    I was diagnosed with prostate cancer in 1993. My PSA was around 300 - cancer spread to every part of my body and given 6 months to live. 300 years later I’m still here❤on Lupron/elegard the entire 30 years. No sign of cancer. OSA started creeping the last few years to a recent 3.3. Began Nubeqa 2 months ago. Psa today down to 0.5. Very happy with results.

    I appreciate your message. June of 2023 I was hospitalized with extreme urine retention, acute kidney injury and lymphadenopathy. My PSA was 715. I was told it should be 4.0, not 400, 4.0. Most of what I come across refers to PSA levels around 10 or so; yours is the first reference to a high PSA number. Hearing you heard it thirty years ago and are doing fine - THANK YOU! Positive news for me to hear! Very much appreciate hearing your experience!

    @@kellyrippin7712 Thank you for the reply! I've been started on Trelstar and Casodex with so far none, or at least very limited negative side effects that I've read about as possibilities. Radiation is next which based on my appointments should start sometime next month, mid- to late in the month. Doctor said 8.5 weeks of radiation, a four week break and then another five weeks. The day I received the Trelstar injection I also had PSA tested with the result coming in at 221! Way down from the 715! At 68 years young, divorced for decades and not being Hugh Hefner... well the effects of the ADT therapy don't upset me so much I guess if it keeps me alive to appreciate another blessed day!

    I'm 69 years old and I am currently in the middle of being treated for prostate cancer. Initially, my PSA was 9.88 as measured through a routine blood test. Of course, that led to me having a prostate biopsy. 14 of the 16 biopsy cores showed various low levels of cancer. My risk factor was put at "intermediate". So, I chose NOT to have surgery. Instead, I went through 4 weeks of daily radiation and received testosterone blocking shots. My most recent PSA reading is now 0.038. In 3 more months my hormone treatments will be complete and my doctor says my PSA will probably be zero. Then, going forward, my PSA will be monitored to ensure it does not start creeping up.

    @2:00 My PSA only went to 0.3 so my radiotherapist increased the intensity of my androgen suppression and got it to 0.1. They did delay radiation until the PSA got lower. I did have Gleason Scores of 9 so that fits in with what the doctor was saying. My PSA took 3+ years to get to 0.2 ng/ml. Turns out that longer intervals to nadir are actually favorable, which was a surprise to me. I would have preferred a lower nadir, but I'm now six years out and my PSA is low and stable. This is great information. I give this compliment from the perspective of a physician who has studied this problem extensively.

    In August 2007 I got a private health check and my PSA was 5.64. Early this year 2023 I went ot the doctor for frequent visits to the toilet during the night and the doctor took a blood sample for a PSA test which was foun to be over 10 at 11.5. So my PSA went from 5.64 to 11.5 over a period of 15 to 16 years during which I have been very healthy other than the frequent visits to the toilet during the night for a wee. Anyway this was followed up from early this year ...scans, biopsy etc to find that I have Gleeson 4/3 and now I am on Hormone therapy for 6 months during which I will receive radiation therapy over 3 weeks. My testostorone level is going down and I feel aged, body sweats and fatigued. I read that my PSA should come down to near zero after Ratiation Therapy, but if it goes up slightly, I could be back to square one as it indictes that the cancer could still be present. Is there no escape for this nightmare?

    Thank you all for this channel. My husband and I have been watching your educational videos since he was diagnosed in July 2019. I have always researched everything you all have discussed. Went to his oncologist and discussed all the different treatments he could receive. Even through his case was extremely different from most other men he had a first psa of 356 at the age of 45. We were told his cancer was already an aggressive advanced stage of 4 and a Gleason score of 8 and 9. His fight against prostate cancer ended this past Saturday. He fought until the very end. Even though it’s very hard to stay positive that how he kept going. Thank you all again. 🙏🏻💙🙏🏻

    I completed MRI-linac radiation treatment (Viewray) in October. My 3 month PSA went from 7.7 to 4.3. Dr. Perloe had this treatment and now he is one of your patients. My treatment schedule was altered to one day on, 4 days off, then 3 straight days on, then one day off, then the final treatment, so 5 total treatments. This is not the usual every other day and I wonder if Dr. Scholz has any comment about this schedule as I cannot find any information on anything but the soc for this treatment but was told it is safe. My genetic profile is intermediate risk so no ADT at this time. Thanks Jim

    Very informative, as usual. I do have one suggestion/request, though. In this and many of your other videos, you omit discussion about what may happen in a salvage therapy scenario. I had a radical prostatectomy and saw first signs of biochemical recurrence about 5 years after surgery. I had salvage radiation therapy (SRT) with concurrent androgen deprivation therapy that was started two months before the radiation (ADT; single six-month dose of Eligard). My radiation oncologist told me that it would be a minimum of 12-18 months before we saw the true results of the radiation, and that aligns with what Dr. Scholz presented in this video. But where things get fuzzy for me is when you have multiple treatments that impact PSA simultaneously and can do so in opposing ways. For example, as the ADT wears off, it would make sense that the PSA would start to increase again. But the effects of the radiation will begin to kick in and slowly drive the PSA downward. I'm guessing that that may actually show a small, short-duration increase / bump in PSA again that I shouldn't overreact to. For specifics, I was given the six-month Eligard dose 10 months ago on 3 May 2022, and the SRT ran 7 July - 26 August 2022. Three months after the completion of the SRT and seven months after receiving the ADT, my PSA on 1 November 2022 was 0.05 ng/mL. Of course, we attributed that PSA to the ADT and not the SRT at that point. I have no idea where to set my expectations for my next PSA test coming up this month. As long as it's considerably lower than when I started ADT/SRT, I guess I'll be happy.

    I really appreciate these videos. You have helped me immensely on my PCA journey. It would be helpful to do a video on BPH and PCA. I have Gleason 3+4 tumor in the peripheral zone and a 80cc prostate. This limited my options for certain focal therapies. Brachy therapy has a limit of 60cc. HIFU and TULSA can depend on the location of the cancer. FLA seems to have the fewest limitations in this regard. Thank you for your information you provide.

    3 months after my Cyberknife treatment (with no other hormonal treatment) my PSA has dropped from 45 to 4.1. Hardly any trace of the cancer (was Gleason 3+4) is visible in my PSMA PET scan and MRI scan. Absolutely no side effects so far. I have become a strong votary of the Cyberknife treatment (though it is very important to identify a good doctor with an excellent track record on Cyberknife).

    I had this, though I have to say the side effects in the first two weeks, urgency and frequency, and residual pain and soreness - mainly due to the catheterisation I suspect - was awful. So people shouldn't believe all they hear about this being a painless therapy, though no doubt it's better than more radical treatments in the long run.

    Thanks for these videos. Very informative and will inform my next weeks' discussion with my hormone therapy oncology team. Diagnosed in May, Gleason 10, T3b (yes, I'm one of those unicorns - not how I imagined this to be a competitive sport). Coming to the end of six weeks radiation, I see the PSA 'pop' that happens during radiation, I am on three week PSA tests, last one was 0.66, the one before 0.48. I started at 60 in April. My insurance, with all this urgency coming from my oncology team, refused to pay for PSMA PET scan, so we did it the 'old fashioned way'. Working the plan here, will report back somewhere, when some more developments occur, if they are interesting.

    My husband has T3 PSA 19.5 two tumours one in each lobe mri gradex 5 and 4. After 4.5 month of hormone therapy prostap hus PSA was 3.2. Should it have been lower. He then had one month radiation therapy, after 6 weeks history PSA Was 1.6. We are seeing oncologist in a coupe of weeks for 12 week post radiation PSA. What questions should I ask because after seeing your video his PSA doesn't seeing low enough post treatment. Thank you. Your videos are so helpful and really clear.

    Father had 226 PSA. 1 month of firmagon and it dropped to a PSA of 3. A month after that it dropped to 0.3, and a month after that it dropped to 0.17. Does this sound favorable ? He now completed 5 weeks of radiation and he has to wait 6 months before new scans and a new PSA test

In 2021, was diagnosed with T3bN1M0 (gleason 4+5, extraprostatic malignant tumor), had 20 external repetitions of radiation and also internal bracky boosts 2 times over a period of about 2 months. Just before the radiation I was injected with Firmagon and also initiated with Bicalutamide. I had severe fatigue problems with Bicalutamide so I had to stop after 6 months. Of course, Psa showed a rapid response from less than 0.1 to 4.2 in 5-6 months. Is it only referring to a cancer recurrence or is there a small chance it could be the effect of chronic prostatitis?

    So, on Chemo one can not expect fast results, PSA is monitored, and as long as it is trending down all is good, right? I am asking because my PSA was 124. Prostatitis had me on antibiotic for a month and PSA dropped to 97. Another month waiting not doing much actively myself to make changes, my PSA came up to 120. Then I decided to see what I could do to the PSA without medication. A month later my PSA was 93. I just tested it again about 2 months later and it is 78.71. It is clearly trending down, and it seems at a rate better than often seen with chemo.

    Doctor Scholz, you need to cover some case where a person has a high PSA (in the region of 40- 50) due to localized cancer (seen on PSMA scan) + BPH + antibiotic resistant bacterial prostatitis (all concluded from positive tests and from biopsy). By just treating the cancer with radiation and/or ADT, will the PSA drop to the very low mark or will the BPH and bacterial prostatitis continue to keep it high giving a misleading conclusion that the cancer has not been eradicated. In such a complex case, how reliable is PSA to indicate the aggression of the cancer?

    I would love to see a video of you all discussing PSA being lowered via diet. I was psa 6.55 and 3+4=7 after biopsy went on a vegan diet and in 6 months dropped my psa to 5.47 I hear Dr use vegan and vegetarian interchangeably but all studies I’ve read specifically state NO animal protein or fat. For me I would like to put off radiation since my psa is decreasing but my dr seems to be pushing for radiation. It would be great to hear you all discuss a decreasing psa. Does that mean cancer is shrinking too? Will my psa continue to decrease on a vegan diet? Will my prostate shrink back? Thank you so much!

    yes I went on no animal fat or dairy based on seeing a video here and then other research and found when I had an MRI 10 months later that my cancer cells had not grown so I could still have focal therapy. That was quite a long interval so I believe, though I can't prove, it had something to do with the diet. PSA also stable at low level and went down before FT.

    What if PSA is .1 one year after SBRT radiation of 3 Mets to nearby lymph nodes? Orgovyx and Zytiga Included . (Original PSMA scan showed 3 Mets with clean prostate bed).original Gleason 4+3 treated with Lupron and SBRT. Thanks….

    It would be interesting to know about potential effects on PSA from chronic prostatitis particularly after FT where a larger part of the prostate remains. This is also a problem on active surveillance.

    My father was diagnosed Stage IV last June with a PSA of 800. After 3 Lupron shots, ongoing Nubeqa (Darolutamide) and 6 Rounds of Docetaxel (Chemo) which he completed in December, he just had a PSA test done last week and it has settled at 0.5 ng/ml. We are hoping it might drop a bit lower still since he is only about 2 months post-chemo; however, I am curious to know if anyone thinks settling at 0.5 ng/ml is worrisome or if it will likely drop some more? We are waiting with baited breath for the next PSA test 3 months from now...

    i finished radiotherapy a year ago having prostap3 injections every 3 months my psa before radiotherapy was 83 at the moment my psa is 0.5 i was diagnosed with gleason 7 non mestatic nothing in lymph nodes nothing anywhere else the cancer was at the tip of the seminal vestules but i have heard nothing apart from the doctors are happy with the results are my results good what are the chances of recurrence

    My dad has a psa of 202 he has been put on zoladex injection, casodex, zytiga, neutas and zolendronic acid Gleason score 8 (4+4) it has metastised to the lymphnodes and pelvis girdle, do think the treatment is enough? Should he be started on first generation drugs?

    I'm curious about the seeming to me growing number of prostate problems. Not just prostate cancer numbers. I'm wondering if the numbers of prostate problems are growing or if we are just becoming more aware they exist. And then if we are getting more prostate problems why? Diet, modern chemicals, loving longer,. .....Or have men always had this rate of prostrate problems?

    6 months ago I had a psa of 56 and diagnosed with stage 4. Put on Luperon and Apalutamide and it is dropping at a rate of 50% per month. Last test was 6 . I have a blood test today and expect it to be 4.5 . Any comments on whether this is “normal “ or slow. I am also having radiation treatment ( going well-one visit a week for 6 weeks)- but as the Dr helpfully pointed out this will not have any impact on psa

    I have been diagnosed several months ago with Gleason score 8 and no clear path to treatment yet. My PSA went from 3-4 then after 3 yrs. to 9 to 16.6. Now the very latest is at 11.0. All I've done is that I eat a lot more Broccoli every day. What is the significance of the lower PSA? Should I ask for another PSA test? Please advise. And thanks a bunch.

    Hi, i was only recommended radiation. After 6 months my PSA has dropped from 9.6 to 2.4. Should i have also asked for hormone therapy.

    In 2004 I had a PSA of 95 Gleason grand 4+ 5 = 9, had 7 weeks of radiation, a lot of hormone therapy which I still have to day .PSA under 1, I am 81 and still in reasonable good health.

    How about avoiding the biopsy by having a yearly psma pet scan and if it appears that there is no cancer then proceed with watchful waiting.

    won't work ..i kinda tried that..but psma showed no mets but cancer in right lobe...finally gave into biopsy and oops..psma was inaccurrate cancer in left lobe as well..need biopsy -psma makes mistakes..

    They told me my PSA was 5000. Also said there was little they could do. Hormon therapy makes living terrible with hours of sweats and clothes soaked.

    know if someone would see this comment but,My dad (has high risk cancer) had hormonotherapy(that decreased PSA from 70ng/ml to 0.3ng/ml right after he had his radiation treatmant then mesured PSA after 3 months and surprise PSA is 239ng/ml?!!!(Like a month before radiation it was 0.3 and boom it turned 200 after) So his doctor is now shocked and recommended to do different test and change the type of hormonotherapy from triptolin to Zoralex .Tmrw he'll re-do the test hopefully those results were false

    I'm in this scenario, just had prostate removed & 7 out of 14 limp nodes. The urologist is very unconcerned, My 1st major surgery & she has no helpful information. Sad we pay all this cash for health services that we barely get. Like how is it supposed to feel to me inside. I have a spot near one of the incisions that hurts when I move. I'm so disappointed with the specialist that don't give a rats ass about anything but getting paid...my Dr.

    I have been on Eligard, and abiraterone 250 mg tablet 4 X a day for a year PSA down to undetectable (0.018) do you have an opinion on lowering the abiraterone 250 mg tablets to 2 X a day as the side effects is too much? thank you

    My prostrate cancer was caught late. PSA was over 200, Gleason score was 12. Metastases had occurred. Initial oral drug was a abiraterone , along with lupron injection. Along with meds, I adopted a macrobiotic eating plan. PSA dropped to 1 within 1 1/2 months. I firmly believe that the nutrition aspect helped significantly but I am disappointed that doctors don’t seem to want to give nutrition much credit. Your thoughts?

    Dr. Schulz, first thanks for clarifying how radiation works, damaging cancer cells so they die over what may be an extended period of time. I wondered if you are familiar with amphicrine prostate cancer, an aggressive sub-variant form of PC? There doesn’t seem to be a well defined recommended treatment for this cancer that has spread to my bones for which I’ve started receiving Oxfigo and Zometa treatments. Thanks for any insight you can give me on this.

    W O W..... Totally Confused??? I kinda get what is being said, Kinda. But my Last PSA was 1.95, and I got Peeing Issues, so my Urologist say's to just do the Green Lite Laser to Treat that Issue, and that he has "NEVER SEEN" a man at my AGE 69, that had a PSA "THAT LOW" that had Cancer???? I'am Skeptical...??? Kinda Makes me WONDER if he Really Gives a RIP???

    Why isn't a "chemo wash/rinse" of the prostate bed area done immediately after prostate removal but before the abdominal area is closed by the surgeon? I would think this would help kill possible remaining cancer cells left behind via minor bleeding, etc.

    I had also relied on a liquid biopsy (blood test) to confirm the presence of tumor cells in the circulating blood which was offered by Trublood@ Datar Genetics before going for a TRUS biopsy. It is claimed to be a highly reliable Yes/No identifier of prostate cancer.

    So let me get this straight. Every oncologist seems to agree hormone therapy (removal of testosterone) can be effective at treating prostate cancer. But these same oncologists have no problem (after having an undetectable PSA for ~18 months) turning around and prescribing the same patients synthetic testosterone. Does that make sense to you? The only people who agree prostate cancer feeds on testosterones are the ones who stand to gain financially (big pharma) and the doctors who are brainwashed by these same people. Talk to any cancer biologist and you'll find out there's no valid peer reviewed data showing prostate cancer feeds on testosterone.

    Tom, The reasoning is that if the cancer is successfully treated, then there is no reason for a patient to remain on testosterone depriving medications. After treatment, if a man achieves remission, then his testosterone is expected to return to normal and there will be no further need to lower his testosterone unless there is a potentially dangerous relapse. Testosterone does not cause cancer; it is just a component that the most common forms of prostate cancer need to survive and spread. However, if the cancer is treated, there is no longer a risk of the testosterone feeding the cancer since the cancer is gone or dormant. If a man's testosterone does not recover normally, then, Dr. Scholz is saying here that he does not see any reason why other sources of testosterone could not be used to achieve the same eugonadal levels achieved by patients who recover normal function of their testicles once hormone therapy is stopped. It has been extensively documented and it is universally accepted in every relevant scientific and medical field that "Most prostate cancer cells rely on testosterone to help them grow.     @@Monk-Amani. I’m not March 11,2022 is going to be a year since my radical prostatectomy and I don’t regret it at all. I didn’t want to go to all the radiation or hormones therapies and sides effects. And so far I’m cancer free with last PSA 0.01 undetectable.

    In this age of precision radiation why would anyone primarily opt for a prostatectomy where the possibility of incontinence / urine leakage is a major side effect along with blood loss and hospital stay?

    @@kamalsh6123 You may be right about the incontinence and one day stay in the hospital, but this is a choice that your dr gave you and I had a whole year to decide during my active surveillance time. Radiation put the cancer asleep and it can spread without knowing. Also you if your cancer spread you can't get surgery. Either both ways are hell and luckily I'm cancer free and back with my continence but sometimes with radiations you get long term side effects. Nothing is going to be peaches and cream. What is important is that we can be cancer free. Good luck with your journey.

    I thought the conversation regarding PSA following treatments missed an opportunity to discuss PSA bounce following brachytherapy. Also…time to nadir following radiation is longer than 2 years, I think.
