Gerstmann Syndrome

Testing for the four features of Gerstmann Syndrome in this patient with two separate left sided strokes (left frontoparietal ischaemic stroke followed by left posterior parietal haemorrhagic stroke). He exhibits (i) acalculia, (ii) agraphia, (iii) left-right disorientation, and (iv) finger agnosia. Complicating the issue is his obvious nonfluent aphasia (expressive dysphasia) with paraphasic errors (replacing words with associated words (e.g. says 'fork' instead of 'spoon')) and some comprehension issues.
Here I will concentrate on testing for Gerstmann Syndrome specifically.
I will, however, test for his expressive and receptive language functions first so I can then reliably deduce from the findings of the test for Gerstmann syndrome.

    Acalculia (0:52)- loss of the ability to perform simple arithmetic calculations, typically resulting from disease or injury of the parietal lobe of the brain. Agraphia (1:20)- loss in the ability to communicate through writing. Left-Right disorientation (1:56)- confusion of the right and left limbs Finger agnosia (2:31)- loss in the ability to distinguish, name, or recognize the fingers-not only the patient's own fingers, but also the fingers of others, and drawings and other representations of fingers.

    I'm a little confused as I associate the paraphasic errors "spoon" instead of fork as a Fluent Aphasia problem? Is this also common in Broca's Aphasia?

    Gerstmann Syndrome (Fluent aphasia, not nonfluent) occurs due to damage in the angular gyrus, at the inferior-parietal lobe, not frontotemporal as you stated in your video. Though these symptoms fit the mold of Gerstmann Syndrome, your neuroanatomy and classification of disease are incorrect.

    Well as I'm studying neuropsychological diagnostics at this moment with my university course right in front of me, I can say that this comment (nearly) hits the spot. It's typically located in the parietal-occipital lobe, specifically the left angular gyrus.

    +Mohanned Alnammi we are talking about a syndrome here, that means all of the symptoms must be present. Gerstmann syndrome's symptoms are acalculia, agraphia, alexia, finger agnocia and nonfluent aphasia. If any of this symptoms are not present it is not acurate to talk about Gerstmann syndrome as a diagnosis. The man in the video is being interviewed and we can see he is able to respond to the interviewer's questions, wich shows that, yes, his comprehension is still intact BUT he was never tested for alexia during the video so there's no way to know whether he has it or not so there's no way to know (for us) if this is actually a patient with gerstmann syndrome.

    +Ariana Sánchez My resources do not include alexia as a component of the syndrome. I believe according to ICD 9 & 10 that this patient does have Gerstmann syndrome based on the 4 positive tests conducted during th examination. Let us know if your resources states something differently and what it is. Take care.

    We should always take the patient consent until and unless it's a suicidal patient or if he is in unconscious state.. in case of mentally unfit we can ask the patient but his or her consent doesn't matter the legal guardian has to be asked for..

    Gerstmann’s syndrome is what I suffer from after a bullet or wire through head as a child. Stabbed through roof of mouth and through the parietal lobe I would have preferred death when young. I was used by military in banking warfare in federal banking regulation immediately prior to 2001. I can withstand many mini brain strokes due to my brain being like a large mass of scar tissue in the left brain lobe. I don’t burst a big vein but instead burst small veins with less amnesia effect 18 year amnesia from federal robbery hostage situation during 1999 I have had severe depression my entire life which explains my willingness to die. Belligerence was part of my malady as well. I chose occupations and hoped for my death because to me suicide was forbidden During the robberies of 1999 I walked up on robbers and sat among them and they thought I was in their crew. One said he would go by an actors house nearby. His statement was in jest I suppose. A side effect is the inability to recognize shapes. But I tend to recognize horrific images in everyday things like contorted faces in mold on a wall or images of the half dead contorted from ordinary items like a blanket or jacket in the yard. I view it as being in hell on earth. I used to linger in graveyards when young sometimes taking a flower from a grave I also walked in my sleep when young. I also was able to escape the robbery of 1999 by walking out of a federal office and passing out in my automobile and then being awakened by a robber and driving off. I was passing out between sacramento and San Francisco and between San Francisco and Santa Rosa California. I have not been paid workers compensation. I would prefer painless death It is my belief in 1999 we lost because I have not been paid and the attacks on the wtc and pentagon in 2001 followed. I am labeled criminal and insane by a doctor or agent on federal government medical records when I mistakenly trusted them in California in 2001. I was taken hostage and knocked unconscious in 2004 due to notes on medical records from 2001 by foreigners in federal healthcare in California in 2001. Federal agents allowed a sheriff to steal and abused me instead in 2001. The sheriff was terminated in 2005. Jesus said heal thine self physician. I no longer use government healthcare and want to be euthanized. It would be easier to have a sex change. The USA government does not want us to take our own lives. I was also shot through testes or operated upon and I appear infertile. I look forward to my death. I do not believe there is a god as proclaimed in texts. I do believe in mercy in death and see it as a big sleep. Danglng chad as I call him is my illegitimate son. He is a mifepristone abortion pill survivor. He has microcephaly and learning and emotional disability. He goes to court December 8 2022. I wanted to euthanize he and I in 2021 but he declined. Dear lord what have thine wrought?
