As we get older, it's common to experience receding gums and it may be referred to as getting long in the tooth. Gum recession has many different causes, and when the gum tissue recedes, it is advantageous to get it treated.

How to repair gum recession?
Many periodontists still do gum grafts, where they take gum tissue from the palate (or the roof of the mouth) and use that tissue to repair the area of gum recession.
This often times gum grafting can be very painful, disfiguring, and can take months to heal completely. Also the number of teeth that can be treated is limited to the amount of gum tissue that an be removed from the palate.
As a matter of fact, I had a patient who had 2 unsuccessful gum grafts underneath his tongue. I was able to use the modified pinhole technique to restore the area even though there was excessive scar tissue.
With the Pinhole Surgical Technique, we can treat as many teeth as we want in the mouth. We can treat one tooth or 30 teeth, there is no limit.

    Im glad I found you. I will be setting an appointment soon. I have gum recession on the right side of my mouth really bad and it is indeed a combined result of genes and well as orthodontics when I was a teen. I believe educating patients, in any field of medicine & dental health helps patients understand 'why' we should be doing, pertaining to what is recommended by our health care professionals.     I had pinhole surgery a year ago, and so happy with it. The best part was how painless and quickly it was. I can't imagine doing gum graft surgery. Some people mentioned the price; however, remember these are your teeth. An implant will be as costly as the pinhole surgery.

    I paid 6500€ for pinhole surgery in Austria and lost all my money and have a worse situation than before. I didn't find many comments on KZread maybe they get deleted. But i searched the Internet and found a thread where many people write they lost thousands of dollars or euros and for nothing. They all regret it. I only found two comments from ppl who said it has slightly improved but hundrets who regret it. I shouldn't have done it, it's one of the biggest regrets of my life and i cry everyday because it's so much worse than before. The dentist said it's a bad result but didn't really care. The surgery itself was very brutal and painful torture. I also found comments from professionals on the Internet who warn people, one doctor had it done on himself and ist warning people not to do it. My Front lower teeth got lose from the surgery and i found a comment from another person who wrote the same. Other ppl write they lost teeth or had nerve damage and the gum looks like before. The pinhole dentist said my teeth might have been lose before but my main dentist doesn't believe it. They were strong. No matter how much you wish to find Help: Stay away from this surgery!

    I live in Ann Arbor and I’m terrified of dentists because of the level of pain my gum recession causes during cleanings and how many hygienists dont take it seriously. One even laughed while I was yelping every time she touched it. Praying that you’re taking new patients on now. Four of my bottom front teeth hurt so bad and the top left side of my mouth seems to have a never ending bad smell from a crown that other dentists have said nothing is wrong with. I need the help desperately but I’m so scared 😞

    So glad he said orthodontics. I wore braces from age 12-15 and I had a feeling that that played a major role in my gum recession.

    Yes Alli, it is extremely common. I would say about 70% of my recession patients have had orthodontic treatment in the past. Most people don't know the risks until it's too late!

    Well. I just discovered that I have 3 of the risk conditions. I had orthodontics 10 years ago. My mother and her siblings all had receding gums. And I clinch my teeth while sleeping. At least I discovered it early with only one tooth.

    Hi Dr. Nemeth, thank you for your informative videos. What I wanted to know was, does this technique work for those who also have significant bone loss where the interdental papilla is blunted? I have seen your video on the LANAP laser and its ability to generate new bone tissue and I was wondering if this technique can be used together with the LANAP to restore the dentition. Also, I live in Southern California and I am having a very hard time finding dentists who practice these two procedures. Is there a resource I can go to to find out if there are dentists close to me who provide these services? Thanks so much in advance for your help. I truly enjoy your videos.

    I am glad I came upon this video for about 5 years now I had one tooth that I felt had a lot of gum recession which seemed to have started post pregnancy but my dentist never mentioned anything to help then recently I found I had 2 cavities so far only had one filled but it was a horrible experience and not it seems like I’m getting more recession along with more cavities popping up or wear on my teeth. I’m thinking it’s time to switch my dentist

    Ive never had gum recession, but in the last weeks at the cleaning it was discovered at 4 back teeth and since the cleaning I am in so much pain. I havent been able to chew for a week now. My dentist said I caused it by using a soft brush, the dental plaque didnt get cleaned properly. Now Im trying to get pinhole treatment here in Germany, if you have any recommendations :)

    The cause of gum loss is bone loss. Whether orthodontic settling or trauma from occlusion, the bone loss around teeth lead to gum loss and recession. Gum tissue needs to be supported by bone. You can increase the thickness and stablize some gum loss with graft or pinhole technique but if the bone is not rebuilt or tooth angulation corrected with orthodontics, the gum graft will turn into pockets that will later cause problems and periodontal disiease issues. you never have gum recession without bone loss.

    Doctor, I may be considering a trip to your state. I had a gum recession and an exposed root on an upper molar after previous poor brushing technique. I went to a periodontist who told me that even if he used pinhole, that because of some enamel loss, he wouldn't be able to bring my gum down enough to attach to my tooth. So he recommend that my dentist fill that at the gum line, which I had done. While my root is now covered, my sensitivity is better but not completely gone. And Im worried that because i never pulled the gum back down that the gum could possibly get worse as I get older (my brush technique is good now but I still worry) Do you ever recommend filling on the front gum line to patients? Am I right to want a second opinion?

    Hi Doctor, how long do the results of the pinhole technique last, assuming that the patient then begins taking good care of the renewed gums? I had a consultation with a periodontist about this technique a few years ago, but she couldn't give me great answer because there hadn't been many cases that were more than a few years post operation (due to this being a fairly new procedure). Are the effects of the pinhole technique projected to last permanently?

    My swollen gums brought me here. Also, i noticed some parts of my gums is receding...huhuhu. I dont know if it is due to my brushing or due to my braces. I just had brace 2 months ago and my orthodontist did not explain to me the consequence or complication of having braces. Im seeing a periodontist in a week so I hope everything will be ok.     Dear Dr. Nemeth, I have gum recessions at upper 5+6 molars after orthodontic treatment. I was told, that it is difficult to make a gum lifting in molars region. Actually I found only very few videos about gum and bone reconstruction in molar area after orthodontics or as result of parodontitis. I asked german periodontists (over KZread) if a pinhole technique with bone filling exists. Normally pinhole technique uses collagen stripes to fill the gum. But a essential problem of gum recession is the behind lying bone recession. And simple gum lifting with soft tissue filling seems like a pure cosmetic operation. And without new bone the gum may recess after some time ? I could imagine to yield some bone mush by drilling into the massive upper hip bone and filling this living hip bone mush into the pinhole. But it is unclear if hip bone cells can transform to sense full alveolar bone to reconstruct a healthy parodont. My question to You is: Are there any sense full methods of doing gum lifting with alveolar bone reconstructions ? Regards: Wolfgang Renner

    Hello Wolfgang, great question. If someone has minor bone loss and recession, simply covering the recession and reinforcing the soft tissue with collagen is enough to reverse it. This will stop the sensitivity and make the gums thicker and healthier. However, if someone has severe gum recession and severe bone loss, we may recommend bone grafting along with the Pinhole Technique. It is impossible to tell without seeing your case, but yes in some cases it may be beneficial to correct the gum recession and reconstruct the bone. It may need to be done in two separate procedures, or it may be done all at the same time. It really depends on the individual case, what is causing the bone loss, how much bone is left, etc. Thanks for commenting, I hope this helps.

There is a long video about an architectural idea for an educational and cultural building (SchlossFuenfeck2019Neu). There are a couple of pictures scattered in, which shows my orthodontic problem. 1975 I came as 12 years old boy to an orthodontist. A 4*4 extraction should be done, but I didn't understand the sense of the treatment. Resulting in extracting upper jaw 2*4 and in lower jaw a middle incisor. This was bullshit and only done because insufficient pedagogic. The result was bad, and I made a second orthodontic treatment 1994 as 32 years adult. Therewith the overbite was partly lifted and the lower three incisors could be moved to straight line. The bite lifting was done with the upper braces anchored at the 6-Molars. This Molars where not moved, but using them as anchor resulted in (severe) bone lost, showing immediately after orthodontic treatment as long teeth. The orthodontist said it came from teeth brushing. But this was only to cover the true reason. Better would be to use some headgear (J-Hooks) to gain anchor forces for lifting the front. But there was no talking about it. It didn't make any further treatments on my teeth up to now. Currently the recessions becomes little larger and also "age" parotontitis starts in light amount. Because looking the KZread videos I understand now much more about this theme. The second treatment 1994 betters the front teeth position but the back molars position became worse. Before the treatment the upper 8-molar bite on the lower 7-8 molar area. But after the treatment the 8-molars standing too far outside, not really reaching the lower molars. All 7 & 8 molars got no brackets and where not included in the treatment. It was again a bad quality treatment. Currently I am 57 years old and inquire the question if a third orthodontic treatment could rise the overbite further and adjust the back molars to better position. But because I am now an old man with beginning parodontal age decay, a third treatment must be accompanied with anti-parodontitis treatment and with reconstractive recession lifting. But for my age it looks still quite nice. Seeing KZread videos teaches me, that periodontal problems can be much more severe then my case. But if I would start any third treatment I want to plan best cooperation of orthodontical fine adjustment with periodontal treatments. In my video it is easy to scroll to the drawn models showing my case. There are also some discussions about extraction therapy in general. The 4*4 extraction result in smallest teeth movements. But most patients feel it as lazyness strategy for orthodontists for getting higher profits in saving working time. I have a problem with hampered nose breathing. And in the case of mouth breathing, it is said to avoid extractions to motivate better skull growing. But expansion therapies (palate expander etc.) are regarded to traumatize the paradont resulting in later recessions. This are all things which I am investigating. More general and academical and not only to cure any invisible molar problem. But the question remains: How to treat a 6-molars recession with bone reconstrucion ? Sometimes the mucoderm tissue is filled in tunnelings. Could it be possible to fill such an tissue with bone minerals, which are often used as bone growing support ? This means my question tries also to discuss future developments and to find a best solution for cases like I have. Regards: Wolfgang Renner

    Hello, my gums are generally OK except for a part above one tooth. This part heavily bleeds even when I lightly brush it. It hurts so much when I brush it as well. Afterwards, the bleeding stops and it becomes white. Any ideas on what it is and what I should do?

    I have noticed rapid gum recession over the past 3 months that even my dentist was surprised about, he checked all my teeth and said there were no problems whatsoever my teeth and gums were very clean and in excellent condition, however I was diagnosed with Ankolosing Spondylitis a few years ago and started a immune suppressant injection last year, could this be the cause.

    I am so gl ad l am educating myself on utube about taking better care of my teeth & gum , my gum is receeding now so am trying to revive it with oil pulling , sesamee oil , coconut oil, clove oil & using a few drops of tea tree oil with my toothpaste

    If you are experiencing gum disease please consult with a periodontist. I waited too long and ended up getting a bone graft and sinus lift in order to get implants which I had to take out a medical loan. Please do not try to repair gum disease on your own 😟

    Teeth needs to have a little mobility, recent studies show that holding bars on the back of the lower cause gum recession...plus that people usually dont keep them very clean once they put them on.

    Doc can i do oil pulling if i have uncovered healing implant. I dont think they put a healing cap on it. Is it safe or can the oil loosen the inplant because its metal? Can the oil leak down into the bone and implant screw?

    First I'd like to say that I wish you'd open shop in DC so I could have my teeth be done by you! I found in a periodontist that has the laser for lanap, however, I dont think he's familiar/knows how to pinhole. He said we'd graft wherever gums were needed, but I think he didn't expect me to know what pin-holing is and said "results would be difficult for perio patient to maintain" :-/ But, I guess he didn't realize that I learned the technique from you, so after he does the cleaning I will be going to a periodontist who does do pin-holing to see if it can be done, and will be seeing my dentist who is more of an emergency cosmetic dentist to do 4-6 porcelain crowns on the bottom. The one I am seeing for the lanap tomorrow made me feel that I shouldn't expect the aesthetic part for a few months because the healing process will take a month, then there will be grafting, and then he wants to do the bite..... What do you think? Can I start the process for porcelain crowns in a week post-lanap? Thank you for making these videos! :-)

    I my office, the main reason for gum recession was always, overbrushing, with a hard brush. I note you did not show pin hole examples. DMD

    Then you have nothing to chew on, and then you have to spend money for an implant, or for a partial, and then you spend the rest of your life fixing the partial and whatever damage it causes to the teeth around it. Although that does not always cause damage, I believe.

    I’m only a teenager and I have braces but I noticed a few days ago that there are like a line of gum missing on my front bottom teeth. I only brush my teeth twice a day and I’ve never had cavities and the dentist always compliments my teeth but I do brush pretty hard.

    I have gum recession and bone loss due not only to genetics but also all the harsh chemos I've done over a period of time (years). I had aspen dental tell me they want to pull all my teeth... Nope

    Hi there, this is a very interesting procedure. Can I know if patients’ gums and teeth are properly cleaned prior to pulling their gums down in this pinhole surgical technique? Wouldn’t that make sense or the gums will be pulled down to cover all the culprits that cause periodontal disease?
