I’ve been trying to avoid bilateral knee replacements with 8 weeks of PT. My surgeon just gave me a high five because I’ve improved in strength so much I’ve been able to slow down the replacements for awhile. I guess that’s not bad for a generally sedentary 73 year old woman. I’m very pleased. Thanks Doc for all your help and hope!

I'm 55 years old and have been diagnosed with osteoarthritis. I know now exactly how it developed. I know it was from my job, we had a store renovation that tripled the size to walk and work in. There was a ton of heavy lifting as well. My thigh muscles built up very quickly and within a year, the pain started. Now that I know how to help fix this with these videos, I'll do my best to make it work. Thanks again, David!

Thank you so much for explaining in such clear detail about knee arthritis. I was doing everything you mentioned. Couple those things with wearing high heels, and, walking on a callous on the ball of my foot on unforgiving concrete, I was a painful mess. I was doing quad exercise that now I see was making my knees worse! I understand now. Since starting the glute exercise, the pressure and pain has deminished by upwards of plus 85%. I can't thank you enough Dr. David, you are the best!!!

You are a Godsend. I stopped working out my quads as was prescribed by my physical therapist, and after watching a few of your videos decided to focus on my glutes. My knees have been improving ever since. Snap, Crackle & Pop have been run out of town. And I can see the normal contouring of my knees as opposed to the puffy painful patellae. Also the tailgate swing and stationary bike with the knees a bit farther out have helped so much. I feel like I can say this since I'm probably old enough to be your grandmother: but oh my goodness your brown eyes are devastating! Thank you so much!

I mentioned another problem under one of your videos, but also problems occur if I've been sitting for a while. Standing can be embarrassing, as I might even look drunk as I try to gain balance. I overdid it years ago, when I got a chance to teach Zumba to kids, while in my mid-60s. I did not listen to friends who warned me to rest, until other circumstances forced me to stop.

Hello! I subcribed to your channel a month ago,. I've been running, and doing squats since 2013. I had a knee pain on my right knee. I did my own therapy. But, when I went to Disneywold for a week. I did a lot of walking on my first day and it started having knee pains again. But, I was still able to walk from 17 to 20K even in severe pain. Anyway, when I got home, the pain subside a little bit until the pain level went down to 1. Anyway, I still went to my doctor, I had an x-ray done and I was diagnosed with mild osteoarthritis. I went to my physical therapist (one-time only) coz I decided to do it myself. But, when I saw your video, the exercises you showed, I followed it even though I do not have knee pains anymore. I stopped running coz I do not want my knee to get worst. I only walk for 30 to 45 mins 3X a week & strength training. Thank you!

Hi Dr.David. I subbed your channel a while back and find myself watching more and more of your videos. I'm 65 and I have "worn myself out". LOL I commented a few weeks ago on a video about using the glutes more when walking or climbing and I have been incorporating this into my daily routine. I also mentioned hiking the Guadalupe Mountains as well as many other west Texas mountains which I love. Rather than hiking and backpacking, I have replaced my love of the outdoors with cycling. On-road and off. In October 2020 I cycled from the northwest corner of the Panhandle to the mouth of the Rio Grande/Gulf of Mexico. Without the impact of walking and carrying a heavy load, I seem to do so much better. What are your thoughts on cycling and bad knees, especially for people my age? Thanks and keep up the great work. All the best, Tony (North Central Texas)

when you get Lyme disease it gives you co-infections ( parasites ) that eat the cartilage and why a high percentage with late stage lyme have arthritis. When I began treating the parasites my arthritis pain subsided significantly ., The swelling I ( I inch hard bump )n my outer knee went away and I could walk w/ out pain. I also let go of Gluten and plant matter and went from Hardcore Clean vegan to a ruminant meat based lifestyle. Also Lymphatic manual / self dry brush massage helped my lower outer leg pain significantly, . It breaks up the lactic acid buildup .

Thank you so much for this explanation! I plan on watching more of your videos and have already modified my way of walking up and down stairs and working on modification of walking in general. After years of long distance running and hiking over difficult terrain, I began experiencing pain in my left knee and can no longer bend it all the way back. My knee doctor took x-rays and almost gleefully told me that I was bow-legged, born that way, and that I had bone-on-bone in both knees, that 100% of doctors looking at the x-rays would agree that I will need knee replacements in the future, and he would be happy to perform the surgery when I can no longer tolerate the pain. I wasn't ready so he gave me a steroid shot (temporary relief) and wanted to put me on medication. I cancelled the 30 day follow up. There was no mention of arthritis although I had previously been told I had arthritis AFTER they performed a kneescopy ten years earlier in the same knee which only got worse. I chose not to get the kneescopy in the right knee and it did not get worse. No MRI was suggested. I have since lost 30+ pounds. I can bike and row. I started doing leg presses which always take the pain away. I am now experiencing significantly less pain and noticeable improvement on the stiffness. I look forward to watching more videos and implementing your suggestions and hope to correct any muscle imbalances.

Thank you doc for the clear explanation. Now I understand Osteoarthritis so much better listening to u. I do have Osteoarthritis due to a dislocation when I was about 12. Somehow managed and now I am 62. Retired last year so I didn't let that problem make me retire early. Have been doing exercises like cycling and taking supplements. Right now I have a problem with my right knee on the left side. Xrays show all the things u mentioned. Walking on flat ground is no problem. Stairs is a nightmare. Knee is tight. I think it's the bone spur which is restricting movement. What supplement would you recommend. Taking inflammation meds when there is a need. Daily dosage is Osteoeeze . As of today included caltrate.

When I walk my inner knee locks with pain when I stretch my leg or bend my knee it clicks with some relief but doesn’t last . Wondering if could direct me to an exercise, I couldn’t find one ? Thanks so much for your expertise in helping my pain.

Whoa.... You're telling me that strong muscles in the thigh or a bad thing? C'mon, man, that doesn't make any sense! I thought that one of the main goals of physical therapy for the knee was to make the muscles stronger, and now you give me this comment which really sends me for a loop.!

  • @epmanualphysicaltherapy

That exactly right Artman710. Too much quad strength is bad. And you’re right, physical therapy clinics often make it a goal for patients. It is only appropriate to have that goal of there’s been a knee surgery or a traumatic knee injury and the quads have shut down. Other than that, the quads are usually over dominant and the glutes need to be strengthened instead.

Why don't drs know this imbalance thing? I'm actually kind of annoyed that they don't try this prior to surgery. I'm sure my knee pain-after hearing this-is caused by muscle imbalance because I used to rollerblade a lot. So outer quads get huge, but inner quads not so much! Or is it vice versa? This is cool. I will watch the next videos! Also, your video about alignment of bottom bone is actually working! I can walk almost a mile before my knee bags out!

I’m 78 and have always been very active (golf, hiking, etc.). I don’t have knee pain, but my knees have gotten progressively weaker over the past few years. I lost the ability to squat down several years ago, and now what I constantly worry about is my knees buckling. They just give way, and when they do it’s a real battle to keep from falling. No one can tell me why, other than I’m just old. Any ideas what could cause this? I have fairly severe lumbar spinal stenosis and degenerative disc disease - could that cause my knees to weaken? Any thoughts would be greatly appreciated. Thanks…

My pt said she could not reduce the swelling because I was using the wrong muscles to walk and that I was using the wrong muscles to walk because my knee was swollen! Now years later my knee is still swollen. Will strengthening my glutes help me? It primarily hurts in the area just above my kneecap and there is also a sharp pain which runs down my outer thigh from my hip to my knee. How do I treat that pain? I hope you can help.

I have studied all of this from a dietary standpoint as well and the idea that you can’t regrow that tissue is just wrong, if you get rid of the inflammatory chemicals in your diet and replace those foods with others that have the same nutrients without those sugars and chemicals and pesticides and natural inflammatory parts of the plant And feed it with the healthy collagen‘s in the building blocks of those tissues combined with sleep or even fasting and a lot of research even shows higher rates of oxygen and even sunshine and natural infrared light on the surface of the knee are all things that can set your body up to regenerate that tissue as best as possible and that along with healthy safe versions of exercise provides your body with some time and the ability to regenerate that tissue and I see no reason it can’t. I don’t think people should expect a miracle to wear if they just change their diet all of a sudden in six months their knees gonna be fine but I’ve been studying and eating that way for 14 months and actually have cured many other things in my body as well and basically now other than my back and knees tinnitus and my eyesight isn’t perfect there’s nothing else to fix come out my metabolic system is completely restored my immune system is doing amazing and I fix that by eating correctly and I’ve gotten rid of all of my medication‘s All signs of heart disease are gone and I have nearly perfect blood pressure at 54 years old, I’ve lost a ton of weight and I’m still losing fairly quickly and my body is set up to regenerate and heal those tissues optimally. Like you were saying studies have shown it can grow back. I’m very realistic about it but my knee doctor just flat out said if I want new knees they’ll replace them, it seems like they should at least tell you to try these other options first but then again most of our medical community is kind of like any other job, most of them sort of suck at it and you’re lucky if you find the good ones who actually care and try instead of just getting their degree and pushing prescriptions and sending people for surgeries without even trying to fix the problem in a better way

The other problem with these treatments with the stem cells is insurances won’t pay for them so you got a cough up about 600 bucks a shot to get them. For people who can afford it that’s awesome but a lot of seniors are on fixed incomes and with inflation and gas prices and everything else skyrocketing coming up with 600 bucks a shot on the primary treatment my doctor offered being six shots that’s a good bit of money. If it’s actually showing to work insurance company should pay for it they pay for drugs like statins which best case scenario barely offer any advantage and in most cases actually cause more damage and they’re pretty dangerous but every insurance company pays for that. There are a lot of expensive procedures insurance companies will pay for and if these things help and don’t cause harm then they should be giving people the option, they might save tens of thousands of dollars from double knee replacements and they should support diet programs and much more therapy. I got therapy for my knees for about three weeks and the insurance ran out and the doctor said adios. Thank God people like you put these videos out because I’ve had to learn to give myself the knowledge and figure out how to eat healthy and lose weight and how to do these exercises and what not to do from you guys not my doctors

Simple osteoarthritis is usually a sequel of some low intensity trauma with damage the one of the meniscus or hyaline cartilage or some ligament. Goes unnoticed, no MRI done. Simple Painkillers momentarily reduced the pain but daily activities keep of damaging the cartilage. Aggravated by increase muscle tension or trigger point in the said muscle further worsen the condition. Added by high or upper level S. Uric acid . Some have low intensity seronegative monoarticular R/A , ofter misdiagnosed as O/A.

Good thinking NAB. It's a relative imbalance that is connected to the level of activity that individual does. You can be a weaker person but have relatively strong quads and develop knee problems. You can also be a very strong person with relatively strong quads and develop knee problems. It's about the balance more than the overall strength when it comes to chronic knee problems.
