The correct interpretation is LBB with acute myocardial ischemia and 2:1 AV block. remember that, left bundle branch block can produce secondary STT changes. Thereby, diagnosing acute ongoing ischemia in context of LBBB is difficult and sometimes impossible. Many criteria have been proposed to help clinicians diagnose acute myocardia ischemia in presence of LBBB. Sgarbossa and modified Sgarbossa, and negative QRS T concordance are the most common ones. In the present video we will discuss negative QRS T concordance in details.
