Should you have fake gums on your full set of dental implants or all on 4?
There are 5 different ways to rebuild a smile using dental implants, and a lot of people get hung up on if they should or shouldn't have 'fake gums'.

    He is great. I too have learned so much from his videos. I’m not in a good place with my dentures. I can’t hardly wear them because they have been made so horribly wrong. My mom recently passed away and my husband and have used everything we’ve saved for my teeth to pay for her expenses. Now my sisters are causing problems so more money for a lawyer. My mom didn’t have anything, but that’s not important. She would be heartbroken with everything that is going on. Especially with what we’ve had to pay for. ❤

    This is an excellent explanation, however coming from an almost toothless upper patient who’s researching a lot about which option to take, I can say almost all of us would take the gum free option every time, as it’s the only procedure that looks and acts like your real teeth. Unfortunately most of us have significant bone loss, to me an all on 6 looks fine until the gums are retracted and then it just looks like a denture attached to your gums, and I’m being asked for £20,000 for this. I don’t understand why you cannot have the fake gum seamlessly butting up against your own gum line, so no bone removal required, and original teeth height etc restored - I’d have that in a heartbeat.

    Wonderful information.keep up the geat work. Iknow people who had bad natural teeth causing pain, and they were replaced with fixed implants ,now the look wonderful and the individuals confidence has increased a hundred percent.

    I totally understand everything your talking about. I have the old ugly dentures and I can’t even drink a glass of water with them in. The lab did a horrible job and I’ve had them relined, I’ve even had to do some sanding in order to simply talk. 😢 Thank you for another amazing video. ❤

    An excellent Video explained so clearly. I have FP5 and agree with everything that Dr Gurs Sehmi. My 24 year old son who has hyperdontia is today, having a Sinus lift and implants at Eastmans hospital. It was interesting to listen to the video's to understand exactly what he will be going through.

    I wish to preceed my question by thanking you for your honesty and your amazingly informative channel, in providing information on subjects incredibly difficult to find information on. I would like to know, how is it possible to adjust a temporary bridge that feels slightly twisted and the teeth seem to protrude forwards, when they are fixed and screwed into four implants, in my mind leaving no flexibility for adjustment. You may feel that this is not important because it is temporary, but due to sinus lifts, I will be using a temp for a possible 14 months. I would be most grateful for an explanation of how this is done, not only for peace of mind, but I am naturally curious too and think your channel is amazingly informative.     The reason for adding the Bar is so that it distributes the bit more evenly and helps with putting pressure on just one implant and possibly having that implant fail in the future because it could start wearing the bone down at the implant do to the top law bone being a lot thinner then the lower. Dr…….?

    I just had my full upper implants done! I really liked the temporary acrylic before my final was made. Is there a way my dentist could add gum to the zirconia arch? The temporary one made my face look filler!

    Thats great of course but for someone like me who never had a good smile i definitely want them to look good. Not for other people, im married and i dont care about it in a way that i want the looks from others. But i do want to look good for my wife, and definitely for me too. Never had a beautiful smile, so i do want one and to know what its like. I recently had implant surgery. Still have to wait 3-5 months to go back for the crowns. Really excited for that! Kinda wish i had that remote from the movie Click to fast-forward lol anyway, cheers.

    I hate how thick my prosthetic ! I cannot speak and it is huge ! It is on implants behind my natural teethe… I had a beautiful was only missing back teethe I now I have buck teethe and a ridiculous overbite huge teethe and I cannot speak well.. I sound like I have removable dentures and it is not worth 37000 dollars! These are horrible.. I wish I had this dentist in uk instead of the “specialist “ here in us ! No one warns you of the bulk and how it feels just talk about. The look…this thing has made the curve of my teethe on top HUGE and the teethe look nothing like my real teethe… I don’t know what to do because the implants are behind my teethe line and hold the bulky prosthetic so it can’t be thinner… buyer beware. I trust this doctor on youtube more than mine! .. I wish I had Dr Gurs

    I dont know if you know this or not but for FP2 situations we can create healthy looking pupillae with an additive that makes the desired area of the porcelain gum colored. When you look at it out of the mouth it seems like a whole bridge with a pink arch over it. No fake gums but partly pink teeth. Im struggling to describe it tbh.

    @@alexwill1784 What is the coverage for this in Sweden? All of my mom's family is from and lives in Sweden and I lived there for part of my childhood. I have citizenship. But I've lived in the US most of my life as my parents decided to live here. I'm really not healthy enough to make it back there even though I'd love to (I couldn't make the plane trip). In the US this the costs for full mouth implants are prohibitive. Seeing your comment made me curious if the Swedish government covers dental implants. I have very fond memories of Sweden and wish I could make it back.

    Very helpful video, thank you! However, I am now second-guessing my planned procedure. I am scheduled to have all-on-4 using G-cam graphene implants. I can’t find much information about them, but I’m realizing that there’s no space between each of the “teeth” and I’m concerned that makes them look more fake. Do the ones with the fake gum have that space? You know- just enough to swish the water around in your mouth? Also, if you have any insight on the g-cam graphene procedure, it would be so appreciated!

    No spaces on any of them, it's just grooved to look like it. The only way you could get spaces is with a full set of individual implants.

    I understand you provideimplants teeth using titanium arches+ acrylic [which absorbs water] and false teeth which wear down and the teeth break off, while zirconium arches are a one-piece arch, much stronger, lighter and very durable. Why do you use titanium when zirconium is better?

    What to do if you had plenty bone and too much was shaved off by the surgeon is there anyway to get bone and gums back or is it just lost forever.

    Rebuilding bone for 'full arch' cases is very difficult, so I would assume that it's gone for ever. However the biggest issue is normally the clinician doesn't take off enough bone. For people who have little to no bone, there are other types of implants which use bone in distant areas (like cheekbone)

    Hi Dr. Sehmi. Back in 2005 I had ( upper & lower) orthoganthic surgery. A year ago I went to my dentist and she said that I have significant bone loss. Am I still a candidate for all on 4? Or other implants?

    Yes, you should be able to have dental implants, but the cause of the bone loss needs to be addressed first. You may need bone grafts first, before implants.. it might be worth seeing an experienced implant surgeon

    Thanks doc I am a dental technician in Dallas Texas working for a surgeon, one thing about the fake gum bioh hope bar zirconia teeth is that the cost is very high making it possible only for the elite 🙃

    And that doesn't make any sense. It's the same as cubic zirconia and that stuff is cheap as hell. It's the time they put into it. But mostly it's the profits being made from both the dentist/surgeon and the fabricator. The mark up is probably 80% profit. This is why we need to write our Congress and get implants to be insured and have contracted rates, keeping these money hungry implant surgeons in check.

    Can you do mini implants for individual teeth? If extraction many year ago and no or little bone left what can be done?. Dentist says can't do anything.

    I had an accident and I guess my implants are fp3 w/ 6 posts. The gum area is HUGE, like as bulky and thick width, not height. The dds keeps telling me its to keep down wrinkles as I get older, but I feel like I have a baboon mouth. This was done less than a month ago. Is this typical? He said I too much bone in areas and it needed to be shaved down. I had bucky teeth(which I liked), now all my arch sticks out REALLY far from my bottom teeth, about 1/4 of an inch difference from top to bottome(bad enough I can push my tongue through the space) They just look weird. Not to mention the finish isn't smooth, there are rough spots and cut my lip. I really hate how they look, even with my bucked teeth, I always had a pretty smile, now I am embarrassed to smile. What do I say to my dds to help him understand my disappointment. I paid out of pocket and never thought I would have a ridge that can be seen when my mouth is closed of the top gum area of the upper arch. My top lip doesn't even cover all of the top teeth unless I bring my bottom lip up and over.

    Which can take the most load or force for chewing? In real teeth I know it is at 100% but goes way down with dentures, to what I've heard can be as little as 30%. Do you have any idea what the bite force is for all these different procedures? Also, speaking of bite, is that taken into account? I've heard horror stories of people having to have these procedures partially redone (snap on teeth replaced, but in some cases the implants too because of an improper bite), because they developed TMJ or TMD in their jaw from an improper bite. So, is there a way to make sure that the bite is correct when having any of these procedures done? Thank you for ny information you can offer.

    From the day I had dentures I couldn't bite an apple or even confidently hold a burger and bite into it. Now I have my final zirconia prosthetics on my all on 6 I can easily bite apples, burgers and chew everything I could when I had healthy teeth.

    @Joshie Market I'm 59 and my surgeon told me that provided I follow the correct hygiene routines and report any issues quickly and have a checkup every 6 months the implants themselves should last for life. However, the prosthetics can break and may need replacing after 10 or 15 years.

    @@peterd788 From what I've been researching periodontal disease does not disappear when you have implants. It is a hereditary condition. I've had dentist tell me that the periodontal disease will go away with implants. Now I know they were lying to me.

    @Joshie Market Hey Joshie, I'm going to guess your dentist doesn't place implants or he'd know better. My brand has a lifetime guarantee though traditionally we've referred to implants having a 30 year longevity. Please understand, they don't fall out at 30 years. ;)

    I will be setting up a appointment to get the process going of getting all on 4 implants top and bottom. Has anyone got the top and bottom implants done at the same time? My only fears is being put under and kinda the pain level. I dealt with TMJ pain and honestly thats worse to me than a regular toothache.

    We often do upper and lower at the same time. I would recommend having sedation, it's a lot to go through

    You are not 'put under' at all. If you wish you can request sedation either injected or via mouth which will help relax you. Your mouth will still be numbed during the procedure. Pain is minimal if any but your mouth may ache a bit for a few days due to some swelling but ibuprofen or paracetamol will help with that.

    Yes. That's the protocol and is one of the reasons you need space for the MUA / tilted implants.

    Water pick and super floss under the bridge. Dentist cleaning 2* a year is preferable but at least once a year.

    Sir, my natural teeth are healthy, I have a couple very small gaps, very very light staining from when I was younger and didn’t really care for brushing... how ever as I got older I looked after my teeth religiously. But even after being professionally cleaned I can’t get the stain removed, and I do not have the patience to wait years for braces to work. Given these circumstance and that there is nothing wrong with my natural teeth, will a dentist refuse to remove ALL of my teeth and do a all on four implant? I will be a cash buyer if that helps. I am aware natural teeth are a lot better than implants, but I have severe OCD and require perfection, my natural teeth really knock my confidence. I am after a perfect aligned set and a perfect white tone. Thank you

    Hey I noticed no one have answered ur question so I'll try my best Patrick. The answer is no, whatever work u want done? That dentist/surgeon is gon do it for u 100% my friend! They'll never ever say no to u! They'll knock ur healthy teeth out then do the all on 4 for u to give u the perfect white smile that u want for urself Pat! Especially when ur paying cash & up front, make sure to ask them to give u a discount b/c there's a good amount of ppl that get this done? They'll be doin monthly payments b/c it's literally is/like buying a new car! It's sad to say but I think every implant place? They usually care bout the money part which is the #1 most important part imho even tho this doctor is super cool & seems super genuine n cool as hell! Don't worry Patrick, if u can afford it? No place will turn u down I promise u man!

    You're crazy. There is nothing like your own teeth and never will be. First off, it will cost 50K or more just to have this procedure done, not to mention all the pain and suffering. Do you like to be able to bite into a big juicy steak? You can forget that in the future, along with a lot of other foods you might like to eat now with fake teeth. What people fail to realize is that teeth have something none of these options have and never will have - nerves. There are nerves in the roots of your teeth that help with the sensation of eating. Biting into an apple or crunchy potato chips is not just enjoyable for the taste, but also the sensation. That is something you can never, ever get with false teeth, no matter what procedure you have done, and anyone who tells you otherwise is lying to you. Maybe another option to look into is veneers. There are several different types, but the least invasive (meaning the less natural tooth surface that has to be removed), would probably be best for you. My sister had all her teeth veneered and there is no difference in bite or sensation of her natural teeth because the nerves in the roots of the teeth are still intact. I believe the surface of her teeth are porcelain and they look beautiful. It will also cost a fraction of the price of any of the above procedures mention. A word of advice - be wary of any dentist that does not give you all the options, whether it be veneers or caps, (the next most invasive as it means the almost complete destruction of natural tooth short of pulling them out), and tries to steer you into any one of the above procedures right away. I would leave that dentists office immediately. Whatever you do, good luck!

    @@Bossmanrocks don’t eat red meat, I live off a fish diet and rarely chicken if I do have meat. Don’t eat anything crunchy as it is, won’t cost 50k at all I’m talking £ not $ Il be looking at 30-35k End of the day it’s my choice not yours, I do with what I please with my mouth

Bottom line, there is nothing like your own teeth and if you only care about the aesthetics of how they look then getting all your's pulled out is a drastic step and one that you'll likely regret, especially when there are better options out there, that costs 1/10th of the price, like veneers. There is no suffering involved either, which is not the case with having all your teeth pulled out of your head. But it is your choice. If you have 50-60K to blow and don't care about the lifelong issues and likely regret because of that decision, that's your choice. A stupid choice but as free willed individuals you have the right to make that. Just remember, everyone is out for making a buck today, no matter if it destroys another human being or not. Money has become god to some.

    After going through a major surgery I come to the conclusion that it is better to just go to a denture place. get 2 implants a pay for a fix denture. Doctor's only wanted make money and they paint implants like a natural teeth wish is not the case at all. They never mention anything about the big piece of plastic you can feel around your prostatic wish is not comfortably at all. There's a lot's of things they don't talk to you about.

    @@judyyoung2634 Exactly right. You still choose the material based on what your clinician uses and is familiar with. If you choose acrylic due to cost you get that denture feeling.

    I hate fake gums. Anyone can tell you have denture just by looking at your gum.. I would rather go to a denture place you get 2 implants and pay for a fix denture removable. To me Is the same and is only 7thousand dollars.

    They say angle implants are the worst things you can put in somebody’s mouth because it goes against the laws that say physics but the mouth there’s not like angled prosthetics like implants like your roots are not angled on them so why are you implants on an angle they’re going to fail they say no matter what it is made out of so this person does not recommend the all on 4 hybrids, The bite load will be on the front of the teeth not on the mould is because there’s no implants on the molars

    Some angle especially on the rears is ok and helps distribute the force properly BUT severe angle on frontal implants is a recipe for trouble... my gf had an all on 4 hybrid denture.. the dentist had one front implant coming out of her Upper gum area ,above the tooth! absolutely nuts... that implant went bad and the screw securing it broke, snaped in half soon after it was installed... the whole upper had to be removed becuz infections were starting due to the dentist leaving No gap for irrigation or flossing... just an aweful experience and the end result was failure of 1 implant and the all on 4 being pulled off ,never to be put back...why? becuz the fit was aweful from day 1 and that dentist said he was Done.. nothing more done... some dentist should Not even be attempting implants becuz they arent trained properly.

    in other words it's much easier and more beneficial for dentists to produce fake gum implants than think of what the patient really wants and deliver, and that is more natural looking. Forget the patient because it's too difficult, costly, haven't got the tech so let's just give them sub standard cumbersome fake looking implants and charge them a tonne of money, then laugh behind their backs.
