This Stanford Medicine 25 video was created in conjunction with Stanford's AIM lab teaching the examination of the deep tendon reflexes.
The Stanford Medicine 25 is a Stanford School of Medicine initiative to teach and promote the bedside physical exam. Here you will find videos teaching bedside physical exam techniques.

I've watched a few of the Stanford videos and this doctor seems like he'd be such an amazing teacher. I'm not even interested in medicine and I'd want to take his class.

I've learned more from him than my actual professors in PA school. Highly recommended and seeing the difference from a real ranked school vs 150th.
Thanks for the great demonstration as always! In case you need a quick reference for a refresher: 1:00 & 3:00 Brachioradialis reflex (C5/C6) 1:12 & 3:15 Biceps reflex (C5/C6) 1:40 & 3:35 Triceps reflex (C6/7/8) (1st with zoom-in views on reflex responses, 2nd on muscle contractions) 2:30 Finger flexion reflex 4:00 Ankle reflex (S1) 4:25 Knee reflex 5:12 Ankle reflex in a recumbent position (S1)

1.) The Doctor's name is Dr Abraham Verghese. He's a General Internist who works at Stanford. He also has a fabulous TED talk on the value of the physical exam. I highly recommend it. 2.) Re: the unprovoked reflex at 4:27 in this video, the gentleman being examined is still a "simulated patient", which means he's been hired to play the part. Consequently, there will be acting. But whether the hammer hit his patellar tendon or not, he's still performing the exam correctly - swinging the hammer correctly, and aiming it at the right spot.

No need 2 watch. Doctors haven't done physical exams in 20 years. Its 3-5 min and your out of there. Its about patient volume now. If I watch his TED talk it will only exaggerate what is missing.

How do I mean, what? The "people-pleaser" bit? Because it looks like he made a reflex action without any contact. Doing it for appearances and so the doctor "doesn't get upset." People-pleaser.

Can someone explain why the brachioradialis test seems to evoke motion at the wrist rather than the elbow, when brachioradialis is an elbow flexor and does not cross the wrist...?

If finger flexion reflex is present only on one side , what does it indicate ?

What is the difference between tapping directy on the tendon and tapping over the examiner's thumb?
@jakeabc123
Жыл бұрын
Tapping over the thumb helps you feel for contraction in case the reflex is diminished or trace and you can’t visibly see it well.
I get tingles too!! :)
Good

4:27 Is it my impression or the patient delivered a contraction without actually being hit by the hammer?

Yep, it looked awkward so I had to slow down the video to see. Seems like his patient was instructed to fake all the reflexes. Fail communication between the two.
@rodrigoferreira1175
7 жыл бұрын
Yes, I think that might have been the case. Even though these contractions are primarily involuntary, they provoke some inner instinct that makes us uncomfortable, and we don't want to feel that again. After all, our reflexes exist to protect us, and nobody gets protected if no harm is detected. So the reflexes might automate without physical, but emotional (fear) stimuli.

One thing of note. A doctor must be proficient in being able to elicit reflexes. It is never appropriate to say I'm no good at it, I just can't get them, and so forth. A doctor must be able to see the reflexes in all patients other than those who simply do not have those reflexes for one reason or another.
@Kim-kd8or

I agree. I had a Doctor do that with me on two different occations when i had a stroke this year at 32 saying oh i am not good at the reflexes or i do not have a hammer i am not good at them anyway i can just slap and see if we get anything!! With a stroke !! In neurology ! Can you imagine ! I am a senior nurse and perform them better ! I am still shocked about this.
didn't quite get the finger flexion reflex 😐
how do you know the movement itself is not coming from the strike and not a reflex? its clear on some its a reflex but on others the reflex looks like it was the movement caused by the strike and not a reflex??
