Dr. Pelto, in your experience, which surgery has the better outcome/prognosis for hallux limitus/rigidus: cheilectomy, fusion/arthrodesis or implants? FYI-I am undergoing a cheilectomy on 8/18/2023.

I had Hip Surgery (replacement of the right hip] on January 29 and for four weeks between the end of March and the third week of April, I did rehab in a swimming pool. As part of this, I decided to do running in the water and probably overdid the springing from the bottom of the pool. A few days later, I experienced bad pain in the big toe joint of my right foot and just above it and assumed it was due to the overuse in the pool, but also worried that it might be gout as I do have many of the risk factors for it. A week after symptom onset, I went to a clinic where they took x-rays and opined that it was not gout (even though they didn’t take a blood test] but rather an irritation of the big toe joint due to the activities I mentioned. The doctor pointed out that I had incipient osteoarthritis in that joint with a small cyst and that the pain was an understandable result of the overuse, he told me to wear a rigid soles shoe, and to reduce walking, for 3 weeks. After I started wearing the shoe, the symptoms got a lot worse. I have no way of knowing whether the shoe caused this worsening, but the worsening was significant pain increased everywhere in the foot, including across the top of the foot, and in the other metatarsal joints, but also underneath and some spots in addition the swelling increased significantly , everywhere in the foot and the big toe became painful and harder to move at times. I’m wondering if this progression was normal somehow after a week of pretty stablesymptoms,if if this worsening was somehow due to the shoe, or perhaps a stress fracture or some other insult. Do you have any thoughts? (This is particularly frustrating because after two months of pain,I’m nobility and rehab, I was finally emerging from the period of recovery from my Hip surgery and working on regaining strength and mobility. This process is nowhere near complete but now I’m back to being immobile due to the foot, of the foot which is unpleasant and also harmful to my continuing hip rehab. )

I was told that I have hallux rigidus. I don’t have pain in the joints. My main pain area is in the second metatarsal. It starts on the top of my foot on the metatarsal and I have pain on the plantar side of the foot

I was recently diagnosed with this and have been getting cortisone shots. It does improve the pain but weirdly the pain I am okay with, I've been an athlete my whole life - pain is just part of my life. The thing that is driving me insane is I want to stay active but sometimes the toe just fully locks and cannot be moved. Its infuriating for someone as active as I am. Is the locking something that could be resolved by contiued shots and PT (you expresed skepticism over PT) or am I pretty much only going to be able to resolve that with surgery?

I have hallex limitus (joint is swollen, minor pain when running) and have a football season approaching. Should I just use an insert and play through it? I only have 3 months so I'm not sure what other treatments I can do in this short frame.

Is it possible to have Hallux limitus/rigidus and valgus/bunion? I am fairly young. My left toe has a bunion forming in the "usual" valgus place which is pushing my toe towards the other toes but i dont usually get pain at the bunion. But the toe is very stiff, doesnt have much extension movement very painful and swells and pain increases when i walk and bend it which has got worse over time

I have had X-rays and have arthritis and a spur on top of my big toe. But no pain in that toe. The pain is in my second toe, at the back where it goes into my foot. It’s on the bottom. Like the ligaments/tendons. The pain is more like a burning sensation and soreness. I would rate it as a 6 in pain when it bothers me the worst. It comes and goes from day to day. Have ever heard of this? Thank you.
