Yes all self administered. Watched a really interesting documentary on this in poor countries. They can’t afford Synthol so the inject cheaper non sterile MCT oil or at worst mineral oil. The documentary was highlighting that in these poorer countries that these guys have nothing to loose and can make a little money and gain notoriety injecting to these freaky proportions. They get their 15 minutes of fame at a cost.

Either it's self injected, or it's done by "street" doctors. No actual doctor (a competent one) would do this. Sure, some street doctors do have licenses, but it won't stop them completely, they just won't be able to do legal procedures on top of the illegal ones. its not surgery its a synthetic oil known as synthol i believe typically used by bodybuilders in small amounts before competition to enhance there looks. But these people are basically insane they have filled there bodies with many pounds of oil and it oftentimes leads to infection or muscle necrosis or other complications
