Expert Chat: What to Know About Sudden Hearing Loss

When someone suddenly loses his or her hearing, it’s hard to know what to do or when to get help. Ear specialist Dr. Steven Rauch offers insights about the different types of sudden hearing loss and when to seek treatment.
Please note: Any patient with a hum test that shifts AWAY from the blocked ear should seek urgent evaluation by an ENT doctor.

That test at the end is gold! The 2 months of hearing loss in my left ear, seems to be 'conductive' as the humming test sounded in my left (deaf) ear! So that's excellent information!!

@AMWF Hello! I first had facial numbness for a month and then bells palsy for about 5 days. Then I lost my hearing. I got lucky because the ENT prescribed a large dose of the steroids and that ended up working for me and I avoided the injections! My hearing has come back fully! I had an MRI and I have inflamation in the pons of the brain stem on the left side which is where all of these symptoms have been occurring. I went to the neurologist today after a referral from the ENT and now I have a spinal tap scheduled for tomorrow. They don't know what is going on with me yet but think it is viral or autoimmune. I no longer have any numbness, bells palsy or hearing loss but am ready for all of this to be over.

I also opted to get three (3) separate shots in my ear. After two (20 weeks I regained about 50% of my hearing (prayers answered). I'm not entirely sure if I have, while having tinnitus I always notice these strange moments where the sound of my computer engine or something suddenly stops and that temporarily triggers my ear or ears to full a tad bit full or like they're close to popping with a slightly louder than usual ringing... Afraid that it could be SHL I have to talk to the doctors to have the ENT appointment which was expected for 2-3 weeks to be made urgent and happen as soon as possible.

I'm 66. I lost hearing in my left ear after years of Meniere's symptoms including mystagmus. Basically, an over-pressure in the inner ear causes vertigo-like feelings of imbalance lasting minutes to hours. The loss occurred during an episode lasting 2 hours at 3:00am. Slowly, I am recovering, currently plateaued at 30% hearing. But, tinnitus has doubled in both ears. I've gone from thinking that I'm a cochlear implant candidate to a hearing aid candidate.

Hearing Problems I have had trouble with hearing most of my life, mainly Neural-Deafness and Acoustic shock disorder so in speech, what I thought I may have heard is not always what was really said or complete. Like when someone says “The cow jumped over the moon” what it can sound like to me is, “da ow umpt ower da noon”, however I look at lips while listening and my brain is highly trained to extrapolate and interpret, so I still get on in life, although it is stressful at times. Consonants like a P, a G, a D, a J, a, K, a Q a, T, a, B, a V & F, I cannot tell apart. And I cannot hear the S sound, unless it is very loud. This hearing deficit over works my brain and after a while I get what Audiology calls “Acoustical Fatigue” and so I usually snooze for bit after the struggle. Acoustic shock disorder When there is an outburst of loud sound, it shocks my brain to pain, so the loud automobiles with no or poor mufflers, are terrible when they drive by my house. It is no wonder I do not like to target shoot, even with ear plugs on. Most people that know me cannot fathom this hearing situation, like when they are talking to me a big loud truck goes by or a car without a muffler, they just keep on talking all-through the sound moment and expect me to understand. Any background sound like in a meeting, church, school, etc I am essentially 80% deaf during the background sound play. “You talk well and I hear you well, so I assume that you can hear as good as I do” This in not correct! I have recently been doing some experimenting with my hearing loss and it looks promising, so any continued luck, I will be glad to pass it on to others. Robert Shrewsbury

I got sick around Oct29th and lasted until around Nov.7th maybe? And somewhere between the 7th and 14th I woke up and noticed my left ear had this pressure ringing fullness sound and sensation… I’ve experienced this similar sensation before in my right ear but it went away, this has lasted a lot longer than I remember… I’ve been seen multiple times at this clinic since Nov.28th… they’ve cleaned my ears, evaluated and treated me for possible infection, although I haven’t experienced any severe symptoms of infection like vertigo, discharge, swelling, sharp pain in or around my ear, fever, blah blah blah… the medication they’ve put me on (antibiotics, nasal spray, antihistamines etc.) is for middle ear problems (FLUID) which is what they all keep coming back to saying, that I’m just full of fluid… but my CT scan they did on me came back normal, including my middle ear and Eustachian tube etc. all came back normal and that I have no fluid? So I’m just confused why 3 physicians would be telling me I have fluid in my ear if the CT scan says I don’t? I have an appointment with an ENT Jan.26th and I’m on a cancellation list but trying to get in sooner has been difficult… I can hear out of my ear? Sound may be slightly different but nothing too drastic, only when the ringing and pressure irritation sensation comes back is when sound is slightly different… when I put headphones on it sounds like one sound, I’m not hearing one better than the other? I don’t vertigo or anything like that, I can talk on the phone fine, hear my phone ring fine, hear other people fine etc… This past week I’ve noticed my symptoms ease up at night and then amp up during the day, makes no sense… the only thing I can truly say that is driving me crazy is this pressure ringing sound, low pitched ringing sound, like when your ear is full of water and you can hear the pressure from how full it is? That sound… everything else has not been that extreme, the pressure never gets too intense like when you fly, and the fullness sensation never gets to a point where I can’t hear out of my ear? I noticed early on, when I was on medication the symptoms would ease up a lot, for days, but then I would notice my ear being sensitive to certain sounds like bass or vibrations, like my own voice or somebody with a deeper voice I would notice my ear felt like it would vibrate or flutter? But I haven’t really noticed that lately, it’s been just this ringing irritation pressure noise that’s been consistent, never worse, and it’s kind of been on a routine? Idk… can anybody tell me if they think I have an inner ear problem or SHL??

Again not too severe on paper, but nonetheless those frequencies drops will certainly give you shitty fullness pressure sensations and the low pitched humming ringing. Maybe there was fluid in there at one point, but some time between Dec.5th to Jan.13th it went away and when I went and done my hearing test at this audiologist office Jan.13th the doctor told me there is no fluid, type A normal tympanogram which means nothing conductive and bone conduction hearing showed sensorineural hearing loss so…. Yeah pretty pissed off these physicians don’t even know the slightest thing about “sudden hearing loss” or just saying “hey you could go right down the road and do a free hearing test because there’s nothing else we can really do for you” or something as simple as the humming test or some shit… literally self diagnosed myself and have found out more about SSNHL then any of those physicians will ever know I can promise you that… and I got ahold of an ENT doc online who helped me more than you could ever know smh he’s the whole reason I even found out about sudden hearing loss, or suggesting a hearing test, he even predicted what my hearing loss most likely was before I went and done the test smh done has been spot on. But anyways. So yeah, was being misdiagnosed for an entire month and being treated for something I may or may not have ever had, I have no idea. I believe what I ever I got sick with attacked my inner ear for sure. There’s like a 2 week window they typically want to treat it with high dose steroid treatment, anything after that your chances of saving your hearing decreases. And I’m like WORSE CASE scenario 2 months in, but I’m not exactly sure when the sensorineural hearing loss happened because my symptoms mimicked ear congestion fluid etc. so that’s what was confusing and why I never thought “oh yeah I have a hearing loss” because I could hear out of it? It was just different. But this ENT doc online feels very strongly that odds are in my favor for a full recovery because my audiometric hearing loss is not too severe and based on the symptoms and my age he believes I’ll recover so. I started high dose oral steroid treatment 2 days ago and I’ve already seen BIG TIME improvement, for a while my symptoms fluctuated a lot, flare during the day, ease up at night etc. but this has been steady consistent improvement, ringing is at a very minimum right now, pressure is very minimal, still present but nowhere near as bad as it was or has been. BUT. Everybody is different, I’m not even sure if I will recover 100%, my ent doc I saw the other told me she couldn’t really tell me because odds are kinda stacked against us because I’ve missed the window for treatment by a pretty good gap. She just told me there’s a chance but I chance I couldn’t. But there are plenty of ppl who have started immediately after onset of symptoms and didn’t recover anything. What I suggest for you, if you haven’t already, try the humming test at home and or go see an audiologist and get a thorough audiogram work up and no for sure if it’s conductive hearing loss or sensorineural.

@@wobekillin9991 it started with my common cold on 26th January, as a work as a professor at a college of nursing, i was asked to conduct and officiate republic day celebrations and when i woke up in the morning, i could barely speak, could barely swallow anything because of irritation in larynx, and almost complete deafness in the left ear. Even then i did my duties and went home to rest thinking rest will make it all go away. Took some antipyretics and antibiotics and three days later, i could experience that my overall health is getting better, except my hearing. Reading on google and watching on KZread about hearing loss made me scared more on third night. Went to see a very good and renounced ENT specialist on 30th January as they diagnosed me with Otitis Media. Said my nose and throat infection transferred to my left ear by Eustachian tube. prescribed about three to four medications and after a week, i was feeling better. Today, i believe my hearing is 95% restored, but the main issue it tinnitus, and it is genuinely persistent and prominent. I can hear the ringing more when i’m in a quiet area. I loved being in a quiet places and now i’m scared to be in a quiet area. I read somewhere Tinnitus itself is not a disease rather than it is a symptom that something is wrong in the auditory system, which includes the ear, the auditory nerve that connects the inner ear to the brain, and the parts of the brain that process sound. I also read about Tinnitus that the greatest majority of new tinnitus cases will resolve within 6-12 months of onset. I am hoping i’m one of them as i am a MAJOR AUDIOPHILE, spend quite a huge account of money on good quality earphones, headphones and home theatres. I can’t imagine living with this constant ringing in my left year for the rest of my life. I’ll try to do everything that i can, even if it costs me to proceed to a surgery. Let’s just hope it doesn’t come to that. Thank you JC, for talking to me. I do feel better about it talking to you here. Very interesting Test! I feel like I can hear equally in each year. This is DESPITE having had an audiogram that said I lost hearing in my R ear to 55-65 db across the range of frequencies. I have thought since the "onset event" in which I ALSO got BLARING tinnitus with the hearing loss -- that some of the loss of hearing is due to the "fullness" and tinnitus masking. I have another audiogram in 5 weeks to test my hearing. in the mean time I've had 4 Intratempanic steroid shots in my R ear. I want to some hearing has improved but again, it could be because of dips in the "fullness" feeling in my ear and tinnitus also not as pronounced. I had extreme neck pain for about a week prior to my hearing loss event. I have no had an MRI of my brain which revealed no cochlea damage, no noted nerve damage, and no other abnormalities. So I am hopeful that I can find medical practioners who will focus on the neck and be able to solve that issue. I should note that in July 2019 I also had sudden onset hearing loss + tinnitus. Which I had thru Dec 2021 until I had a foraminotomy and disectomy to remove a herniated disc -- and I woke up and all my hearing was returned as well as the tinnitus gone. I have a cervical MRI tomorrow to check the spine. I have some tingling my 3rd-5th digits, I'm dropping a lot of things, and I also have shooting pains down my left leg (Lumbar cause I'd think) as other symptoms. Hopeful for answers soon. But this test would also support the MRI and other ENT related tests that seem like no real issue with my cochlea or related nerves. As the hearing for this test seems equally bilateral.

i am going to the doctor tomorrow, but here is where it came about. it is my right ear. ever since i was about 15 or so i have often had a lot of wax build up every few months it seems. in fact my grandmother had the exact same thing maybe it is herditery, not srue. anyhow, normally i get it removed ever3-6 months, however, i am pretty sure it has been almost , or maye over a eyar since last time they were cleaned. my right ear has always been the one to have more build up then the left. and recently it has gotten gradually more and more "full" and over the past few weeks i slowly noticed that ear is slowly becoming harder to hear in the th eleft albiet i think ther eis wax in both. last night i was tryign to flush them both out with warm water and a ear cleaning bulb fo rabout 10-15 minutes. i had jsut only a very minor tiny tiny tiny bits barely come out. albiet at one point i inadvertantly think i might have gotten the bulb tip too far in my ear, not sure if i hit my ear drum or not, although it didn't hurt at all. but then i was going to try doing H peroxide, but it put some in and i didn't feel any bubbles, i think i opeend it almost 2 years ago maybe it is no good, so i immidialy rinsed the ear otu. but i spilled some on me. so i took a shower and tried in each ear for a fwe minutes to let water drain my ear of wax, with warm water. not sure it did much. although i think for several seconds here and there i did have the shower strem from the head of the water sometiems it went direcly into the canal, not sure if that was too much pressure? but again no pain was casued. and the ear felt fine other than the alredy muffled sound but this morning when i woike up and since the rest of the day the hearing in that ear is siginficantly less than the left ear , although i am not deaf in it. i am not sure if maybe i have tons of wax and in addition maybe got some water trapped in with it, or maybe wax plus high level irritation. any thoughts? i hope i didn't do anything to injure it

This hit me, left ear, July 25th 2022.. 7 months now.. I will say, when I 'hum', I can hear something (humming) in my left (bad) ear and good (right) ear to, of course.. I truly feel there is fluid behind my eardrum. When 1st diagnosed, by 3 different (MD) Drs, all said fluid behind the eardrum. Then, after loopholes with my (then) insurance I finally get to an ENT Dr about 6 weeks later, & after a hearing test, etc, they say this shows no fluid behind the eardrum.. *I just don't think a hearing test is 100% accurate*. I ask both the physicians asst and audiologist this same question at different times.. What if, I did have fluid and by the time I got to them for them to check most or all was gone BUT! like any fluid, it dried on the backside of my eardrum & area, and possibly the little 'bone' structure, not allowing them to perform like they should.? BOTH said, that's really an awesome question, never crossed their mind(s) and neither had an answer.. IT was left at that. Having issues with sinus in my ears all my life, I honestly feel like there 'is' fluid or some issue behind my eardrum.. Maybe!?? OR could be both that and SHL? Thank you!! Any thoughts, PLEASE share.. Blessings Randy A Carlisle (FB) & DJ at KSGV Seagoville Tx

@AMWF I really don't know. TBH, it is hard to notice, i felt confusing, but i didnt know what was disturbing me. later I noticed it while testing my headphone. The doc told me about things that causes this, like infection, loud noise, sometimes ear clog etc etc, but they cant tell exactly what caused my hearing loss. I saw many videos and read articles about hearing loss, but IDK why i found the most important one last :'(

@AMWF whoa dude!!!!? I am not going to reveal all my personal info here. the world is a scary place. if u r experiencing some problem like this, I would suggest go visit a Ent doc ASAP! even tell them about this video, about "sudden hearing loss". best of luck bro.
