Hey All!
Let's discuss the critical difference between a retinal detachment and a posterior vitreous detachment. Both can cause flashes and floaters and require an eye exam. However, only 1 is a true eye emergency. Let's jump in!

    I just got diagnosed with PVD today and was trying to get more info on it. I came across your video. My symptoms were floaters and flashes of lights only for a short period. The floaters are still present. I work for several eye surgeons and when I went in to describe my symptoms today, I was immediately examined and given this diagnosis. I think the flashes of light was the scariest for me. Thank you for posting this video!

    Thank you for the very clear explanation. I live in the UK, 3 weeks ago, a floater came in my eye. I also have flases, the very next day I went to a optician, they gave me a scan and a Manual test. They said they couldn't see anything wrong. I was worried, 2 weeks later I went ti another optician and explained I had been for a scan, they said its not worth doing another scan as it was only 1 weeks ago, but they gave me. a manual peripheral eye test. It came back ok they said. normal. The thing was they never described the potential of what it could be, they only said if you get more floaters ir flashes, then come back. Do you think I should book another test a month later ?

    I always examine floaters at least twice on 90 days, sometimes 3x. A retinal detachment is always a possibility and so I have very strict surveillance

    There are some doctors who recommend activity restrictions. Here is a post on the American Academy of Ophthalmology “To my knowledge there are no data showing that normal chores around the house lead to complications from a PVD. I always tell patients, however, to avoid activities such as skydiving, bungee jumping, or bumper car rides, where there is potential for whiplash injury, when they have been diagnosed with complications of PVD such as those mentioned above (retinal tear, hole, or retinal detachment).” In summary I would check with your eye doctor first to make sure everything is healthy and then go about your day. Just be very cautious and pay attention to your symptoms. Please consider liking and subscribing as it helps builds the channel. What other questions do you have?

    23 year old here. It has been 2 days and I have just started noticing eye flashes.They are not like a lightning flash or a camera flash that some people usually point out but a very very tiny peck of light in my peripheral vision in the right eye (as if there is a surface reflecting light on my eye). It's there for less than a second then it goes away. No other symptoms. Also because of my job, I am mostly in front of my laptop screen. So could screen time fatigue also cause these symptoms - like an eye flash or any new floaters ? because my screen time has increased quite a lot in the last few months, and I am trying to cut it down as much as possible.

    Typically dry eyes / eye strain is different than this. I would also recommend a dilated eye exam with your doctor to inspect the retina

    Hi Dr. Jeffrey Tran! How far in millimeters does the vitreous detach from the retina in PVD? Retina doesn't have that support anymore? Is it helpful to gently press the eyes against the retina - as a kind of eye gymnastics? Is there a risk to the retina when examined with Goldman lenses? My pressure seems pretty strong? After one such examination, one of my eyes gave an image of a slightly pink color for about 15 minutes - then this coloration disappeared.

    Thank you for watching and commenting. Please consider liking and subscribing. The vitreous is jelly-like so it detaches a different amount depending on the patient. The retina is adherent to the back of the eye. The support is not from the vitreous l No, I do not recommend those activities I always recommend an examination with a 90D and 20D by a doctor. The pink light was probably due to the bleaching effect from light on the retina

    Thank you very much for the very informative video . At what point can a patient with PVD resume regular exercise - that includes weight lighting and jogging . My doc did say no heavy lifting for one week . But based on the follow ups since it could happen down the line , would exercise aggravate or cause an RD ? Thanks so much .

    I usually tell my patients to resume all normal activities without any restrictions but I see them for follow ups. A retinal detachment can still happen, but I find that close follow ups and strict return precautions are the most important takeaways.     do the curtains ever come from the bottom to the top for a shadow i was watching tv and i seen a dark shade come from the bottom but it went away im not sure if its the tv or my eye.. i do have mild cataracts and some eye floaters im diabetic.

    Yes it can. Based on that description, I would seek an eye exam ASAP. First thing to do would be to call your local cataract surgeon and ask them directly for medical advice. Please note, this is not a medical advice, but my KZread channel is meant to discuss common eye conditions.

    No. The AC can cause dry eyes through evaporative tear loss

    I have this PVD Symptoms, With just one floating "stick", not too disturbing, and Flesh lights on the side (it was more at the beginning, but very few now - after 5 days) I visited an eye clinic, and I was told that my retina is complete and healthy, not injured. My question is, that since I am a personal trainer and yoga teacher, so my whole life is about sports and moving, what is important for me to know for the future? Thank you in advance!

    Great question. Thank you for watching. Most patients will still notice the floater, and most will learn to “ignore it”. I would always monitor a retinal detachment, so if you have flashes and floaters or changes in symptoms, go see your eye doctor asap. Remember, your eye doctor saw you and so always trust their advice too

    Dr. Tran, I have PVD for 2 years. The lights have a phosphorescent soft glow, in the corners of the eyes, just below the eyebrows - they are in the shape of brackets, (a thin crescent) and move quickly only downwards and I can only see them in the dark. I also have strange photopsias (outdoors, short-lived, not permanent) - they are cottony in color - spots about 5 mm. - and they move slowly - like in a liquid, but also from the bottom up - in all directions (on the periphery) Doctors only believe the examination - they say that the retinas are ok. I have a Weiss ring in both eyes and floaters. How can a doctor know if PVD is complete? Maybe it's not completely in my case - as long as there are lights? And more :) Any recommendation - how to protect the retina (as much as possible) when sneezing and coughing? Отговор

    I am having the same problem, I got cataracts out in both eyes, but before that I had a Weiss ring appear in the left eye, then after surgery in the right eye. I had 2 dilation exams before and after surgery with nothing wrong seen, I go back in July, but the fact is that I am having flashes everyday especially in the dark, also many floaters. I keep reading the the flashes go away after complete detachment at around 3 months, but I am still having them.

    @@omfug7148 How long have you had flashes? I've had my third year now. In my opinion - although it is considered "complete" the separation of the vitreous remains connected and pulls the retina constantly - which terrifies me.

    @@userivanov4566 Several months now & they seem to be getting worse, I see them more in the dark, but they appear just as you describe, at the corners of the eyes sort of shimmering. It is depressing and I guess that nothing can be done about it unless there is a tear?

    @@omfug7148 When we have PVD but no retinal tears or holes - doctors just watch. They don't do anything. However, if we get a dark curtain, a rain of soot or a lack of vision in any of the visual fields - urgently, we need to visit a hospital with quick help. If the tears are small - they do laser therapy. If there is a detachment of the retina - a difficult and uncertain operation follows. I used to go for checkups every 20-30 days because I was scared to death. Now I'm going 3-4 months - but I'm still scared to death - because I know a lot more than before Flashes are dangerous - because they show that there is a pull on the retina from the vitreous - purely mechanical - two surfaces are coming apart - and no one knows how finish this Always during an examination we must demand that they look at us with the "Goldman" lenses - because only with them can the periphery of the retinas be seen. Also, we should not lift objects heavier than 5-6 kg (for the rest of our lives!)

    I've just had a PVD and had two eye exams with retinal photographs under a dilated eye and an eye inspection. I have been told on both occasions my retina is fine but there are two strands of the vitreous attached near the front of the eye where it is also collapsing. I have a small blind?dark patch in my front left vision, which I had on my last inspection, but no damage to retina. Could the vitreous pulling on part of the eye cause this weaker/vision loss? It's been like this for two weeks? Thanks and sorry for the question as I know it needs a proper examination to confirm.

    It’s hard to say. It seems like your eye doctor is doing a good job. You could be having vitreous macular adhesion but that would cause a distortion.

    @@DrJeffreyTran As I was not entirely satisfied, on my optician's advice I was triaged in Oxford eye hospital for a closer examination. It turned out to be a detached retina, which was operated the same day. The surgeon could not see the teal that had caused it on initial inspection, so my symptoms actually appeared before it could be visibly detected. So glad I trusted my instincts and went again.     @@malena7362 No, I had a shower of new spots and floaters in my vision, but my blind spot was like a curtain/shadow that started in my peripheral and began creeping towards my central vision slowly. I've been lucky overall!

    Hey,i have floaters since last year now i am seeing stars like in my vision..i got indirect opthalmoscopy buy dr.did not performed scleral depression..and said everything is ok..buy i am nearsighted...i am worried what should i do ..should i go to dr again i am in my late 20's....pls somebody suggest me something

    Oh boy! I had severe flashing and huge floaters 3 Sunday nights ago. (This comment is on a Sunday night, 3 weeks later) I went to my eye doctor the next morning and he verified it was only a PVD and I shouldn't be alarmed. He gave me specific instructions on how to recognize if it became a retinal detachment and told me to feel free to come in if I had new symptoms that worried me but he would make an appointment for one year later. I've continued to have more floaters and less but still present flashing, but nothing as bad as that Sunday night. After hearing how you make appointments on day 45 and day 90.....I wonder if I need a different doctor???? Can you please advise me?

    There is no one right way to practice medicine. I would rather have my patients come to me several visits for me to give peace of mind than to state call back if you have questions or concern. A professor of mine (when I was in training), always taught us that philosophy. He actually saw his PVD patients at day 15, day 45, day 90. Again, the doctor who knows you best is the one who examined you. I hope that helps

    I already had two examinations and no detachment so far.I have now floaters and flashes,especially when I move my eyes.Can those flashes go away and is computer use favoring this?

    Computer use won't impact this. It can take time, but most of my patients do report their symptoms resolve with time

    Why do doctors say that pvd usually leave 3-6 months . And are pvd floters darker or more aggressive than regular floters?

    Im not sure what they mean by "leave by 3-6 months." What I often tell my patients is that, you often may learn to ignore the floaters. They never quite disappear. The analogy I like using is our noses. Typically, when you are driving you probably are NOT paying attention to the visual field obstruction your nose blocks. But now that I mentioned it to you, you are focusing on that area. But in about 5-10 minutes, your attention will be drawn elsewhere, and you won't be paying attention to that blank space anymore. Same thing with floaters. Most learn to ignore it because your brain realizes its not an important area to focus on. This can take up to 3-6 months in many patients. Not every though learns to ignore the floater. Your risk of a retinal detachment is elevated in the first few months after a PVD but it NEVER goes down to 0%.

    A PVD is a specific description of what your vitreous has done - it has separated from the retina. A floater is anything that moves in the middle of your eye. So a PVD is more specific . Its not necessarily more aggressive.

    There is a difference between non mobile floaters and mobile floaters maybe the analogy with the nose works for those that don’t move but the ones that are very fluid and mobile I’m sure it’s hard to adapt to those. And why do they say it lessens with time the darkness of them?

    Saw my retinal doc for PVD. No retinal detachment, but the flashing is constant. Looks like a ball of flame. Visual acuity is normal. Is this typical?

    ​@@bluejay9890 I'm 7 months into PVD and still get the flashing. I was told in 3 months the symptoms would subside. Now I'm not sure what to do, especially since a lot of eye doctors seem to be wrong.

    @@094340 Sorry to hear that. I'm still occaisionally seeing lights. It's so annoying, but I've seen 3 eye doctors and none are concerned about it. One said the flashing may never go away. Ugh.

    @@bluejay9890 The more comments I read, the less confidence I have in eye doctors, there is so much conflicting information. For me, it's not just the flashing, but also pressure and a bit of pain, been like this since before the PVD. Yet, when I had my initial exam, all the doctor mentioned was the PVD. I scheduled an exam for this coming Tuesday, so I'll hopefully find out more then. But the flashing tells me that the PVD hasn't fully resolved, so I'm a bit worried about the whole thing. I wish you the best.

    Pretty rare. I've seen 5 flashes and floaters patients a week, and I think I've only seen 4 over 2 years

    You can always try the county hospitals! I used to take care of many underinsured at the county hospital (like Cook County in Chicago, parkland in Dallas, Grady in atlanta)
