Doctor REACTS to PEEP SHOW | Psychiatrist Analyzes Mania, Bipolar & Sectioning | Doctor Elliott

I love Peep Show. It's british humour at its finest. On this episode of Psychiatry Doctor REACTS, I'm watching the episode called "Sectioned". Surprise, surprise there is a lot of discussion around mental illness, specifically about mania, the treatment of bipolar disorder and if and when we admit someone to hospital against their will. Hopefully you'll enjoy it and you'll learn something.

There is a game called Hellblade: Senua's Sacrifice that I would love to see you react to. It's about Senua, a Celtic warrior back in the viking age. Vikings sacrificed her boyfriend to the gods and she finds him hanging. This triggers a psychotic episode which takes her on a quest to retrieve his soul from the viknig god Hela. The game shows (in a fictional way) what psychosis can be like, such as delusions and hallucinations. They used both mental health professionals and patients to construct the game in an accurate way, as well as historians for historical accuracy. I recommend headphones to get the full experience. I am aware that you can't very well react to a whole 8 hour game, but some highlights and the intro would be nice. As a backstory; her mother had psychotic episodes as well and eventually committed suicide. I would say it is an odd representation of schizophrenia as she has visual hallucinations as well as auditory and some others, but everyone is different I guess. You're the expert though, hence the request. As a psychology enthusiast I really enjoyed this game.

I can relate to a lot of what you're saying. My best mate suffers from bipolar disorder, and there have been many manic episodes in the last 25 years or more. They can be rather harrowing, and obviously affect family, friends and the general public. Last one, he kept returning these pens to a shop because they kept running out from all of the writing. I had a look at some of these notes and letters, and they were over written in real bold writing. If he mentions writing or gets a vintage typewriter out, lookout as this can be a warning sign that he is on the way. However, he is extremely well read, intelligent and perfectly capable of writing a novel, so this makes it difficult to question his actions. What is the normal level? How do I know? Have I ever really known who the real person is and the level of motivation? Certainly the pacing, well picked up. Even on visiting at the psych ward after they've already medicated him to bring him down. These episodes can be so severe with the adrenaline pumping through the body, if left unattended, they could die. This includes endangering oneself, getting into altercations (he once took over a pub as security and helped himself to drinks behind the bar and bashed a patron), making irrational decisions, which includes abusing the medication he is allowed to self regulate. Then add a dash of paranoia. The stories I could tell, I've only scratched the surface and probably blocked out many of the real bad ones from my memory.

This brought back memories, I remember my GP listening to me rambling on and just saying at the end ‘hmmm, you’re speech is quite pressured isn’t it?’ Got put on antipsychotics and referred for a bipolar diagnosis that day. I remember beaming at strangers and telling my mum to turn up the volume on her emotion dial lol. And I got obsessed with the idea of making a fruit curd business and made tons of jars of the stuff! To be fair, I still think mojito curd is a cracking idea and would sell very well at farmer’s markets!

I was prescribed quetiapine off-label for my sleeping problems two years ago. I've had sleeping problems all my life and so far no doctors could find anything physically the matter with me, blood has been fine all my life, sleep phases and brain waves and everything are fine, my sleep hygiene was fine, but then I got diagnosed with ADHD and autism 2.5 years ago. My psychiatrist and I reasoned that the mental unrest could be attributing to having trouble falling asleep and staying asleep, so we hoped that ADHD meds would give me some alleviation, even though they need to be out of your system by the time you fall asleep since they're stimulants. But unfortunately it didn't do much for my sleep. Since we knew that my sleeping problems were chronic and not due to short-term stress or specific life events or something the psychiatrist didn't want to prescribe me benzodiazepines because it didn't look like it would be 'fixed' any time soon. I read about people getting quetiapine off-label for sleeping issues and I suggested it to the psychiatrist, who thought it was a good idea, and I've been using it ever since. I prefer not being on too many meds but my heavens, quetiapine has been an absolute lifesaver for me. Just like you said in the video, it's very sedating. It has some unpleasant side effects like night sweats, increased nightmares, weight gain and increased appetite (though both of those are mitigated by the ADHD meds during the day) but all of are so much preferable over never being able to fall asleep within an hour, let alone when you're not sleeping at home. The longer you can't fall asleep, the more you start to worry whether you'll be able to get much sleep, etc etc etc. But with quetiapine I just fall asleep so fast that I don't even remember getting into bed. Unfortunately it didn't make my sleep more restful than it was, but at least I'm able to sleep more.

Silly Shrinkie. In American psychiatry, restraints are used liberally and frequently - including as punishment and for the convenience of staff. Lying on the patient’s chart is no biggie. Oh, and lawsuits against mental health professionals are extremely rare - and quite unlikely to end in an adverse judgment, considering the backgrounds of most psychiatric patients.
