Ending Heart Disease: Cardiologist Kim Williams, M.D.

America's #1 killer, heart disease claims 1 out of every 3 lives in the U.S. But it's avoidable and often even reversible with simple diet and lifestyle changes. This week on the podcast I sit down for an important conversation with cardiologist and former president of the American College of Cardiology Kim Williams, M.D. to discuss the factors that contribute to heart disease and how best to avoid it. Enjoy! ✌🏼🌱 - Rich

Hi I'm Rich Roll. I'm a vegan ultra-endurance athlete and podcaster. I write books and travel the world giving talks. But mainly I'm a dad of four. My wife Julie is much better at stuff than me. as long as his prices are incredibly low... this is a problem of the PEOPLE, te people who refuse to take responsibility for their figh fat food addictions.. many of us poor people had to become our own doctors because we dont court affluence. Too many people are pretending like their incompetent intuitions or personal wants are more important that our communal need... weve legalized being able to damage our empathic brains, the ones we use to differentiate adults from child, able from not... drugs are the things that make people say "its not that simple" or "nutrition is complicated" same as with affluence.. its as simple as the fact that we refuse to stop consuming "its not that simple" is the gaslighting of the affluent while people who are disproportionately responsible for climate change, but the most affected by it suffer. 99.9 percent of society literally has to get over itself.. but we wont outlaw the animal products that are facilitating the apathy needed to sabotage the plan the sustainable plan is ruined by personal ambitions, and the gaslighting of the acquisition of those ambitions, being "deserved" by everyone. sociology is simple as fuck if you cultivate a multidisciplinary approach

@ 35 min - the bacon and eggs after stenting happened to me in 2012 in my local hospital. It's insane. I changed my diet right after leaving the hospital having already been familiar with Ornish and Esselstyn's work. Vegan for 5 years now. Blood work has been excellent throughout this entire time with numbers almost half of their original values. Took myself off of beta-blockers (my heart rate is < 60 and BP is excellent) and statins the first year.

I think every cardiologist might benefit from making their own 'keynote speech' documentation to present to their patients in the same way, and cardiologists share the common hurdles of different types of people and what is said to convince them the most effectively to switch. If there's science behind that presentation method, it can work better. It might mean that there needs to be a lot more time included with the patient, also, and with dietitians providing foods that will tempt them because they are so tasty. Taste is a big thing. They need to be full and satiated. If that happens, it can help greatly.

On the topic of coconut oil, the following is the breakdown of all of the saturated fats in coconut oil compared to butter. You mentioned that coconut contains a lot of lauric acid (C12:0) and that is true and that there are a lot of different saturated fats, and that is also true. And you said that lauric acid is more easily digested, and that might be true, I don't know. But from a cardiac health standpoint what is important is that some of the saturated fats raise LDL cholesterol more than others. The three that raises LDL the most are Palmitic (C16:0), Myristic (14:0) and Lauric (12:0) and it these three saturated fats constitute *80%* of the saturated fat and 69% of the total fat. These three particular saturated fats constitute 62% of the saturated fat in butter. So from a saturated fat and its impact on LDL, coconut oil could actually worse than butter. Now butter also has about 6% natural trans fats, which is even worse for cholesterol than saturated fat, and so this is likely why butter as a food raises cholesterol more than coconut. But it is pretty clear that coconut oil is not a health food. Per 100 grams Coconut Oil Butter grams % sat % tot grams % of sat % tot Total Fat 100 81.1 Fatty acid Name 86.0 100% 86% 51.4 100% 63% 4:0 Butyric acid 0.1 0% 3.2 6% 6:0 Caproic acid 0.8 1% 2.0 4% 8:0 Caprylic acid 8.2 10% 1.2 2% 10:0 Capric acid 5.6 7% 2.5 5% 12:0 Lauric acid 43.7 51% 68% 2.6 5% 14% 14:0 Myristic acid 16.8 19% 7.4 14% 15:0 Pentadecylic acid 0.0 0% 0.0 0% 16:0 Palmitic acid 8.4 10% 21.7 42% 17:0 Margaric acid 0.0 0% 0.6 1% 18:0 Stearic acid 2.5 3% 10.0 19% 20:0 Arachidic acid 0.0 0% 0.1 0% 22:0 Benenic acid 0.0 0% 32% 0.0 0% 49% Lauric + Myristic + Palmitic 68.9 80% 69% 31.7 62% 39% Fatty acids, total mono 5.9 6% 21.0 26% Fatty acids, total poly 1.7 2% 3.0 3% Fatty acids, total trans 0.0 3.3 3% Fatty acids, total trans-monoenoic 0.0 3.0 3%

I am hearing you address the issue of drugs to lower cholesterol. I just cant take any of them without nasty side affects like having it poison my hair and loose 3/4 of my hair in about 3 months. I was fighting Cholesterol and had to find what was causing the inflammation and it looks like what was left was some dairy that was sending out cholesterol to patch the holes in my arteries. I was known that I had an inflammation issue more than 5 years before I had to go to the CATH lab last Dec. I had very few symptoms that i was having anything serious going on but the LAD was 90 % blocked. My cholesterol had been Inching up over those 5 years and nothing was stopping it. I went on a whole food plant diet with almost no Meat, dairy or refined foods and I dropped the LDL from 223 to 185 in about 2-3 months. I think I can drop to 145 or less but I am so sensitive to meds and some foods and eating food away from home is a nightmare. I don’t use milk but I could without feeling bad until they homogenized it. I was still using a small amount of cream and ice cream but that is gone too since I noticed it was making me fat. I didn’t cook meat at home so only ate a hamburger or fish if I was in town and had to get something to eat to keep from crashing but even that was too much. I use some bread but it has to be good stiff bread or I will not buy it - no soft white bread in our house. Don’t drink or smoke either and we do not drink water from the tap either. i am listening to my body so as I clear out my arteries I dont work too hard and make my heart need more blood flow than it can get right now. I have to slow down if the little finger edge of my right hand and the finger tips get tingly and a bit numb - that is my warning sign that I must slow down and let my heart not have too work as hard. That is working well to tell this type A person that if I dont slow down I will have a heart attack. Hope what I have shared will help someone else. By the way I dont use any oils and very little sugar and do corn syrup solids either

No one is asking you to exercise forever. But can you do it for a short 28 days? Do it and you will see a change that is unexplainable. If you choose to go back to what you were doing, fine. A new "burger" coming out is the Impossible Burger. It is made with heme iron (so that it tastes like beef and is also the same type of iron in beef which we know to be unhealthy) and coconut oil which is 85% saturated fat. This burger is definitely not meat but is definitely not healthy. If one eats whole foods one doesn't have to worry about what is in the 'product' that one is eating. Just eat a whole food naturally grow and you don't have to worry.

I drink 16 ounce fruit smoothies everyday, 2 to 4 times a day. My A1C has gone down from 6.4 to 5.6. The sugar in the whole fruit does not increase blood sugar, b/c of the fiber. My fasting blood sugar has also gone from 110 to 75.

I feel so lucky now in retrospect that my LDL was 85 but my total was 210 so I went full red alert early and to whole food-plant based. I didn't realize until later the 85 and although it's not ideal, how low it was compared to a lot of people with problems in cholesterol. Of course, I easily fixed it being strict whole food-plant based. 5 years now I'm still fine.

He says complex carbohydrates are not gonna spike your insulin. When he talks about sugar he means sugar which isn't attatched to its fiber as it would be originally in the whole food. Sugar (including syrups, refined flours) is as bad if not worse for heart health than saturated fat (including coconut).

Sadly, wouldn't be surprised if straight sugar was worse than saturated fat, since the deleterious effects seems to show up earlier in life. Both should be avoid though, that's a fair compromise.

Artery Inflammation caused by sugar/carbs are healed by cholesterol, which means the body needs to naturally produce more cholesterol to cure the damage, if so, the problem is not the cholesterol but the sugar/carbs instead, which are causing the inflammation. Am I correct?

No you are not correct! Saturated fats (meats, fish, dairy, eggs) are inflammatory but so are sugar, processed foods, preservatives, colors, and artificial flavors.

The real problem is the concept of "opinion" in the face of "fact". "There are two kinds of cardiologists. Vegan's and those that haven't read the data." The data is clear. "Opinions" are the problem. The only concern with the doctor's observation is that the terms "Vegan" and "Whole Food / Plant Based" are treated as synonymous, and they are not.

What about Dr. MacDougal's research on sugar?

  • @peterz53

    @peterz53

overall, McDougall has a good message but I'm skeptical about his high starch approach versus someone like Joel Ferman who also emphasizes a plant based diet, but not so heavy on starch. And certainly doesn't downplay sugar. I think one key is to understand the impact on insulin, which is implicated any many diseases and is anti-aging as well. Would be valuable to collect insulin/Hba1c, and other data, on people who eat a McDougall type diet vs a Furman type diet. My guess is that high starch will have higher (more detrimental) levels of insulin even is better than most Americans.

  • @papparocket

*Peter* When you get down to what ends up on the plate, my impression from reading books and menus from both is that there really isn't that much different between McDougall and Furman. Due to the very low calorie density t is simply not possible to eat enough non-starchy vegetables to get enough daily calories or even a substantial portion of one's calories. The only alternative is to eat foods with high enough calorie density to allow a person to get enough calories in a the volume and weight that a person can eat. Whole fruit has a higher calorie density and can serve as a source of a substantial portion of one's calories, but whole starches (not just potatoes, but also intact grains and legumes) have a higher (but not ridiculous) calorie density and so serves the foundation of calories in both diets. I would love if somebody would take several weeks of menus from the McDougall plan and the Furman plan and break it down to the percentage of calories coming from non-starchy vegetables, fruits, nuts & seeds, and starchy vegetables to really see how much difference there really is. If this has already been done, I would love a link to such a comparison.

papparocket I think the real difference is in the micro not macro nutrient ratio. I would definitely say that anyone who says superior health is at the end of the macro to micro nutrient ratio would be the winner. Greger etc. has also stated the classic "Micrountrient bang" for the "macronutrient buck". I don't think you will find any real truth at the end of macro vs. macro unless we start talking to much protein or fat.
