed once in 24 hours (usually when med is admin). If not used, then a daily check is our process

    The changes to look for our those in your facility. Sometimes they favor a circle motion for swabbing - other than that the process is probably the same - there is not often changes

    It is creating ‘negative pressure’ and prevents blood from entering the line (reduce risk of clots)

    Hi, you mentioned we put in 2 ml saline before we pull back. why don't we pull back first to check the patent first?

    My syringe as 10 cc - so there is no room to pull back to check patency. The 2 ml first also lets me know the line is flushing and working. We can have a patent line without flashback

    Hello, I'm curious. If medication needs to be administered through the PICC line, wouldn't it be necessary to withdraw the sodium citrate as is done with heparin? If so, it would be important to determine the amount of sodium citrate being injected into the line so that the next caregiver knows how much to aspirate prior to administering the medication. Please inform me if the procedure differs with sodium citrate, and whether it is acceptable to administer it without aspiration. Thank you!

    Yes. The 2 ml flush is to make sure it is not clotted, to check patency, blood flow. Then when we aspirate, we should reasonably expect blood return.

    Great question. This is one of the elements where you may see differing approaches we use a push-pause (stop-start) method to promote removal of blood/fibrin and to help prevent buildup of medication precipitate on the internal lumen of the device.

    Ideally you see the blood flow before it mixes but often a small amount of blood reaches the syringe. I have no policy to reference on this so your educator will have preferred practice guidelines

    If I use a syringe for flushing the PICC line with normal saline can I use the same syringe for flushing after the medication. That is almost 30 minutes later

    There is a lot of debate about "how much air" is too much. In a PICC line if you have air bubble then I would think you did not follow the procedure. Review technique. Even small air bubbles can lodge in a small vessel and cause harm to the patient.

    Policies about glove use changes over time. At the time of this video, gloves are used when it puts us at risk for contamination with the patient blood - always follow your facility policy The place where I use cloves is when I change the cap - removing the gap increases the risk of blood exposure

    Always follow your facility policy. They do change with time and I think post Covid- gloves are work far more often for even basic care. Clean gloves are a barrier for us and protects us from patient body fluids. If there is no risk to exposure- you don’t need them. That does not mean we relax our sterile techniques- maintain the tip and the syringe sterility and you are good

    @@deangeltv1222 the gloves can be used for the entire process but they are not necessary if you are only attaching or detaching an IV line. I put them on in the video because I was removing the cap and gloves are needed for this part of the process. If there was no cap change - you would be okay to not use gloves. Again, always check with our agency policy to see what they require. I have seen gloves used for everything and this causes panic in the patient population!

    You are doing your own care? We do teach patients and families to do that as PICC lines are ofter long term and people go home with them

    If your hands are washed then they are as good as non-sterile gloves. It is fine for a non-sterile procedures which do not involve bodily fluids.

    Yes today’s work culture wears gloves for everything - even when it is not needed. I cringe at that. Patients freak out because they think there is something wrong with them when you do a blood pressure and wear gloves - overkill

    What are you wearing gloves for? PPE is used to reduce risk of exposure to blood - there is no risk

    You have to consider the reason we use gloves - they are to protect us from exposure to body fluids. Many facilities will have you wear gloves for everything - always follow policy

    what happened to you gloves? you are talking about cross contamination but if I was a pte I would not have a clue if 5 minutes earlier you have been fishing or had a visit to the toilet and wearing a watch is a no no. Need to repeat your video sorry.
