Thanks Gail. Sadly, many are done poorly - easy to go out into the weeds with these. Also, a causal tree is a good tool for when we ask "why?" and there is legitimately more than one answer.

This needed at least one more why - why were they short 1 nail? Could have been a supply chain issue or someone forgot to make/order nails - both responses warranting yet more whys. This example doesn't quite get to the actual root of the problem.

Thanks LK - while this classic example has been considered for decades to have reached an actionable level you bring up one good point - if we ask why? and legitimately have more than one answer, then moving to a tool like a causal tree works better. We can also take the actionable terminal point on a 5-why and start a fishbone diagram if further diagnosis is desired.
