Foot Drop Peroneal Nerve Injury - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes the condition known as foot. Drop , which occurs due to Peroneal nerve injury . Causes and treatment of Peroneal nerve injury is outlined .
What is foot drop?
The inability to raise the front part of the foot due to weakness or paralysis of the tibialis anterior muscle that lifts the foot.
A common symptom of foot drop is high steppage gait often characterized by raising the thigh up in an exaggerated fashion while walking.
When the person with foot drop walks, the foot slaps down onto the floor.
Foot drop usually results from injury to the peroneal nerve which is susceptible to injury at any point of its course.
The peroneal nerve begins from the L4,L5, S1 and S2 nerve roots and joins the tibial nerve to form the sciatic nerve.
The sciatic nerve starts n the lower back and runs through the buttock and lower limb.
In the lower thigh, just above the back of the knee, the sciatic nerve divides into two nerve, the tibial and peroneal nerves, which innervate different parts of the lower leg.
The common peroneal nerve then ravels anterior, around the fibular neck, dividing into superficial and deep peroneal nerves.
The deep peroneal nerve gives innervation to the tibialis anterior muscle of the lower leg which is responsible for dorsiflexion of the ankle.
Conditions causing foot drop:
1-L4-L5 disc herniation
A herniated disc compressing the L5 nerve root may cause foot drop.
2-Lumbosacral plexus injury due to pelvic fracture
3-Sciatic nerve injury: hip dislocation
The common peroneal division of the sciatic nerve is commonly injured during fracture dislocation injuries involving the hip.
4-Injury to the knee: kee dislocation
In the event of a knee dislocation, it is important to check for common peroneal nerve and popliteal artery injury.
5-Established compartment syndrome
•Foot drop is a late finding
•Irreversible muscle and nerve ischemia occur in patients if fasciotomy is not performed.
•Fasciotomy should be done early.
•4 hours of ischemia may be tolerated, but by 8 hours, the damage is often irreversible.
Treatment of peroneal nerve injury/foot drop:
•If a disc herniation in the low back is impinging on the nerve and causing symptoms of foot drop, then the herniated disc should be treated or removed.
•Obtain EMG and nerve studies of the patient.
•Recovery may take 1 year to 18 months.
•The big toe is the last to recover.
•If no recovery is achieved, explore the nerve for repair, graft or tendon transfer.

    My son sprained his ankle whilst running earlier this year. Afterwards he was unable to move or lift the foot and it felt numb. It took just over 10 weeks before he recovered and was able to regain function of his foot. I am posting this here because nerve injuries can be scary (and I could find so little information at the time). May his story with a positive outcome give someone the hope they need on their journey to recovery.

    ​@@Krishna-fn5nv All it took was time... The time it takes for the body to heal really depends on how severe the injury is. Nerve injuries unfortunately can take a long time to repair which is why you must please see your doctor for a medical diagnosis. Speak to your doctor about vitamins to boost nerve repair. My son also saw a physiotherapist who assisted with appropriate exercises.

    I have drop foot due to damage to the nerve in right leg from a broken fibula. Hit by a SUV in a crosswalk. 7 months ago. Am taking therapy for drop foot. Pray nerve will regenerate. 66 yr old female that was independent. Now i walk with a rollator or cane. Great explanation on video

    I still have foot drop 18 months after the hip replacement surgery that triggered it. There’s some improvement to the movement and lift of my foot, but I still can’t wriggle my toes or properly pull my foot up towards me. 🤷🏼‍♀️ It’s doubtful I’ll ever have a full recovery now but I make sure I work the muscles and tendons every day.

    I was diagnosed with foot drop in late June 2023. The neurologist said the cause was likely the result of entrapment of the perineal nerve in the right knee due to my weight loss of some 60 pounds in previous months. I began physical therapy in September. In my follow up with the neurologist this week he said that my foot strength and range of motion is normal.

    Thank you for the information. Unfortunately getting the proper care is not easily obtained. Trying to impress my Primary Care Physician is the hold up. By the time one gets to the final stages you mentioned, critical time elapses. I will advocate. I will be guided by your presentation and will make suggestions for my treatment moving forward. At this time I don't have an etiology for my sciatica and neuropathic symptoms. MRI suggest L4-L5 stenosis. God Bless you. Any suggestions?

    Sounds like what happened to me. You know us guys don't like to go to the doctor. So we make excuses why our hand is numb. First it's the fingers than its the hand and it's the arm. By the time you say I better get it looked at it's in your leg. So that's when the run-around starts. They start sending you two doctors and physical therapy and chiropractors and nothing seems to work. Then you finally get to the right doctor and he tells you you should have been in here months ago. He schedule surgery he says if it gets any worse to go to emergency. This will help speed things up with the insurance BS. I wish us well 🙏.

    I have had 1 cervical disectomy & lumber L4 &5 disectomy L5 S1 operated on twice & L4 & L5 Laminectomy. I have terrible numbness down my left leg & into my foot. I now have stenosis. My issue is my walking is terrible. I have drop foot but not confirmed! Why is my numbness worsening, my last MRI scan was 2019. Showing 2 bulging discs but not serious enough to operate on! Obviously I’ve no claudi equina but I only feel like urinating after 9 to 12 hours later! Is this a concern

    I blew my knee out sideways and tore 4 ligaments and my nerve. I have had foor drop for over 20 years now. With this long of an injury will it cause back damage due to the gate of my walk? I am having severe back issues and am wondering if it is caused from my complete foot drop?

    Crossing your legs causes peroneal pinched nerve. Easy fix! I was misdiagnosed with trochanteric bursitis.I finally recovered after I stopped crossing my legs.

    My son suffered anoxic brain injury from dying and loss of oxygen to the brain for at least 10 min. The icu didn't "keep up" with the bracing schedule and thus my son has bilateral foot drop he is still mostly paralyzed. He has come of the vent and only on feeding tube. What can I do to help this condition that is mirrored in his hands also. His toes are pointed and turned inward and cannot be flexed to a flat state and his wrist are bent over and drawn under his chin in t-rex arms. What can I do to be proactive and not allow him to degrade farther? I wish I had known those early days how serious this is. While everyone was expecting him to pass away I was focused on him healing. At this point he is aware, breathing on his own and trying to learn to eat and speak again. While he works on that, I want to be prepping his feet and ankles for the work ahead. Please any information is a blessing! Thank you!!!

    After my son kicked the soccer ball he had a drop foot symptom, went to the emergency room, thankfully after 7 to 8 hours later he became able to lift his foot again and walk much better .. is there anyone who have had this happened to them.. it was very scary several hours

    @@xdior752 hey bro, it’s been about 5 hrs and I’m getting a lot better. What you need to do is make sure that you rest for a couple hours with little to no movement and every now and then grab the foot that is hurting and pull it back slowly so it can get used to go back up slowly. Do this about twice every hour for a minute. You’ll start feeling better.

    this is the best video I've found that explains this foot drop thing in a way that makes sense. is it possible to be born with foot drop? I'm not sure if I have it, but my dad would get upset about me dragging my feet since always, even though I do my best to lift them. sometimes I trip for apparently not lifting my feet high enough.

    Hi, my name is Safanja Dorff i am 27 years old and i am born with footdrop on both feet. This condition has caused me to fall multiple times. I have weak toes, not a good amount of strength to lift my upperfeet, i am limited in wearing shoes, my knees and ankles have witnessed many painfull injuries.😥 Even today (Sunday) in this new year of Jan 2022 i have scraped my ankle and fell down on both knees.     I had trouble dropping, and especially difficulty moving my left toes up and slightly down, so I had surgery on three of my lumbar discs on the advice of doctors. But now, 45 days after my surgery, the problem of my left toes not moving has not been resolved. Where does the problem come from and what is the problem? Am I being treated?     I developed sciatica after working in the garden three weeks ago. I had what I think may be partial footdrop. I found it very difficult to pull my toes towards my leg but could chieve some pull. I also was in a lot of pain. I await a referral to the muscular skeletal unit. I have noticed that the pain is easing and the foot movement is increasing but still not complete and I have been slapping my foot still. Is this improvement a good sign that the nerve is healing?

    Yes. The pain was gone after three to four weeks. It had been the worst I had ever experienced but was gone by December. The scariest thing was the numbness and partial footdrop. I did the physio, the cobra, and constantly wagging my ankle, and going for walks upto three miles. I did not know whether this would get better on its own, but it did. Four and a half months later I am walking properly and the numbness at the front of my leg has gone. It still feels a little bit different but gets better every day. I can confidently tell people to avoid surgery. My MRI scan was scary, my disc was extruded at L4 18mm and pressing on a nerve. It still got better.! @@KingZTV78

    @@sandracrawford9813 Awesome!! I currently have been experiencing it since the end of January when I aggravated my sciatic nerve. My entire right leg had pain but my foot felt like it was losing circulation and as if something was pressing on top of it. I had X-ray exam in my back but they didn’t show any damage. I don’t have any pain or numbness now but I still have foot drop. It doesn’t cause me to trip or anything but I walk with a limp and not able to walk as fast as I used to.

    Yes, I had two patients with drop foot and they recovered after treatments to their lower back L4/5 and sciatica nerve.

    Yes, there is a one month rehab with different exercises that the patient must do. I monitor them and once their physical therapy is satisfied they can stop these exercises but must keep up with daily activities

    I was recently burned in a fire with 25% of my body burned then on my first week of treatment I fell asleep wrong on my left side because that was the only side not burned and somehow cut the blood circulation off on my entire calf muscle and foot now have no feeling and can stand in toes

    By the time you fall a couple of times, get to the Dr., have X rays & various tests run its already too late. Once that nerve is damaged that's a wrap!!

    I got blood cancer my foot was droped at the age 9 I treated it with some exercises and thearopy ...... But after. 2 years I leave it ..... Now I am 18 I feel my foot is not growing . I cant mentain my balance and I feel disturb in walking .....and my feet skin is changing too means its like damaging ....please tell me any treatment or exercise for this..... And this also effecting on my brain means. I can't sleep at night I feel pain

    I respect you opinion but your just trying to make money man this can definitely recover on its own with physical therapy. You don’t always need to have discs removed or nerve and tendon surgery. This is like what a dirty doctor told me thankfully I got second opinions.

    Hello everyone if you read this, I had an EMG test done for hotness of my bilateral feet. The next day from that test my left leg became weak, complete numbness in my left shin, and weak foot. Anyone relate ? I’m going through a medical malpractice to sue.
