So what if you have sloughin’ of the stuffin’ in your uterus that constitutes bleeding when you shouldn’t bleed? What’s next? What constitutes an evaluation that can give you an actual diagnosis for the cause of your bleeding? That’s what I’ll explain in this video.

Hi Dr. Taylor! I am 64 years old. I am still learning so much from watching your videos. I have been menopausal for 13 years now. From watching your videos, I have learned to listen to your body, and if something is off, take care of it! for example, I was experiencing some vaginal bleeding about five months ago. The minute I saw this I called my doctor right away. first thing I had done, was to get an ultrasound, than a hysterosopy , pap smear, and finally a D&C all came back negative. I was so relieved! I am now taking a lower dose of estrogen, but I still worry about not getting enough. I am going to go back on your videos and refresh my memory I want to thank you!🙏 so much for all you do!!❤️.

I'm in a slightly different situation. First, I take both estrogen and progesterone. I experienced the "bleeding when you're not supposed to bleed" scenario, they did the ultrasound and found fibroids, then decided that was the source of the bleeding. That was about a year ago. Then I stopped bleeding all together and the following happened: routine PAP smear found endometrial cells in my cervix, vaginal ultrasound found my lining had thickened (6mm) beyond what would be considered normal (5mm according to my doctor), nurse practitioner and OBGYN both tried to sample cells from my cervix but could not get me dilated enough to get deep enough, I am now scheduled to have a D&C this Thursday to get a good sample. I'm glad to see that they are following the path that you have laid out, though I wonder if I had a D&C a year ago if they would have found something then. . .

I am about a year past menopause (it’s been about two or two and a half years since I had a period). I started continuous HRT approx 3.5 to 4 months ago (Estradiol 0.05% transdermal patch, along with 100 mg progesterone pill at night). Up until two weeks ago, I had no issues. Then I began spotting. Spotted for two days, before bleeding increased to a continuous drip. It was as much blood flow as a regular period. I was able to get into my OBGYN. She said the amount of blood means nothing - that a smidge of blood vs a bucket of blood is “irrelevant” because “blood is blood”. She assured me that I wouldn’t need a biopsy, and ordered an ultrasound. She was not concerned about the pelvic cramping/pain I experienced while bleeding, or the “butt pain” I had the night before the spotting had started (felt like someone was attempting to drive a plunger up my arse). I explained that I used to occasionally get bad butt pain when I had my period, and that the butt pain I had that night was identical (even though at that point, I had not yet even started spotting). She dismissed this, advising that “lots of women” have this kind of pain when on their period. I’m not on my period. After four days of bleeding, it ceased altogether. The day after the bleeding stopped, I had the transabdominal and transvaginal ultrasound . My left ovary could not be visualized, which the tech speculated was gas in my bowel. That area just looked black on the sonogram. My endometrial stripe measured 10 mm. .I reached out to my doctor, as I had not heard back. My doctor called today, advising that it’s quite “normal” to have a thickened endometrial lining when on HRT. She said that while she would have liked to see it at 2mm, it’s not uncommon for someone on HRT to have a measurement as high as 20 mm! (Yes, 20 mm!) - and yet still be fine (no cancer). She told me to take two 100mg progesterone pills for the next week (rather than one) - at which time I could go back to 100mg. She still does not feel that a biopsy is necessary. She suggested we cancel the follow-up appointment we had for Monday, and meet in a month. She’s REALLY nice and is a Castle Connolly “Top Doc”. She’s got a fantastic bedside manner. That said, I have been unable to get a good night’s sleep since this started. I am scared. From your videos, I expected a biopsy. Is it “normal” to have a 10 mm endometrial stripe, especially after just starting HRT a relatively short time ago?

@Teiteiboo Hello, and thank you for asking how it turned out. I ended up reaching out to Memorial Sloan Kettering, a cancer research hospital on the East Coast. It just caused me too much anxiety, and too many nights sleep. The surgeon I met with explained that performing a biopsy was a no-brainer, given the ultrasound and the symptoms I was experiencing, along with my recent bloodwork. She performed a biopsy on my endometrial lining, as well as a vulvar biopsy. Thankfully, cancer was not detected! Polyps were found and removed. The vulvar biopsy was to confirm a Lichen Sclerosis diagnose I had received a year prior, for which the same OBGYN advised against biopsy. At the time, I was prescribed topical Clebatasol to use everyday, for the rest of my life. I have CD8 cell insufficiency, and I had read that the continued use of Clebatasol was not advisable where a CD8 cell abnormality is concerned. Because of my CD8 insufficiency, and a strong history of both breast and brain cancer in my family, I have a much bigger fear of cancer than I do of Lichen Sclerosis, so I discontinued the use of the ointment once the rash cleared up. When I went to Sloan for the issue with the thickened lining, the surgeon suggested a vulvar biopsy as well. I was so relieved! As with the vaginal bleeding, I was shocked when, the year prior, my OBGYN didn’t think a biopsy was necessary. The only thing I can think of is that she might have been concerned the biopsies would be triggering. I don’t know, but she is such a lovely woman that she must have been confident without performing the biopsy(ies) - and might have thought that I would feel more comfortable not doing them. Even though I expressed wanting biopsies, she assured me that I “would hate her” if she did them on me. My experience at Sloan, particularly on the day of the biopsies, was wonderful. Everyone I met with, including the anesthesiologist, was very personable. Recovery was straightforward and followed the course I was advised it would. Oh! Another great thing about the surgeon was that I didn’t even have to ask for an IUD. I had been interested in an alternative to oral progesterone ever since learning about the benefits from watching Menopause Taylor’s invaluable channel. Again, my own OBGYN refused, explaining that the insertion process was “painful” and that, additionally, I would “feel it” once it was inside. Well, wouldn’t ya know I don’t feel it inside me at all! The insertion was absolutely painless being that I was already under anesthesia. Any pain I had after the procedures was likely the result of the vulvar biopsy (hard for me to differentiate). The result of the vulvar biopsy came back inconclusive for Lichen Sclerosis, siting “radioactive inflammatory changes”. My surgeon suggested continuing to hold off on the use of Clebatasol, until and unless I had a “flare”. It’s been a year since I was given the LS diagnosis by my OBGYN. No flares yet. Fingers crossed. I’m giving a lot of information, hoping it might help you and others. Not going somewhere for a second opinion, while there was still a rash to biopsy, was a mistake on my part. I found this channel after that happened. If I had found Menopause Taylor before or during, I would have felt more empowered to advocate for myself. Thankfully, I DID find this channel in time to advocate for myself when experiencing the bleeding! Lesson I learned was not to participate in a game of Russian Roulette where matters of health are concerned. Get (insist on) a biopsy. PS: My feelings regarding the OBGYN, who I have not seen since, haven’t changed. The best bedside manner I’ve encountered, just perhaps not an ideal fit for someone like me, who worries a lot and who appreciates a more conclusive diagnosis. PSS: Typing on my phone. Sorry for all the typos/edits!

Ready for class Dr Taylor! Your detail in this video is excellent! You go into very fine details with the procedures. It's long, but your breaking it down to layman terms is helpful... I never had any tests, nope...Amazed me how the radiologist missed reading the ultrasound. Perhaps my doc saw more on the ultrasound than the radiologist. My She Shed was ripped out, along with egg crates. The pathologist found the problem my doc may have suspected. Still cancer free two years later... Hmm, does next week's lesson include more avocados as props?

45 and still get a monthly cycle but my estrogen levels vacillate erratically up and down, not just around ovulation. I tracked it with a very sophisticated fertility app just to see what my hormones are doing. If I start a small daily dose of bio identical estrogen cream and use it during just the first two weeks of my cycle, will that balance my estrogen levels and even them out to help with weight loss or will adding more estrogen cause weight gain?

If the positive rate is only 9%, what are the possible causes for the 91%? Can HRT simply cause benign bleeding? I began spotting about 4 months after starting HRT and have been spotting for about 18 months. I’ve had ultrasounds and my lining is about 5-6mm so I know I need a biopsy but I’ve been hesitant due to sexual trauma history.

Dr Taylor I would like to thank you so very much for your program I as a 👠 woman found myself having from light pink to red bleeding not heavy bleeding but blood is blood I do have about 6 fibroids that are still there and the other ones dissipated over the years I was taking a woman's hormonal pill 💊 that causes some women to stop bleeding and for other women it does not help 🆘 at all for me I truly believe that this pill was helping to keep bleeding 👇👇👇 down and just when I did not see blood then I found myself cramping just light one does prior to a woman having her period I actually went to the emergency room and I was totally surprised that they did not call in an OBGYN doctor to take a look at me and this situation. I was there all night long 🥀 on what I called a dry run for nothing. I hit actually contacted my OBGYN doctor and that earlier today. When he came in the room with the nurse he was actually listening to what I had to say and the next thing I knew he was actually saying that he wanted to do a biopsy and an ultrasound ASAP and he also said that what I was describing sounded like maybe 🤔 cancer. The next thing he said to me was that he wasn't going to play around with this and that he wasn't even going to wait for a medical clearance that people usually have before they go through any type of procedure when they're being put to sleep 😴 so he said that this was going to be marked urgent urgent so this could be done ASAP he said that he wanted me to call and have the ultrasound scheduled and meanwhile I would be receiving a call from the scheduling department 🏬🏬 for when the actual biopsy was going to be scheduled for putting me to sleep. The only thing about this was that I did not understand why he actually did not check me physically in that area it's like he had heard enough and he just wanted to get down to business as a woman I really don't know how I'm supposed to be feeling because that actual exam I'm wondering why did he not do that. And not giving a medical approval working the case urgent urgent can the doctor actually do that is that legal. Because again I was always told that whenever any procedure is performed on you where you need to be put a sleep you must have a medical clearance first any other hand I can say at least he was jumping on this ASAP now I have actually had breakthrough bleeding before and by doing a simple test such as taking of small piece 🧩 around the vagina area it was sent to the lab because of bleeding but thank God it came back before as having no cancer however that was then which was a year ago and this is now. He did say that his reason for jumping on this so quickly is because he feels like there might be cancer there and he doesn't want to play around keeping in mind I've had a breakthrough a bleeding before but it was not cancer I am trying to be very calm after my seeing the doctor 💊 and I just hope that he is doing the right thing buying me by not giving the medical clearance to do 🥺 this however please keep in mind he saying that as far as he's concerned this is an emergency situation that needs to be addressed now so dr. Taylor if I could please give a feedback from you because you sound so very very inspirational as a doctor and I have really truly enjoyed watching and listening to your program may God bless you reaching out to women who are into me in the dark 🌑🕶️ about these type of medical issues. I was told that the procedure that he will be doing on me at the hospital you go in that morning 🌅 and you go home later on that afternoon am I scared 😨 yes a little because everything is happening so fast ⏩ but at least he's trying to work on this ASAP opposed to going to the emergency room being there all night long 🥀 and nothing was done. Again I do thank you so very much and whatever words of wisdom that you might have for me would be greatly appreciated 🥀 again dr. I thank you so very much. ♥️🥀♥️
