I was one day away, yesterday from putting a down payment on laser surgery. I cancelled the procedure. Thank you so much. I felt like the surgeon was condescending and treated me trivially. I'm doing much more research before I put my future well being at risk.

Hi karen Burn, While I do always recommend a good rapport with the surgeon before committing to surgery, their experience with laser treatments are the most crucial. I still perform laser procedures every day in my practice, but it's making the treatment safer for patients that separates different doctors and practitioners. I will be publishing a video this week about making laser treatment safer for patients, which I think will help you in planning your treatment.

I feel I can't question on different treatments. We are conditioned early on that"Doctor knows best" This is my life, not his. It's not about money. I want the best result on careful planning and research and have the opportunity to question on treatments. I'm not rich and can't afford to not choose wisely.

I'm glad the video helped you, Andrea Vanda! However, keep in mind that the device does not determine results, but rather the experience and judgment of the doctor using the device. The device is just a tool, and it’s the doctor’s judgment using the tool that separates great results from ineffectiveness, or potential complications. I hope this clarifies things for you.

Thank you for speaking on this! I am 30d out from a NeoGen Plasma with 3.0j around eyes, 1.5-2.0 elsewhere. The tear troughs are now more visible than ever…. and as you mentioned in a previous video, my 2016/2017 tear trough filler migrated to about the malar area (blue, watery bags). I’ve had those injected with hyaluronidase, and while that is now gone, the hollows and crepey skin make me appear worse than ever. I’m going to see a local plastics person here, hoping to avoid the need for a bleph at this point.

Thank you so much for this informative presentation. I was planning to have some type of laser treatment in hopes of improving the loose skin on my neck that developed after weight loss. I'm often told I look much younger than my age (54), but I see the signs of facial aging that you would expect for someone in their mid fifties. I thought that a laser treatment would be a substitute for a face lift/neck lift to get rid of the "turkey neck" and the jowls that have become visible after losing weight. I thought it could also tighten my very droopy upper eyelids non surgically. I see now that I can't risk any fat loss, as my face has already lost a lot of it's round softness. Listening to your presentation has really made me stop and consider what I hope to gain from undergoing a potentially damaging cosmetic procedure. I can't imagine the heartbreak of having a procedure only to look worse than when I started.

I am 69 and am going to do the laser treatment to my lower lid. I had a lower lid surgery done 25 years ago and it still looks pretty good. So glad I did it. My surgeon is recommending the laser treatment when I go in to do my upper lids this time. He said I did not need a lower lid redone and that I had good skin. I attribute my good skin to an organic diet and adequade vit. D3 levels which I do think kept any bone facial loss to a minimum. I also stay out of the sun, but do not use make up or sunscreen. I do now use retinol and a good self tanner. I love your channel..thanks.

Thankful to come across your channel before I blindly rush in to a CO2 treatment. I had my face threaded almost a year ago and was very disappointed that it didn’t address my main concern which is the deep wrinkles around my mouth and above my lips. She injected a filler but I don’t see much improvement. She also suggested a half face lift but I read that it wouldn’t address mouth wrinkles and that a co2 laser treatment would. Came across your channel and I’m back to square one. I certainly don’t want it to look worse. Thank you for your informative video.

I am a hospital clinical pharmacist and have learned much through your videos , thank you for being educational and caring ! Btw, “ fat is nature’s filler “, I say , ha ha. So, I once heard an easily understood explanation of volume/ bone loss: it’s like having a beautiful form fitting dress and then losing 30 pounds. The dress will hang loosely and sag , but it’s the structural/ weight loss underneath that is the culprit. So....next major advance needed: prevention ! ( sigh.....) Looking forward to more videos, thanks!

Thanks for your appreciation, Angela! Volume loss due to aging is not necessarily weight loss, but rather diminishment of solid support structures, which is mostly bone, but also includes fat, soft tissue, muscle, and even the skin that thins with age. Yes, fat is a natural filler, but unlike hyaluronic acid fillers, grafted fat needs a blood supply to maintain its volume, and fat doesn't retain a predictable shape once removed from its original blood supply. Fillers do maintain a consistent shape, and don't need blood to remain viable. Not trying to argue with you, just letting you know. Thanks for watching!

This is such a great segment. I had lines around my lips and a cobblestone look on my chin due to skin laxity. I had a face lift and neck lift but they did not fully address these issues. So he did fully ablative old school laser and basically burned the skin he was looking to fix. Even tho he had applied a numbing serum beforehand, I was in excruciating pain as he seered my skin which is pale and sensitive. Guess what. I am now 3 months since the laser was done, and my skin has still not healed. So my assumption is it won't. I'm going back to see him next week. My husband is really upset with him about what he did. We were quite shocked at the results. And I am still saddened by the outcome. I wish he had your level of knowledge, even tho he is a board certified head and neck surgeon. I was fine with the face lift, and only wanted to tweak the area of my face that wasn't addressed by my surgery. Thank you so much Doctor for this informative segment. I learned a lot.

I genuinely appriciate your straightforwardness. My skin was harmed by an IPL treatment. I've been using a product called, "Psoria Gold" Which apparently haults inflammation of the skin after heat damage, lasers, etc - inhibiting phosphorylase kinase. PH-K induces scars, wrinkling of the skin, etc. Sadly, once scarring is there after these laser and IPL devices, it's tough to get your skin to where it was previously at. Some people had results after many months using this product... It's like waiting for the flames to finally die down before the healing can begin. I am definetly inerested in PRP once hopefully my inflammation goes down... Again, thank you SO much for your straightforwardness. If only I seen your video before I got the "treatment..." :(

I’ve had several skin peels and laser treatments. The results have been dramatic at times but usually just give me a fresher look. I have always used a qualified plastic surgeon or dermatologist. I’ve bought a home device but I’ll be careful to stick to the functions that stimulate rather than risk damage! I do exercises to lessen bone loss and it seems to help.

What about concerns for spreading of dermal fillers after placement? I believe there was recent evidence showing that over time as the treated areas "deflate" again, rather than being reabsorbed systemically, these hyaluronic acid fillers actually disseminate throughout the face, leading to that unnaturally plump appearance we often see.

I went to a doctor to explore some options and one of the least invasive was : co2 cool peel for my skin damage/brown spots and Virtue RF ( microneedling) a series of three treatments. Hi Doctor! Thanks for all the information and replies. I'm 23 and from Brazil. Sadly I had a very bad experience with 3 sessions of IPL and red LED therapy for rosacea and acne, and laser hair laser for beard lines. The certified dermatologist had vast experience in technologies, but I was under photosensitive medications, including low dose Accutane in the third session prescribed by her, and wasn't advised by any risks. My skin was already sensitive too. It's been two months now. First came the redness and swelling, and burning sensation. Then area around nose peeled. Later on came de fat loss and weird texture. Having a hard time trying to figure out what to do now. Derms that I went to don't really recognize the damaged caused. PRP is not a liberated practice in Brazil for now, so doctors who practice it are not under the regulation practice. Looking forward to have a consultation with you, but going to NY is now problematic due to visa authorization time delay. I'm 2 months post damage and still seeing skin worsening. How long after the laser damage PRP is recommended? I'm running against time right now? Which other techniques my be useful in my skin rehabilitation?

Hi Alexandre Duarte Iusim, Sorry to hear about your problem. Aggressive thermal treatments shouldn't be done while on isotretinoin because of the fragility of the skin while on the drug, but it may be late to correct that. PRP may help with skin rehabilitation, and it carries little risk since it's not an aggressive treatment. I would have to examine your situation for more specific recommendations, but I do suggest you seek PRP doctors in your area. Just make sure they are carrying out their their treatments under a sterilized and stringent setting because as you say, they are not fully regulated. I hope this helps you.

I had 5 microneedling sessions all done about 3-6 months apart. At first, they seemed to help my skin, but after about the 3rd one I'd say my skin got worse and worse. Now it looks like an orange peel/pink undertones. The last one I had was a year ago and I've since tried to heal my skin with various ceramide creams, but to limited success. Are the techniques used to correct laser damage the same for microneedling damage? I am considering flying out to see you to see if my skin can be helped, if it can be helped. I live in Texas. Hi Lauren B, Overly aggressive microneedling can cause scarring, disruption of oil glands, and since your skin was likely problematic prior to treatment, aggressive microneedling could have possibly made it worse. The techniques for skin rehabilitation for aggressive microneedling or laser treatment are similar, but an examination is needed before I can make specific treatment modalities for you. Hi Dr. Amiya Prasad, thank you so much for your integrity and your informational videos. I would like to ask you if you would recommend PRP/PRF for skin rejuvenation (reducing the signs of sun damage and skin laxity) over fractional CO2-laser? I realize that PRP/PRF is not going to create radical tightening of the skin but I assume it might perhaps induce a bit of elasticity/collagen for a better quality of the skin. I am somewhat afraid of fractional CO2-laser in case it might damage my skin or cause fat loss. The laxity of my skin is caused by recurrent edema of the face (over decades) so I don't think it is due to bone/volume loss and therefore not treatable with fillers. Thank you for your help.

Hi, It’s important to remember that the particular device does not determine results, but rather the experience and judgment of the doctor using the device. The device is just a tool, and it’s the doctor’s judgment using the tool that separates great results from ineffectiveness, or potential complications. If you are using them at home, not sure if they cause fat loss, and used them multiple times in just a week without recovery time, then you likely do not have sufficient experience in using thermal devices, so I advise you have thermal treatments done by an experienced doctor. I hope this clarifies things for you.

Hi Doctor. I’m reaching out to see what your thoughts are on my tricky situation. Many doctors weren’t clear in their replies and I appreciate your attention to detail. Your answer would mean a lot to me. 3 years ago I had hyaluronidase which ruined my tissues on my left cheek. Over the past three years I’ve been doing facial acupuncture and some demean filler here and there to help. These things over time helped some however there was still some skin sagging. Recently, I decided to try Forma rf laser as I read about temp control and safety. I had two sessions 5 weeks apart. The second one, she showed me the temp was 40 degrees as I was really nervous. I notice now my left anterior cheek (close to temple) is more depleted looking while everything else is looking and feeling lifted. I know- I must have the worst luck. Questions: -Can fat loss occur in just one area with laser despite being done all over the face + is what I see now what I get or can other areas still get worse? - she said she did 15 mins on both sides but I feel one side was longer than the other. More like 20 and 10. Would 40 degrees over cheek and jaw for 20 mins cause fat loss? I’m losing some sleep over this. Any insight would be very much appreciated.

  • @AmiyaPrasadMD

Hi N Ek, I don't recommend collagen supplements as they are a bit deceptive. Your body synthesizes collagen from a healthy diet of all types of food. If you eat collagen, it doesn't mean it stays as usable collagen once you digest it. Collagen supplements are digested like other food, and the body still breaks it down before synthesizing and processing the nutrients it needs. I hope this clarifies things for you.

Hi Eimear Power, Sorry to hear about your problem. I usually don't advise use of personal thermal devices because the lack of training and control for these devices can lead to volume loss and skin damage. PRP is often used for skin and essential fat layer rehabilitation, and will not make the issues worse as it's not an aggressive treatment. Filler is a possibility, but an examination is needed to determine what treatment, or combination of treatments can best help you. Hi SEMI PERFECT LIZ, I hope you understand that an examination of your skin, and your skin type are needed before treatment is recommended, especially since CO2 laser may not be the right device at all for your skin type, and for tightening pores, as well as skin toning. I hope this clarifies things for you.

Dr Prasad, you mention about your TrichoStem method and say that when skin quality improves, hair quality also improves. Can you eleborate what is meant by hair quality? Does this increase the thickening of hair strands. Or increasing more hair in terms of quantity? Or does the hair becomes shinier or stronger? Thanks.

Hi, TrichoStem Hair Regeneration is a treatment for male and female pattern hair loss, which thickens thinning hair, and stimulates growth from hair follicles that are still viable but not growing. The treatment cannot revive dead hair follicles. Hi Shay Yumi, Thank you for your kind words! I suggest you ask your prospective surgeons about transconjunctival blepharoplasty by name since it avoids the common complications associated with lower eyelid surgery, but is a technique practiced by fewer surgeons. Completely avoiding the external incision and skin removal also makes your results look more natural, and preserves your unique facial character. I hope this helps with your decision.

What about Alma fraxel laser at a medium? What is your opinion?

Hi Jenny Quezada, It's important to understand that results are determined by the doctor with their experience, judgment, and choice of tools they use. The laser itself is just a tool they use. I hope this clarifies things for you.

I have watched this video twice now looking for information on volume loss from Laser. I recently started to see a Dr using the non ablative K Laser for skin rejuvenation which uses IR I believe. He told me to do the procedure weekly. That didn't sound right to me so I questioned him about facial fat loss as I'm in my 50's with some volume loss already. He ensured me that I would not experience volume loss as the laser is non ablative and will produce more collagen. My instincts told me otherwise. As the non ablative laser goes deeper I'm thinking theres more of a chance of fat loss. Would I be correct? He started off on a low setting in my first session, then increased it a little in the 2nd session. So instead of going weekly as he suggested I had the second session 2 weeks later then the third 3 weeks later. I didn't experience any redness. I'm still worried that, that may have been too much. How many weeks apart is ideal? I would be grateful to get some advise on this.

Hi @connorapple006, Collagen induction treatments like lasers do need to be spaced 3-4 weeks apart to allow for healing, and for collagen to improve the skin. You can refer to these videos about it: kzread.info/dash/bejne/nY6p3JWJn7fPYKw.html kzread.info/dash/bejne/fIebs6ezpbGynZs.html
