In the norse mythology, Jörmungandr or the world serpent is loki's son which also includes hel (the goddess of death) and fenrir (a giant wolf) and a horse. It was said somewhere in the game that Jörmungandr stated that Atreus looked familiar. Mimir somewhere in the game aswell told a story about how Jörmungandr (The world serpent) was sent back in time in the past where he wasn't even born yet when he and thor clashed in Ragnarok

You are in his intestines. That is not the spine you are seeing up top, that's the lining of his intestines. The actual snake is much bigger as you can see by the game files.
