Fistula Story and Treatment

Hi Guys,
Today I share about a quite uncomfortable topic- Having a fistula. I have found it so hard to find relatable information about fistulas (probably because they are something no one really wants to talk about!), and so I have decided to share my experiences and hopefully, soon, the solution I have found to this problem. It's called Kshar Sutra treatment (medicated thread), and it's been available in India for thousands of years but is hardly known in the west. Thanks so much for watching! I hope to hear from you.
*Update- Have now healed. Check out my "Fistula Vlog" playlist for details*
Also, if you have a fistula and are looking into Kshar Sutra, Norma Johnson has created a great Facebook group: Kshar Sutra For Fistulas.

    *I ended up going to Dr. Ramesh Bhat in Bangalore and getting a partial fistulotomy and kshar sutra, and I have been fistula free for about a year and a half now! I definitely feel it's the best choice I ever could have made and am so happy to finally be living life without a fistula! Good luck everybody!

    @@jenelynfortaleza6332 hi Jenelyn, I am really sorry to hear that and I understand. For me it was very helpful to take sitz baths with hottest possible comfortable water temperature and Epsom salts I would use a bucket and squat which I think also is helpful (squat position) for general health of the area. I also took oregano oil by mouth when it felt inflamed And ate a diet which did not cause inflammation For you, maybe it is different but these things made a huge improvement for me, until I could eventually save enough for kshar sutra treatment

    I thought I had a hemorrhoid at first. I was in the most pain I’ve ever been in. I started googling and realized it was either a hemorrhoid or an abscess. I went to the ER because I was starting to think it was an abscess and not a hemorrhoid. I figured the ER would be able to determine it and treat me for it right then. The doctor wrongly diagnosed me with a thrombose hemorrhoid and told me to go see a surgeon. I went to another doctor and was told that it was not an abscess and that it was a hemorrhoid. Finally I went to a specialist and he immediately knew that it was in fact an abscess and needed to be drained immediately. Most doctors are not prepared to deal with a perianal abscess. I was in the worst pain of my life for three full days. They did the draining procedure today about two hours ago and I feel 80 percent better. It’s a miracle. If you think you have abscess you are probably right. Do not let them tell you it’s a hemorrhoid. That was the worst pain I’ve ever felt.

    Thanks Maxadon, Yes that's exactly why I made the videos, I didn't talk about it for years and it was horrible, so I'm hoping that in some way I can make other people with the same problem feel less alone. I have all my fistula videos in a playlist on my channel- the most recent is from April, and I'll probably just do one a year moving forward since there isn't much to say anymore! But who knows. I'm glad it's helpful and all the best! :)

    I had a complicated fistula some time ago so I feel for you. I went to several doctors and no one knew what my problem was. When they finally cut and drained the abscess it was torture. I had surgery but it returned. It wasn’t anywhere near as bad as before the surgery so I opted not to have it done again. It healed by itself after a few months. Come back once more then went indefinitely. I’m sure that had I not had the surgery it wouldn’t have gone away. From start to finish it was about a year. Many months of pure hell where I couldn’t sit, walking with a limp and at times unable to walk due to pain. I was afraid to eat cause about 30min after a bowl movement I’d be in serious pain. So I was of course afraid to go to the toilet, too. If any good has come out of it I’d say I appreciate being healthy and things like colds or other ailments don’t concern me at all anymore. Once you’ve been thru hell small things do t bother you.

    First of all thank you so so much for sharing your story. I have a rectal vaginal fistula for about a year now and I’m in pain every day I’m depressed and confused and scared to death that it’s never going to go away. I started with cervical cancer then got colitis and between the two I has destroyed my body. I’m in Canada also and I do have an amazing team of doctors and I m extremely grateful for that but it doesn’t take away from how serious this is and that I’ll most likely be incontinent for the rest of my life or there is a chance that the fistula will just keep reopening. Going through cancer was easier then dealing with this if anyone has had a rectal vaginal fistula post radiation I would love to hear how you felt with it. Thanks again for sharing your story

    Thank you so much Amanda for sharing. I am sorry to hear what you have gone through. I'm hoping things get better for you. It may be worth contacting Dr. Bhat in Bangalore and telling him about your case. He takes on difficult cases if he is confident he can help the person (and not otherwise) so it's worth asking him....there was a woman there with a vaginal fistula from difficult childbirth there who arrived totally incontinent and then she recovered almost fully (to be much healthier and able to live well again) I really hope the healing continues for you and all the best. 💜

    Thank you so much for this video🙏🏻 I noticed a soar bump on my left bum, 2 months ago. It wasn’t so painful but went straight to hospital. Receptionist directed me towards general surgeon. I was told about fistula and given some antibiotics. Painless bump disappeared and reappeared several times in a month duration so went for ultrasound where a tract was seen and did MRI which showed a thicker track. Then I was told to go for fistulectomy and anal advanced flap repair according to my diagnosis. Done it 3 weeks ago and took bed rest as something kept oozing after surgery. Surgery was done with spinal anesthesia and sleep sedation I believe. Only downside was some fear 👻 I had to undergo post surgery as it’s my first surgery in life. Holes were kept opened to dry naturally. It’s healing but seems like it will take many more weeks to heal completely.

    You have come a long way, thanks for sharing a very hard topic. I have found that plant life forces things to heal quickly, However, I walked into a pine tree and it cut a C shaped cut in both eyes. The results was that both the skin flap and the eye ball healed very quickly because of the plant life material. BUT they didn't heal TOGETHER so the flap never wanted to stick to the eye ball. Many years later, it has stuck together. But for many years, each time I had REM sleep the eye movement would rip the flaps and the pain would be unbearable. I thought that was the worst, until I seen this video.

    @@HackYourHealth You should have seen the look on my face when you explained what a fistula was; I had no idea that it existed. After you explained it I kinda wonder why the fistula doesn't close. I guess it is because it attached to the bowel. I cried a little just thinking about how that would be for someone to deal with, then I really felt empathic when I realized how mentally strong that you would have to be because this would have really stressed me out. Thank you so much for your videos, because of your videos, I found the book.

    I too was misdiagnosed 3 years ago. I finally went to see a different doctor after I had blood draining episodes that were very humiliating. I saw this new doctor in early October of 2019, It was then that he told me that is was a fistula and scheduled me for surgery right away. He did the same thing, placed a Seton, looks like a rubber band. I've been seeing him once a month to tighten the knot for the Seton to continue cutting the skin. While it's cutting the skin. it's healing as it's moving and closing up. Like you, the doctor told me that the seton will eventually fall off and the fistula will close up. It's January 2020 and the Seton is almost to the end. I'll be so happy when this is over.

    I have been fortunate enough to have a Dr that has gone through this situation. I use "fortunate" lightly because I wouldn't wish this upon a good person. When I explain status or experience he can relate better than a Dr with text book knowledge. He works the system as much as he can to get things advanced for me and I know how much time he has knocked of in regard to specialist visits. But there is still a system and it takes time. He works to make that time a little more bearable. I have seen docs that have told me that I was just dealing with irritable skin. A world of Wonder.

But as a mom I was devastated for her .. seeing her in so much pain with this ugly fistula at such a young age .. It has been about 3-4 years since she had the surgery I would be so interested in hearing about your procedure and how it has worked for you … I’d love ti see her completely rid of that horrible problem … We are in BC … not sure what part of the country you are from … Thank you for being brave enough to tell your story … it’s a very hard thing to talk about for sure !

    It seems to take a minimum of about 6 weeks to a maximum of 6 months with most people being finished in 3 months or less, from what I've heard. For me, it took 3 months and that seems fairly normal. It depends on length and complexity of the fistula, and he doctor.

You can also ask your doctor that treated you what he recommends... this is only based off of my own experience. In my case, I wasn't eligible for any surgery to close the fistula at home, and decided to go to India for kshar sutra, and that's what finally ended up working for me. But I lived with it for 7 years before that, and sitz baths were basically how I got through life lol. I'm sorry to hear you are having a similar experience. I thought this would be a good forum to share my experience. Hope nobody minds. And as of writing this I am still yet to watch all your videos so I look forward to reach the end to see how you are doing now. (Info below) For anyone that wants the full story please let me know and I can answer any questions. For now I'll just talk about the fistula itself. I've had my fistula for about a year and a half. The treatment I have done is choosing to do nothing. In this time I've gone to the emergency room once. This was because of the external opening turning purple/black one day, and me thinking it was possibly necrosis of the wound (I can't certify I am correct, this was just an idea thrown when I googled reasons that skin turns these colors). Emergency room staff wasn't used to dealing with this kind of problem. They diagnosed it as a chronic wound (which, actually is not a totally incorrect diagnosis). They ordered a CAT scan and found no cancer, etc. So they let me go. The experience/wait time/price for service are all things I can rant about some other time, but I left without a true solution to the fistula. The next day I scheduled an emergency appointment with a colorectal surgeon. Before I touch on that experience, I should mention that after a few days my body was able to crack an opening again for drainage (it did take some intervention from myself), and I came to the conclusion that what happened was the external opening closed, and there was a recurring abscess in the making. A very common risk of leaving a fistula untreated. The consultation involved an anoscopy and in the clinic setting, the surgeon confirmed a fistula. He was very quick to offer a fistulotomy, and talked about taking muscle from my leg in case it is needed to attach to the sphincter (these procedures are usually supposed to be last resorts, if too much was cut, which kind of freaked me out). Despite his accolades, certifications and experience I did feel like he did want to comfort me, but I could sense that he was downplaying a lot of risk factors, which left me with no choice but to research more. He did a great job, so most of my feelings/reservations are my own fault since I get bad anxiety having to think of these things. It is very hard to find information on long term outcomes of people who have taken my route and elected no surgical procedure, so I feel like I am alone on this one. The closest things I found discussed that surgery must be done or else the fistula can foster cancer growth, and the obvious that it does not close on it's own. He asked for a follow up after a pelvic MRI which did conclude a 3cm fistula was present. Sadly the report did not indicate wether or not extensions of sepsis in neighboring areas of my body were found, or the classification of the type of fistula (depending on the type, fistulotomy can be a decent answer) (Also, I found out about these missing details when finding a video about MRI and fistulas). To this day that is the last time I went to the surgeon. I am still not certain on what to do. But I can definitely say that I might have developed anxiety issues this year due to this problem. I can't go a single day in my waking hours not thinking about this problem, as well as having feelings of dread, death, etc. since it is sepsis in my body. I can still work, and on the surface my day to day life appears normal to the average person. However, I would probably say if my body has been able to hang in there without suffering harsh pain I must not be dealing with the worst fistula out there. But the idea of surgery has evolved into a phobia, and sadly any small pain that happens to me these days I blame on my infection. I do have Crohn's disease so just on that principle I don't want to deal with incontinence since it would dramatically change my quality of life. I have considered full fistulotomy only if I am to be found with cancer, or sepsis in other parts of my body. I can say that in my case, it has been manageable physically, and the worst part would have to be always making sure there is drainage, to avoid an abscess. This, along with the anxiety, which I can't exaggerate enough - Causes me to eat once a day and make me lightheaded/feverish at times due to how much I stress/obsess over this. So I am learning to keep my anxiety under control and trying to eat. Wishing you all luck out there. Everyone's problem is different. Some simple, some complex. If I can give any advice it would be to really weigh your options before surgery. Once you are put to sleep there is no going back to what the surgeon might do. You know your body best. And do research. Lots of research.

    Thank you so much for sharing. Your thought process is similar to mine. I must have found some of the same videos, because I also heard about the cancer thing and became very terrified, as I lived with it for over 7 years and it was to the point where I started to worry about that though I really have seen no evidence that that theory is true. I thought about it all the time too. My best advice would be to deal with it as soon as you find a solution you are comfortable with. For me, it was kshar sutra. Because for sure life is better without it. However, like you I did not want to risk incontinence, so I agree that going under and not knowing what they'll do to you...That's something i did once and if I have a choice won't ever do again so i completely understand It isn't easy, but I'm glad you are educating yourself. Don't worry, I have now met many long term fistula sufferers and there haven't been any long term consequences besides the anxiety you describe and the results of any surgeries. Thank you so much for sharing and I really hope you get a good solution soon!

    Thanks for answering and reading. It does bring me a sense of peace knowing that (at last from the sufferers you know) there really aren't other things aside from anxiety you've noticed. Also it is good to know that the cancer speculation might be a myth. Since such few sources even brought up that word it must really be extremely rare. Educating myself has really been the best thing. I just try to take it one day at a time. I am very happy to hear about kshar from you. Before this I haven't heard of it. It gives me hope that in other parts of the world there are sphincter sparing options that have been your answer. I've read some research papers about a very large percentage of southeast asian citizens who suffer from Crohn's having to deal with fistulas. I feel like most of the innovation will start there before the west just because of how much more common it probably is. But perhaps I will consider this more non-invasive method if I am able to save the money. My only hope these days is for no more abscesses, no fistula complications that will corner me into invasive surgery, and keeping my anxiety under control. I've been watching too many surgery videos and reading about incontinence fix procedures, so it's a relief to finally turn something on where my mind can be exposed to the idea that there is hope out there. I'm trying to restore my body back to a state of remission right now and working to get out of a flare up.

    Hi Anne, I am sorry to hear you are in pain. I know how hard it is. I made a video called "Tips for Living With a Fistula" where I shared all of my "strategies" regarding feeling ok with a fistula. Of course, I also know how hard it is once the abscess is occurring. I became very sensitive to antibiotics, so I would take oregano oil (a few drops) several times a day, as well as taking frequent sitz baths. In a flare I'd take multiple sitz baths within a short period of time (like three within 15mins- 1 hour of each other) with fairly hot water and epsom salts, and I found it helped. This is just what helped me, it is not medical advice and I am not a doctor. Of course, from a distance I cannot know what the best thing for you is. I hope you can manage your symptoms and feel better. And of course, if you need it made sure to seek medical care! Sometimes they can drain the abscess. All the best to you

The seton did help. Your doctor sounds like he didn't think too deeply about the experiences of the people he did surgery on, especially since he was a CRS I imagine he sees a lot of fistulas. I've heard similar stories too, and noticed a lot of Canadians (relatively to how many we are!) leaving reviews on Indian doctors' review sites regarding kshar sutra, also from US and Australia etc of course. Is kshar sutra same as cutting Seton in australia

  • @HackYourHealth

    @HackYourHealth

    Жыл бұрын

    No. Cutting Seton does not kill the infection from the inside, and is plastic not soft cotton string. It has a high rate of recurrence (50% or something like that) and depending on depth of fistula a high risk of incontinence as well. Kshar sutra, which is illegal is western countries, is 98% or more effective and has only a slight risk of temporary incontinence to liquid and gas....so not like the really bad risk involved with cutting Seton. The reason I say this is because the difference in risk and effectiveness is extremely different. But travel to India is of course much more financially and logistically challenging. I wish that the safe and effective medicine was legal in the west, but this is the situation....

  • @HackYourHealth

    @HackYourHealth

    4 жыл бұрын

    Hi Abbas, I'm glad you got some treatment. I'm not sure what kind of seton you have. There is a cutting seton, which I think gets changed or tightened (you would need to ask your doctor), and eventually would close (risk of incontinence is high with this method, depending on the depth of the fistula). With a regular draining seton, the fistula is kept open to prevent infection and abscessing, but the fistula is kept open so it won't heal and close with this method. I think you would need to ask what the plan is from your doctor. This is just my personal understanding, not medical advice. If you remove the seton, nothing will happen but there fistula will not be kept open so there is more chance of abscess. Sometimes they put in a seton for a while to prepare for another surgury. You would need to discuss with your doctor, since there is no universal strategy. Good luck!

  • @HackYourHealth

    @HackYourHealth

    4 жыл бұрын

    Hi Abbas, in regards to the seton, I don't know the plan for your case, of course Fistulas are usually caused by an infected gland which causes an abscess that later bursts (according to my understanding) Or they can be caused by crohn's disease, abscess draining procedures, childbirth and some other things.

    @@christopherprichard4952 that makes sense The wound packing is extremely important! To prevent future relapse and proper healing. It is much better if a nurse does it, in my opinion, since then it's not your worry. But I met people whose spouse did it, or who did it themselves. In my case I was getting a totally different procedure, which is kshar sutra and partial fistulotomy. Kshar sutra is intentionally slow since it uses a medicated string to kill infection inside and slowly move thru the muscle to prevent muscle damage. It is the safest and most effective treatment for this reason but it also takes longer depending on depth. In my case it took 3 months but it was only about 2 weeks heaing from the partial fistulotomy and a very easy recovery. I was out and about but tired the first few days. I hope that you have a fast and easy recovery.
