Endometrial Ablation: Is It a Good Option for Treating Uterine Fibroids?

Endometrial ablation is often suggested to women suffering from uterine fibroids. It is also often mistaken by patient for uterine fibroid embolization.

    @@mariabimagnoliaI had one about 10 years ago (Novasure). I haven’t had any complications. I still experience mild PMS (no cramps, no headaches). My endometrial ablation and breast reduction were two of the best things I’ve ever done, in my life. I had my breast reduction 14 years ago and I want to have another one before my Dr. retires:) (I was 31 yes old. As my weight has fluctuated and hormonal changes have continued, my bra size has increased 2 full sizes and I feel like I’m close to where I started 😒 It’s late, but I Hope this information is helpful.

    Thank you Doctor for explaining the difference of the two procedures in detail. So the (UAE) treatment will get rid of all the fibroids but will it prevent new fibroids from happening ? (once the procedure is done).

    The UFE procedure should knock out all of the fibroids that are in the uterus. For women over 40, this is typically enough to hold them until menopause. Some women will grow new fibroids that can become symptomatic before menopause and if so the UFE procedure can be repeated.

    Ablation is not recommended for adenomyosis. In fact, endometrial ablation will typically make the bleeding symptoms worse ! Uterine artery embolization (UAE) is a very effective, nonsurgical treatment option for adenomyosis. Learn more at ATLii.com.

    I had a D&C with ablation 1 week ago. Started bleeding pretty heavy 4 days ago. Going on 3 1/2 months of bleeding. I don’t think it’s ever going to stop at this point.

    No. You need to be checked for uterine cancer. You should not be bleeding at all.

    I have one 3 mm fibroid and a small polyp. They want to do a thermal balloon ablation on me since my periods are heavy, long and every two weeks. I'm worried that I might have an ablation failure and end up having to have a hysterectomy since I have this fibroid but since they said there is only one of them there, would it be a good option for me to go ahead and have the ablation that they recommended?

    Can't say without more information. A 3mm fibroid is tiny and incidental. From the limited amount of information, what they are proposing sounds reasonable.

    I had mine done today.had polyps and fibroids. Didn't feel a thing.but now my abdomen is so painful. Using heat pad and painkillers.

    @manormani1 I haven't got it yet. I'm scared dr I'm 40 no kids. Was considering it. I have both polyps and fibroid didn't even know this was a thing until I researched and got tested. I est healthy and work out but idk

    I came off the pill called lolo, and a month later I went from 135 hemoglobin to 75. I hemorrhaged for 11 days and went t ER. Had blood transfusion, just tried IUD and broke out in hives rash, became feverish etc just had it removed after 1 month. I have a 4 cm to mm fibroid and my gyne is suggesting ablation. My periods are so heavy I can’t leave the house for a week. I’m sob, fatigued etc. I’ve had an iron infusion which helped with my sob. Should I get ablation or get rid of the fibroid? Thanks Tracy

    While i can’t say without knowing more information, ablation should be reserved for heavy bleeding not due to structural causes in the uterus (exs. Fibroids, adenomyosis). Ablation doesnt do anything to the fibroid(s) which often also causes bulk symptoms too. Learn more at ATLii.com. Good luck.

    @@Atlii I don’t know why it didn’t work they didn’t say anything when they discharged me all I was told is i need the next available appointment with the consultant to discuss a hysterectomy, hopefully they can tell me why it didn’t work

    @@lauraslater9121 That’s extremely rare and not acceptable. The IR (not the Gynecologist) that attempted the procedure needs to explain to you what happened. Did they fail to catheterize one (or both) uterine arteries? Did they come from your wrist or groin? You’re entitled to know what happened. Insist !

    @@Atlii they went up internally all i know from the nurse who discharged me said they wasn’t able to do the endometrial ablation and now I got to have a hysterectomy there was no explanation and that was 3 weeks ago I don’t see the consultant for another 2 week

    Hello, I have very heavy periods and I would like to have an endometrial ablation (i don’t have any other problem). However I have a question: does the hormonal cycle continue normally after this operation until the menopause that is to say 50 years (I am 22 years old). because I read somewhere that it causes menopause 10 years after the ablation and stops the hormonal cycle.     You should do a IUD, it will help a lot with your heavy periods. You shouldn’t do this surgery, not at 22 y/o because it decreases your chances of becoming pregnant. You may not want kids now but your story could change 😊 I didn’t want kids…. And I met my husband at 33 and we had our first when I was 36.

    Ok...The UFE is what I'm considering! I was told it only shrinks the fibroids 70-90% and they may come back or new ones grow! With them only shrinking them 70-90%, wouldn't they just grow back because they aren't fully gone? In this vid, he's saying it get rid of the fibroids! Which does it actually do?

    The shrinking is not what is most important. What’s important is that all of the fibroids are dead. If so ( and 9 chances out of 10 that would be so) the symptoms will fade away and that’s why the procedure is done (i.e. to dramatically improve a woman’s quality of life). Now typically we get a lot of fibroid and uterine shrinkage also, but that’s really secondary. You’re rid of the symptoms and you be rid of some fibroids too. Also, those dead fibroids don’t grow back, they’re dead. If you’re over 40yrs of age, the UFE will likely be “a one and done” but even in “worst case scenario” where you need a 2nd embolization, that’s still way better than 1 surgery.     I had endometrial ablation for extremely heavy periods. I do not have fibroids. I found that the procedure did not make a significant difference. My doctor said we can try ablation again or a partial hysterectomy by just removing the uterus. Have you had patients that have undergone endometrial ablation more than once? If so, did they notice improvement? I understand every situation is unique to the individual. I'm just curious.

    You may have adenomyosis. Ablation should not be done for structural causes of bleeding (like fibroids or adenomyosis) It often makes the bleeding worse with adenomyosis. Dont repeat ablation. Ask to get a pelvic MRI to see if you have adenomyosis. Pelvic ultrasound doesnt have good enough resolution and often misses that diagnosis.     Yes. If you have fibroids and symptoms due to these fibroids you are likely a candidate for UFE. We see a lot of patients that have had ablation and previous surgery (myomectomy) too.

    Endometrial Ablation is performed by a Gynecologist, and cannot be performed on a patient who desires fertility. Endometrial Ablation is NOT (and should not be) recommended for patients who have fibroids, because the lining will not be fully treated due to fibroids getting in the way--endometrial ablation treats the lining of the uterus, but NOT the fibroids!. Please consult a Gyn regarding your specific question about whether the lining grows back after an endometrial ablation. It is very important to note that endometrial ablation and uterine fibroid embolization (UFE) are VERY DIFFERENT procedures and are done for very different purposes. Patients with fibroids typically have great results with UFE. Patients with fibroids who have endometrial ablation typically do not have good results, and end up seeking another treatment (like UFE) which is designed to treat fibroids.

    @@zapakmeans1075 Hi, Dr. Lipman does not perform endometrial ablation, and therefore would suggest you consult an OB/Gyn doctor who performs this procedure for your specific question. Dr. Lipman performs UFE to treat symptomatic uterine fibroids, and a patient's cycle does return after a UFE procedure. Hope this is helpful!

    UFE is an excellent treatment for submucosal fibroids. Endometrial ablation is not a good treatment for bleeding from submucosal fibroids as the underlying culprit (i.e. the fibroid) is not addressed by ablation.

    Can't say without more information. Ablation is a good tool used in the appropriate setting. For most women the periods after ablation are lighter and in a small percentage there will be no period afterwards.

    I have very heavy periods and a short cycle. I don't have fibroids or any other uterine pathology. But my iron level is low (because of the bleeding). I want to have an endometrial ablation but I heard that this is only for woman who have a thick lining. My lining is not thick it's normal can I have an endometrial? ablation?

    We do not perform endometrial ablation--we would suggest you contact an OBG for an examination/imaging to determine if this is their recommendation.

    @@betrave7090 Endometrial Ablation is performed by a Gynecologist, and is not the Specialty of our Center. They can determine if Ablation is appropriate for you based on their assessment. At Atlanta Fibroid Center, WE only treat women experiencing symptomatic uterine fibroids.

    Was your pelvic ultrasound absloutely normal. If so ablation is an option. On the other hand, if your uterus is enlarged and “heterogenous in echotexture” you likely have adenomyosis and ablation would make things worse.

    I wouldn't because women's circumstances change. Once a woman undergoes an ablation, she will never be able to have a child again.

    I'm in my 30s, married , 2 live births, 2 losses, ALL high risk pregnancies and it would be dangerous to get pregnant again. I bled every day for 2.5 YEARS and multiple OBs told me ablation was "too radical " SMH!! I had ultrasounds, labwork and biopsies to rule out cancer, infection etc. I'm even bleeding on the highest dose of hormonal BC which is making me incredibly sick. I didn't even have morning sickness with my pregnancies, but this high dose pill has me getting up at 3-5 AM to puke. Guess you have to be over 50 for them to care 😒

    No. We have seen numerous pregnancies after the UFE procedure. Our births are typically full term and vaginal whereas if you undergo a surgical myomectomy and get pregnant, you’ll not be allowed a vaginal birth. Instead, those women get more surgery (i.e. c-section).

    Endometrial ablation is not a good option for the treatment of fibroid-related bleeding. UFE is much better for that. If a woman has heavy bleeding unrelated to fibroids (or adenomyosis) and not interested in fertility, ablation is a good procedure. Typically ablation does not end all bleeding (it does for some), but it makes it a much lighter manageable flow.
