For bone fracture is surgery the only solution? No. At the same time for some bone fractures, operation is surely needed.
In that case, if you look at the options for operations, it can be either a plate or a nail. You would have heard that they have fixed using a rod or using a plate and screws.
This rod is nothing but a common metal rod as we refer to it. It is made out of titanium. But there are certain rods available in stainless steel. In medical terminology, we call it 'intramedullary nail'. The reason we call it so is that this rod goes into your bone marrow.
It will not be on the surface of a bone like a plate. Wherever there is bone marrow, like arm bone, thigh bone and the leg bone, humerus, femur and tibia. In these three bones, there are high chances of using the rod.
Compared to the plate, when we put a rod, the opening is much smaller. We make openings only on the top and bottom. First, a wire is inserted into the marrow to make the marrow bigger, a technique called reaming is used. After that based on every individual's size of the bone, and based on the length of the bone, nails are available in various combinations.
So we pass the nail inside the marrow, and it is fixed using screws on the top and bottom as shown in this video. When doing this, fracture union is better, it is more physiological. At the same time it is also easier for mobilization.
So, for certain bones why do we use a rod and not a bone? technically, rod has a slightly better advantage. The operated bone can handle the weight when you walk relatively soon or at least your partial weight.
If you get a plate fixed, the chances of exerting weight on the operated bone is less. This is because, nail is a load bearing device and the plate is a load sharing device. A nail definitely has more advantage that way.
Do we have to mandatorily remove the nail? Nails need not be removed always. But at the same time, once the fracture has settled to a certain extent, usually we say 18 months to 2 years, the chances of removing the rod after that duration is high.
If the rod causes a problem, then we can remove it. Based on science and research when using the nail, the healing is relatively faster.
There are two reasons. Firstly, we are not going to open the fracture. There would be a hole on top and a hole at the bottom, inside there would be the rod.
So, the fracture is literally not going to be opened. Technically you are not disturbing the stem cell over the fracture site so healing is much better. Secondly, it is load bearing. So, when you exert weight on it and walk and when the two fractures rub, it throws even more bone. So technically, rod has relatively more advantage than the plate.
But there are also certain restrictions about using the rod. A rod cannot be used in all the bones. Usually when a person comes with a fracture, we put them into two groups.
First group is called the low velocity, that is, it is a fall from a standing height or a slip and fall, and it is just a fracture. This is one group.
Second group, it is high velocity injury like a major road traffic accident or if it's like a mass casualty, then you need not be focused only on this injury. There could be an injury in the brain or the back. There could be an injury in the blunt abdomen or the brain.
So we go through a set of protocols where we do not miss any major injury.
Coming to the fracture per se, if only the fracture is there after examining clinically, we decide to go for an x-ray or CT scan depending upon the nature of the injury. Once we see the X-ray depending on which bone, which part of the bone, we will talk to the patient about the treatment options.
Usually, it is done the very next day, provided the implants are all ready. The surgery will take anywhere between 1 hour to 1.5 hours. and we can make them walk the very next day. Hospital stay is usually only two to three days.

It purely depends on the type of fracture and how early the fracture is fixed. Keep a regular followup with your doctor take periodical xray and if needed MRI to see the status of head

Operation date : 7/7/22, 6 month completed but stil my leg is in pain and difficult to walk, why sir? I can easily walking by 4 stick walker by both hand but difficult to walk in a single stick.
