HIV Tests Explained for Clinicians

An infectious diseases specialist explains HIV testing. What kind of HIV tests are available, how should we use them, and when? How do laboratory-based tests compare to point-of-care HIV tests? What is the sensitivity and specificity of different HIV tests? What is the window period for each test, and how does that influence HIV testing algorithms? What if we want to exclude HIV infection? When should we repeat the test or use another one? How do we approach possible acute HIV infection versus chronic HIV infection or AIDS? As always, this video is primarily intended for clinicians, medical professionals, and students. I highly recommend you follow the links to the research articles I used as resources for this video. You will learn a lot. Good luck!

Hi @clinicaltips8926, do you know what the sensitivity of the RNA PCR test is at 17 days, or 21 days? The only study I see states at 12 days 50% of HIV positives are detected. So that would be a sensitivity of 50%, like you suggest in video (median time of 12 day). I'm wondering if it's worth waiting another week to test. It's confusing because online there are some confusing statistics (people quoting average sensitivity across all time periods and making it out like that would be the sensitivity at 10 days).

Thanks for your awesome video doctor, it’s super clear.I have one question though. In a case of delayed seroconversion (even though extremely rare), let’s say the patient did not manage to produce detectable antibodies at 3 months, would a 4th generation test still be able to pick up the p24 though? What I’m actually wondering is, is the vanishing of p24 only a result of antibody formation? Would there be a period where p24 already dropped to undetectable while antibodies are still not formed in sufficient level, hence maybe causing a second diagnostic window period?

You are not exposed to HIV, in simple words you don’t have HIV. Stop worrying. The window period closes at 90 days after the exposure for most of the tests. The rapid test should be followed after 6 weeks of the exposure or 4th gen test after 14 days to rule out the infection. The last confirmatory test should be followed after 90 days for 3rd gen and 28 days for 4th gen test, to be sure that the previous test is negative (chances for test to turn positive are negligible 0.001% ; exceptions if you have other exposures).

if you took this many tests after 135 days then you should be clear of HIV infection, 100%. There is 0% chance you are positive. The likelihood of all 5 tests being false negatives is so incredibly small, that you are more likely to get struck by lightning instead. Many people feel paranoid about this stuff, and it is normal to a degree, but if you are still unconvinced despite taking 5 different types of tests then I advise you to seek mental health resources. Hypochondria, which is the fear and paranoia of diseases, is a real mental illness that affects many.

I took a rapid test at home, and it came out positive and I’m so confused what to do next . There’s no definite info. There’s so many different test options , and when I look up hiv clinics around me it’s so far away and I don’t drive so it’s impossible for me to get there . Trying to go through my health insurance app, it gives me 2 separate options for hiv 1 or hiv2 test which makes things so much more confusing . There’s antigen/antibody tests, some other test that starts with an ‘N’. I just want whichever test is going to confirm it 100% , I don’t even know how to ask for which test I need . Like I have no clue how to make an appointment or where to make an appointment because I just barely got insurance from my employer so I have no healthcare provider , so im so confused on where I can go. Can I go anywhere where my insurance is accepted and ask for an HIV test , it’s so confusing

If I’m worried I have progressed to a chronic infection , because I’ve been infected for years from sexual abuse as a child , and my health has been on the decline in a lot of ways and now that I got a positive rapid test I’m almost certain I got it before 2010, or in 2015 from a sexual assault I experienced as an adult because my health started declining after that and doctors were never able to figure out what it was and just treated me for other things. So I’m almost certain I’ve been infected for quite some years now, and it’s only getting worse and worse and my health and body is getting worse and worse , so which test should I be seeking ??? I’m so confusedddd . When my results come back from the doctor will I be told “you have HIV and this is what stage of it you’re in” or will I have to go in for another test to get checked for more detailed information ??? Will the initial test just confirm that my rapid test was right??? When making an appointment should I tell them I already have a positive rapid saliva test , so they don’t waste my time repeating the same test over again ?? So confusing .

Sorry to hear that. According to your description, your saliva self-test is positive, then you should be infected with HIV, because saliva test is one of the least sensitive methods. If saliva can be tested positive, then your situation is not optimistic. Basically, it can be considered that you are infected with HIV. I Don't know which country you are from but I think if your area has the conditions then you should go to the hospital immediately for re-testing and take antiviral drugs under the guidance of your doctor after confirming the infection If you start treatment now use your current first line drugs If you are lucky, your lifespan may only be 20-30 years shorter than normal people or even have the same lifespan as normal people

It also needs to be emphasized that you'd better go to a large medical center, hospital, etc. for testing to confirm that you are really infected with HIV, and immediately use anti-AIDS drugs under the advice of your doctor because you may have been infected for 7-10 years according to your description. ? If this is the case, your situation is more urgent, you may be on the verge of AIDS or you are already in mild immunodeficiency. In short, go to the hospital quickly and don't give up treatment. Modern drugs are very advanced. Most H HIV-infected people can have no less than 30-50 years lifespan and the future treatment drugs will be more and more advanced I think your experience is tragic The infection is not your fault Don't feel burdened and try to live

Hi, Conclusively you don’t have contracted any HIV infection from the exposure you have mentioned. Stop worrying and you don’t need any additional tests. As mentioned your test at 95 days is conclusive to rule out “you don’t have HIV infection” - Relax and enjoy your life.
