15 years mining and I have seen most of these situations. Been a operator in most. Production supervisor in others. Although never had/let anyone dump without good windrows. But had someone tip over a tip head a month or two before I started on site. Never just pushed on with a articulated truck in mud that deep. Even being logging and worked soft/wet ground as a forwarder op.

People sometimes simply misjudge the situation, which is why accidents happen.
Just completed an annual mine safety course at work over the weekend and saw firsthand videos of what not to do. The blind spots on those haul trucks are everywhere and can cover hundreds of feet.

Many drivers of smaller vehicles are not actually aware that they are in the blind spot zone.

I work at a small stone quarry and operate a Volvo 110 loader and try to be safe but the kids ( only way to describe them because I'm twice their age) think it's funny to walk in front of me as I am loading trucks or just walk behind me knowing that I have blind spots and our fearless leader set in the office playing with his phone not working so I have told the owner if I hit one I will feel bad but won't lose any sleep over it. M.S.H.A. has told me that it would not be my fault and if they are on drugs ( witch four of us six are ,not.me never have never will ) they will back me on any problem.

@@jeffharper7579 The same thing is in my company where I work, when we work night shifts, kids drive a 500-ton truck and their faces are constantly lit up by their phones, they literally don't take their eyes off their mobile phones while driving.

Honestly, this video had some of the best voice-over commentary I've seen. It was well thought out and intelligent.

Really? That's freaking crazy. AI did a better job than most of the older voiceovers. They always made stupid comments. I guess kudos to technology

At fault should be the guys in the pick-up, I've been to couple those mines out west If the pickup is beyond safety zone should have been displaying vertical markers

The off road truck has right of way. I drive off road for many years in many mines and the biggest Iron has the right of way... little shit just get out of the way...
@4.15 re the dump truck running into the suzuki. 1) the dump truck driver failed to honk the horn to warn people that there was a truck moving off. 2) DT turned left INTO his BLIND SPOT, ther is a reason that parking areas are set up to drive straight ahead then turn. 3) The idiots in the suzuki PARKED in the BLIND SPOT!!! One off the first things you are taught(at induction) is ALWAYS park so you can see the dump truck/loader driver. If you can't see them, they can't see you. There is a CAT safty video that shows the blind spots around a dump truck, and they are HUGE.

Of course, the Suzuki guy was stopped on the right side of the truck so that the driver of the truck could not see him, rookie mistake.
