    The Supervisor needs to firstly be charged with their actions in ignoring this operator’s radio calls to stop, completely dangerous and negligent behaviour of a so called “supervisor”, absolute disgrace. Followed by having their employment terminated. Secondly, all these supposed pranksters should be classed as terrorists and also face the same criminal actions as this supervisor.

    The collision proves management at the MTA had spent very little time and effort improving and reinvesting back into the system safety systems and for a long time, had gotten away with it until this collision called attention to this deficiency in management. Same thing is going on at Boeing. The management there care about the quantity of planes and the workers at Boeing are pushed to get as many planes out the door. The quality of the planes at Boeing and the quality of safety procedures at the MTA, in a word, suck because sales orders come first, public safety comes second.

    That is completely false. The MTA has spent the past 20 years upgrading and replacing the signal systems. It just it takes a lot of time, effort and money; and on top of that, modern trains. Currently, the E,F,L,R and 7 all use a different system known as “Communications Based Train Control”, or “CBTC”, which, additionally, allows the L and 7 to operate automatically. This system will soon come online on the A and C as well. The installation process on this system takes time. Additionally, as a side note, this type of incident has only happened once before, and that was 54 years ago.

    A safety system is not only limited to systems made of machines. A human safety system is two or more workers on a train. The president of the US cannot detonate a hydrogen bomb without three Generals turning the key is a safety check. You forgot the human element just as the MTA forgot the human element. This is why there is no humanity for Americans in their workplace. The human factor is excluded in all its planning is a generational crisis. My comment stands as is.@@zycklacon9588

    We all know there are dead spots in the system for their crappy radios. That is NOT a fool proof method safely. Service versus safety is exactly the issue

    Are security cameras installed inside the subway cars? I know in recent years on the LIRR, cameras were installed. Catching people on security cameras in the act could be a preventative. In a situation like this, where the “prank” leads to a collision, charges need to be added for every person injured. It’s not just a simple prank that results in the train stopping momentarily and then starting up again. That “prank” that day is what started the chain of events that lead to this. A slap on the wrist here isn’t good enough.

    The trains involved date to the middle 1980's. Security cameras in subway trains weren't a thing back then. There is an effort underway to install cameras in the newer trains, but I don't know if these R62/R62A trains are included since they're nearing retirement age (subway cars here are expected to last around 40 years)

    God Bless.Tower should have 1st allowed the service train proceed then clearing the vandalized train after. Radio signals can be stepped on where the message is not getting through or even specific dead spots. Safety supersedes all rules regulations and guidelines. We come to work to go home.     It is the rail control center now when comes to numbered lines. They obviously still signaled the in service train to go first (normal protocol anyway). Otherwise would not have had switch set to change tracks.

    Bro, the subway and it's infrastructure was mostlt built over 100 years ago. Signals and many components need to be replaced. I'm an LIRR engineer and the amount of decay I see in our infrastructure is wild to me.

    @@blakemcnamara9105 yes, but the signal should've already been red in the first place. Your signals are always red for a crossover move if another train is on the opposite track. So it's a mixture of both in my professional opinion. Things work a bit differently for the LIRR.

    @@TheLIRRFrenchie... The signal for the train on the Local track was at Danger and the signal for the train which was to crossover was Clear. It was the brakes on the parked train being cut and it being operated from the middle which caused the accident.

    When I worked on the London Underground as a train operator, we had a procedure for a failed train that had to operated other than the front cab. You had an assisting driver keeping watch on the track as train was driven from the rear and came to a complete understanding that IF at any time communication was paused for more than 2 seconds the train was immediately stopped as you were the eyes for the rear operator. You had to keep talking, even if it was about crap! The observing operator was also given a way of stopping the train as well. This brake was tested before the move was commenced. If this failed then that train doesn't go anywhere until another way was found i:e another train to push it out. Think definitely service over safety. Just glad no one lost their lives!

    Running a train with all the brakes isolated on the leading units is sheer madness. This should have been driven in the other direction from the other end, or towed from the front by another vehicle.

    I don't blame the operator if he ended up quitting. I can't imagine how traumatic that must've been. Especially if you're one if the unlucky few who have witnessed people falling into the tracks.

    The system was designed to be failsafe although when you are this specifically incompetent I suppose anything is possible. Whoever gave the order to move a train from middle with the front half having dead breaks that couldn't even be tripped by a signal arm, is the real person at fault here.

    They’re multiple units, not cars with engines at either end. Makes for a more flexible solution, as they can be split up if shorter trains are needed.

    i would say to take out the emergency brake latch on the subway cars and replace them with intercoms that passengers can use to communicate with the conductor or motorman in case of emergency because it seems now that the emergency brake latch on the subway cars are more of a safety hazard especially in the wrong hands.

    car accidents a re a dime a dozen, there are plenty of safety and defensive driving courses. Moderns cars have better safetfy features, you can only cover so much about humans in cars making bad decisions. On the rare case of somethig going wrong they are usually all voer it.

    From what was going on, the front half of the train had its brakes disabled by engineers so that it could be removed. In effect it was 5 "dead" cars being moved by the 5 "live" cars at the rear of the train. Definitely an unsafe move.

    The brakes were cut out in the first two cars, the supervisor was operating from the 3rd car. The motorman would act as lookout and call out signals.

While built as independent single-unit cars, these trains were converted to five-car sets years ago, (probably to eliminate maintenance on redundant equipment) with drawbars replacing couplers between the cars of each set and motorman's controls removed except for the first and last cars.

    🤔 something about his story seems off. If his radio wasn’t working why didn’t he use the intercom these trains do have or the buzzer. The buzzer is definitely a valid form of communication. I’m also guessing the e brakes weren’t working bc of the reason why it was taken out of service but something more seems off potentially. Also it being a low speed incident and him being a spectator watching himself be killed seems like he could’ve gone to the back of the car at least or to another if he had more time (toward the back). Also to blame the supervisor saying the supervisor almost took his life heavily implies a malicious intent.

    I would never want to be the lookout at the front of the train that has no brakes, and no way to guarantee the supervisor in the middle of the train receives my every command. The operator could have died. I’m sorry- it is safety first. Clear the tracks, or have all the trains run express until the train could be slowly and safely moved to the yard. Also, they need to get rid of the Emergency break cords. We have inconsiderate passengers that love to see chaos erupt. Now, I understand how this could be considered a domino effect.

    This smells like a cover up. Let’s blame human error rather than say the system failed. The transit system is over 100 years and instead of upgrading over the years they wait until disaster happens to make changes. I hope the news did their research to see if he is even an employee

    He was in the front of the train which was operating with no brakes. The supervisor in the middle car was controlling brakes and didn’t respond to the radio signal.
