Bhai polywool is used as a water polisher it needs to be thrown out and replaced use black course sponge as first layer and then medium coarse and then poly filter for best results

Both are mechanical filters which provide some amount of bio filter action choose as per tank size

It occurs naturally but will take time, probably a month, the time can be greatly reduced by adding pre used filter media or bacteria culture.

yup... but never use the normal sponges as they are made of cellulose and degrade.... use some kind of aquarium sponge

First y r u running on RO. Ro is required only for certain types of fish and certain setups if not u don’t need Ro...

There is something leeching into the tank please check u decorations and substrates if it’s not killing the fish then no issues if it is then as a emergency use activated carbon
