It's suspicious that it always seems to be right when time is running out, even though there's been several times he could have gotten it much sooner.

    Non-doctor here, but I was surprised he didn't ask for a CT before the upper endoscopy. You have to ask what's more emergent and how long you've gotta wait before the next test can be done. Time is wasted while that clot is doing damage so he can check for acid reflux.

    From a physician, d dimers have little clinical relevance. They are too non-specific to have any significant use. If your clinical suspicion is high enough to be concerned for a PE, you'd skip a d-dimer in favor of imaging.

    @@mathnerdmI had bilateral PE and my d-dimers were slightly increased but the chest pain was excruciating particularly on movement with sats at 78%. Imaging with contrast -clear bilateral PE.

    I would also have asked if the patient was coughing up anything. She said vitals were normal but I would expect possibly reduced oxygen saturation

    Sinus tachy is only 44% sensitive for a PE so more then half of PEs will not have sinus tachy on an ECG

    PEs big enough to cause symptoms and be obvious on CT will cause sinus tachy. Not sure what literature you're quoting. Most PEs are subclinical to begin with.@@callum4387

    Fiber? Fiber is only useful for intestinal transit. That’s their only purpose, to make you sh*t better. They literally have no purpose in the body at all, which is why they are useful at intestinal transit, fibers aren’t even digested and go straight down, fastening waste disposal.

    I will be honest, once you're done with your USMLEs all it takes is 30 secs to answer these differentials. As soon as they ruled out ABGs I knew this was a pulmonary emboli because it's very rare to see sharp chest pain with coughing in an MI/cardiac cause. This is the beauty of doing 7000+ Uworld questions and countless anki decks.

    I was thrown by cough, which is not a common presentation of PE. Hemoptysis is relatively specific but not sensitive at all. (Of the many dozens of PEs I’ve seen, no more than 1 or 2 of them had hemoptysis.) What is more common than cough or hemoptysis? Risk factors, DVT findings, sinus tachycardia, decreased SpO2.

    Hospitals get whatever they want typically, then send the bill later. If you don’t have enough coverage, you’re stuck with what’s left. If you’re too poor, the hospital writes it off as a tax return. They by law cannot deny lifesaving treatment due to lack of coverage

    @@Willynilly_69 It's BS. You don't ever have seconds to make a diagnosis in a stable patient. In an unstable patients you run multiple tests concurrently. So someone is doing an ECG while someone else is pulling a D-Dimer and Trop-T and a portable CXR is organised to exclude other causes, etc. So even with these you can rule in or rule out pulmonary embolism, pneumothorax, pneumonia, aortic aneurism and coronary ischemia/MI.

    I’ve always be frightened that I’ll be in an accident & I can’t get over I have Complex Regional Pain Syndrome in my right leg happen in 06 foot is in state of Equis & wheelchair (big sign so know) lidocaine gel patch & a fentanyl. Leg looks mottled while & purple for periods. If he can’t diagnose me I have to make more of something I’m not sure of 😅so people know, & people can be distracted

    Nope - sinus tachy is 44% sensitive for PEs S1Q3T3 is terrible - not sensitive or specific - only about 20% sensitivity
