I believe that this was directed towards his daughter, Nancy. Though I, as well could be mistaken. But on the 'A Man and His Music' album he recalls it as quote unquote "To me a miracle is a baby girl. My first miracle was called Nancy. And now that I look back, a proponderence of the finest women I know are called Nancy." So I guess it's for you too decide.

I think i saw Sinatra comment that the song was related to both his first child and his first love, as he named his first child after her

This is why Sinatra was and still is the best. And this is why a mob guy who was going to have Frank hit cancelled the hit after listening to a Sinatra record.
